Morphometric--kinetic parameters of the thyroid gland in the Kashmiri population, which is geographically, environmentally, socioculturally, anthropologically and ethnically different from that of the main country (Indian republic), have been analysed. In an attempt to evaluate penile corporal venous outflow, a method that utilizes intracorporal injection of Tc-RBC was developed and used in 20 patients with erectile dysfunction. In the diagnosis of scaphoid fracture, the dynamic phase of the radionuclide bone scan alone has been recommended as an early test. To evaluate the independent reliability of the dynamic and static phases of the 3-phase bone scan in this diagnosis, 3 examiners reviewed the 3-phase bone scans of a series of 60 patients with clinical signs of fracture of the carpal scaphoid and with negative or non-diagnostic initial radiographs. The Authors report a series of seven obese women suffering from Benign Intracranial Hypertension (BIH), all submitted to radioisotopic cisternography. Cisternographic findings showed altered patterns of CSF clearance in all but one patient, suggesting that an increased arachnoid resistance to CSF efflux might be present in BIH of obese women. The pathophysiology of BIH is discussed: different pathogenetic mechanisms may contribute to its development with a relative importance depending on the specific etiology of the syndrome.

A great majority of mediastinal lymph node metastases originate from primary neoplasms within the thorax. Mediastinal metastases from other malignancies are not infrequently encountered but are uncommon from a prostatic primary site. Since prostatic metastases may mimic lymphomatous nodes, it is important to be aware that mediastinal adenopathy occurring in a patient with prostatic carcinoma may be caused by metastases from that site rather than from lymphoma, or be secondary to some unknown primary malignancy elsewhere in the body. The cerebral perfusion was evaluated in 8 patients with stroke, caused by stenosis or occlusion of the carotid artery, and in 4 neurologically healthy subjects, using Tc99m labeled microspheres (LMS). By measuring activity in the brain and lungs after intracarotid injection of LMS, the existence of brain shunts in patients with stroke was demonstrated. It seems that brain ischaemia triggers the opening of shunts which appear to be one of the important autoregulatory and protective mechanisms to cerebral haemodynamic changes after brain infarction.

A series of 99mTc-labeled red blood cell (RBC) scintigrams in a patient with a past history of radiation proctitis demonstrated incidental findings of gallbladder activity, probably related to multiple blood transfusions and renal failure. Variable activity in an ileal-loop urinary diversion simulating bowel pooling was also noted. Thus, difficulties encountered in scan interpretation are presented.

Quantification of left to right shunt was carried out in 15 adult patients, mean age 49 years, with a suspected ostium secundum atrial septal defect (ASD II). Radionuclide shunt quantitation correlated well with the results of right heart catheterization (r = 0.85). The radionuclide technique failed in two patients for technical reasons, but revealed no false negative or false positive results when technically satisfactory. The diagnosis was confirmed at operation. It is concluded that the radionuclide technique is a useful and reliable method which can also be used at follow-up after surgery

A neonate with infantile polycystic kidney disease underwent Tc-99m DMSA imaging. The pattern of uptake in infantile polycystic disease is different from the multiple cystic lesions reported in the literature for adult polycystic kidney disease. The infantile pattern of uptake shows large kidneys with diffuse, symmetric localization of the radiopharmaceutical, which seems to be characteristic and may be pathognomonic of the disease process.

In 24 patients affected with post-traumatic reflex sympathetic dystrophy syndrome (RSDS) with upper limb involvement following humeral fractures, bone mineral density (BMD, mg/cm2) was measured by means of dual-photon absorptiometry in the distal radius of both the affected and the normal contralateral limbs. A 45-year-old female patient with rheumatoid arthritis and Sjögren's syndrome was admitted because of progressive exertional dyspnea and palpitation. The chest X-ray film was normal except for slight overinflation. Pulmonary function tests indicated severe mixed-type impairment, and arterial blood gas analysis revealed hypoxemia. 99mTc-MAA lung perfusion scintigraphy and 81mKr lung ventilation scintigraphy showed multiple nonsegmental defects in both lungs. During six months of observation, the perfusion defect increased in size and intensity, although the chest radiography remained unchanged.

The reticuloendothelial system of the liver was studied in 27 patients with nephrotuberculosis by means of radionuclide hepatography with colloid solution of Au-198. It was found that all these patients showed a reduction of the activity of stellate reticuloendotheliocytes of the liver. These conclusions were made on the basis of distinctly changed values of the period of half-purification of the blood, constants of blood purification, coefficient of liver retention, etc. There was also a distinct reduction of fractional blood flow of the liver.

Dexamethasone-suppression (DS) adrenal scintigraphy localizes an aldosteronoma, but with false-negative results, i.e. 2 of 19 cases in our study. Our aim was to clarify the clinical meaningfulness of this test.
