Torn human rotator cuff tendons have reduced collagen thermal properties on differential scanning calorimetry.

The cause of the high failure rates often observed following rotator cuff tendon repairs, particularly massive tears, is not fully understood. Collagen structural changes have been shown to alter tendon thermal and mechanical properties. This study aimed to form a quantitative rather than qualitative assessment, of whether differences in collagen structure and integrity existed between small biopsies of normal, small, and massive rotator cuff tears using differential scanning calorimetry. Thermal properties were measured for 28 human biopsies taken intra-operatively from normal, small, and mas

[Neonatal traumatic epiphyseolysis of the humeral head. A rare occurrence].

Neonatal traumatic epiphyseolysis of the humeral head is rare, and only a few cases are reported in the literature. We present a case of a 13-day-old female newborn with malposition and relaxation of the left upper limb. The clinical examination showed distinct range of motion particularly for abduction. Magnetic resonance imaging (MRI) indicated epiphyseolysis of the humeral head. Closed reposition followed by immobilisation was done. The following MRI showed correct axis with adaption of the humeral head. Later, malposition of the axis with angulation in the ventromedial position was seen. T

[How to treat massive rotator cuff tears].

The treatment of massive rotator cuff tears must be adapted to the patient's individual needs and preoperative parameters to achieve the best outcome. First, the shoulder surgeon has to determine whether a direct transosseous repair is possible. If there is not enough remaining tissue, the tissue is atrophic, and the tendon stump can be reduced only with great tension, one can use a margin convergence technique for partial closure, perform a biceps tendoplasty, or perform local tendon transfers with the subscapularis or infraspinatus muscle. If the defect cannot be sufficiently closed, elderly

Outcome of rotator cuff repair.

From 710 consecutive open rotator cuff repairs by a single surgeon, the results of 667 were available for detailed analysis. Patient-assessed outcomes and the ability to perform specific activities of daily living, employment, and recreation were correlated with independent nonstructural variables including age, sex, workers' compensation status, and revision surgery status. The study shows that patient self-assessment of satisfaction is very high, with 87.5% of all respondents pleased overall. Detailed assessment is provided of certain subgroups that are more likely to report worse results af

An epidemiological study of high school ice hockey injuries.

Between 1976 and 1983, 42 spinal cord injuries among ice hockey participants were documented in Canada; within a recent 3-week period, three cases involving fractured necks were reported in a metropolitan area of Minnesota. The purpose of this comprehensive epidemiological study was to document rates, types, and severities of injuries incurred among high school varsity hockey players in Minnesota, as a result of participation in the 1982-1983 season, and to determine the variables associated with these injuries. Within a 30-mile radius of the state's major metropolitan area, 12 high school var

A systematic review of brachial plexus injuries after caesarean birth: challenging delivery?

BACKGROUND: Caesarean section (CS) is widely perceived as protective against obstetric brachial plexus injury (BPI), but few studies acknowledge the factors associated with such injury. The objectives of this study were therefore to aggregate cases of BPI after CS, and to illuminate risk factors for BPI. METHODS: Pubmed Central, EMBASE and MEDLINE databases were searched using free text: ("brachial plexus injury" or "brachial plexus injuries" or "brachial plexus palsy" or "brachial plexus palsies" or "Erb's palsy" or "Erb's palsies" or "brachial plexus birth injury" or "brachial plexus birth

Effectiveness of the extended surgical approach to visualize the axillary nerve in the blind zone in an arthroscopic axillary nerve injury model.

INTRODUCTION: The aims of this study were to create a model of axillary nerve (AN) injury during an arthroscopic capsular plication to analyze the site for potential nerve injury and to determine the AN length that can be visualized through standard and extended anterior, axillary, and posterior approaches. MATERIAL AND METHODS: Six arthroscopic inferior capsular plications were performed in 3 human adult frozen cadavers. A nonabsorbable suture was used to plicate the inferior capsule aiming at capturing the AN (at a location closest to the joint capsule). We then attempted to explore the AN

Ergonomics in industrialized dairy operations.

This paper presents a summary of a panel presentation by agriculture health and safety scientists on ergonomics of industrialized dairy parlor operations in the United States. Dairy industry trends in the United States were discussed in the panel presentation, which took place during the New Paths: Health and Safety in Western Agriculture conference, November 11-13, 2008. Dairy production is steadily moving to large-herd operations because of associated economies of scale and other economic and social conditions. Large-herd operations utilize a parlor milking system, as compared to a stanchion

The undiagnosed Essex-Lopresti injury.

The Essex-Lopresti injury is rare. It consists of fracture of the head of the radius, rupture of the interosseous membrane and disruption of the distal radioulnar joint. The injury is often missed because attention is directed towards the fracture of the head of the radius. We present a series of 12 patients with a mean age of 44.9 years (26 to 54), 11 of whom were treated surgically at a mean of 4.6 months (1 to 16) after injury and the other after 18 years. They were followed up for a mean of 29.2 months (2 to 69). Ten patients had additional injuries to the forearm or wrist, which made diag

Surgery should not hurt: A look into the ergonomics of orthopedics.

Orthopedic surgery is among the most physically demanding medical specialties, exposing surgeons to prolonged static postures, repetitive high-force movements, and heavy equipment that significantly increase the risk of musculoskeletal injury. Historical perspectives reveal that ergonomic principles have been recognized since antiquity, yet modern surgical practice continues to neglect them, resulting in high rates of pain, disability, and even premature retirement among orthopedic surgeons. Recent survey data demonstrate that musculoskeletal disorders affect surgeons across all subspecialties

Pneumorrhachis Following Neck Penetrating Injury: A Case Report.

The development of air in the spinal canal is an uncommon and usually asymptomatic event. Also known as pneumorrhachis (PNR), the main information about this phenomenon is based on a few case reports published previously. It is highly difficult to identify this entity clinically, and in most publications, PNR was incidentally identified during image procedures, mainly computed tomography (CT) scans. With the advancement of technology and the development of guidelines for the treatment of penetrating and neck injuries, the number of PNR diagnosis has increased. It is also a common agreement amo

Less common upper limb mononeuropathies.

This article will focus on the less commonly injured nerves of the upper extremity. These nerves may be involved when trauma results in fractures, dislocations, or swelling with resultant nerve compression. Tumors and ganglions can also compress nerves, causing pain and, over time, demyelination or axon degeneration with weakness. Other mechanisms for upper limb nerve injury include participation in high-level sports, that is, those that generate torque about the arm and shoulder, abnormal stresses about the joints and muscles, or muscle hypertrophy, which may result in nerve injury. The goals

Fracture dislocation of the glenoid fossa with open physis: a case report.

BACKGROUND: Scapular fractures are uncommon injuries that account for 1% of all fractures. Ten percent of these lesions involve the glenoid fossa and intraarticular displacement is rare, not exceeding 10% of the cases. The classification of intraarticular glenoid fractures was described by Ideberg on the basis of his findings in 100 patients with this type of injury. His classification, however, does not include a dislocation of the glenoid fossa without displacement of the humeral head. METHODS: We report the case of a fracture dislocation of the glenoid fossa without displacement of the hum

[High vacuum extraction: justifiably obsolete in view of the risks to the infant].

The author was asked to provide an expert assessment of the case histories of 28 patients delivered by vacuum extraction. At the start of the extraction the foetal head level was at station 0 or above in 25 of the patients. In 12 cases the duration of extraction exceeded 14 min and in 7 of the cases it exceeded 19 minutes with a maximum of 45 minutes. Nine of the infants died, 11 suffered cerebral damage, 4 had brachial plexus injury and 1 had both cerebral damage and plexus injury. In 17 cases a causal relation between the unfavourable outcome and the vacuum extraction seemed plausible. Durin

Characterization of Concussive Events in Professional American Football Using Videogrammetry.

Sports concussions offer a unique opportunity to study head kinematics associated with mild traumatic brain injury. In this study, a model-based image matching (MBIM) approach was employed to analyze video footage of 57 concussions which occurred in National Football League (NFL) games. By utilizing at least two camera views, higher frame rate footage (> 60 images s-1), and laser scans of the field and helmets involved in each case, it was possible to calculate the change in velocity of the helmet during impact in six degrees of freedom. The average impact velocity for these concussive events

Stress fracture of the midshaft clavicle associated with sternocostoclavicular hyperostosis-Case report.

INTRODUCTION: Stress fracture is generally a result of cumulative and repetitive stress in athletes, which accelerates the normal remodeling process of bones, and the most frequently involved areas are the tibia and metatarsal bones. Therefore, stress fractures of the midshaft of the clavicle are very rare. PRESENTATION OF CASE: A 58-year-old female was admitted to our hospital because of pain in the middle of the right clavicle. Based on laboratory and radiographic inspection, it was concluded that the stress fracture of the midshaft of the clavicle in this case was caused by sternocostoclav

Successful repair of thoracic outlet syndrome in a growing young patient due to posterior sternoclavicular joint dislocation.

Posterior sternoclavicular dislocation is an extremely rare injury, usually related to heavy trauma, such as a traffic accident. The anatomical proximity of vital mediastinal structures often discourages a closed reduction during emergency situations. In this case report, we present a 17-year-old male patient who was admitted to our emergency department after having fallen down a flight of stairs five days previously. He was eventually found to have thoracic outlet syndrome. Chest computed tomography (CT) and shoulder magnetic resonance imaging (MRI) showed a posterior dislocation of the right
