STUDY OBJECTIVES: Multiple system atrophy (MSA) is a rare neurodegenerative disorder characterized by autonomic dysfunction associated with a combination of cerebellar, parkinsonian, or pyramidal signs. Sleep-disordered breathing such as stridor, obstructive sleep apnea, and central sleep apnea is common in MSA and can impact survival. Several studies have evaluated treatment modalities. However, the optimal strategy often remains unclear in these patients. This review aims to provide an overview of the current evidence on treatment of sleep-disordered breathing in MSA.

The relationship between asthma and sleep-disordered breathing is bidirectional due to common risk factors that promote airway inflammation. Obstructive sleep-disordered breathing and recurrent wheeze/asthma are conditions that involve the upper and the lower respiratory system, respectively. The aim of the present study was to investigate the sleep disordered breathing in children with recurrent wheeze/asthma. This was a retrospective study concerning children older than 2 years who underwent-between January 2014 and November 2016-an in-laboratory overnight polygraphic study. We match the chi

INTRODUCTION: Obstructive sleep apnea (OSA) is a highly prevalent sleep-disordered breathing (SDB).
CASE REPORT: Two 53- and 51-year-old male cases with daytime sleepiness and opium abuse and severe sleep apnea and long respiratory events duration (200 and 275 seconds respectively) noted in polysomnography were reported at Ebn-e-Sina and Razavi hospitals, in Mashhad, Iran. After positive airway pressure (PAP) therapy respiratory events resolved and patients' daytime alertness improved.
CONCLUSION: The long duration of sleep apnea could be the result of opium abuse. Therefore, drug history sh

Adults with obstructive sleep apnea-hypopnea syndrome typically have a history of obesity, snoring, and prominent daytime somnolence. However, children with this condition are more likely to have normal body weight, adenoid and tonsillar hypertrophy, and inattentiveness during school classes. The symptoms of children with sleep apnea-hypopnea syndrome can be divided into those of sleep-disordered breathing -such as snoring-, symptoms of sleep quality -such as enuresis-, and symptoms of behavior or learning disorders -such as hyperactivity.

Most patients with Apert syndrome experience some degree of upper airway obstruction during the first few years of life. The pathophysiology of this form of obstruction and the clinical features are covered in detail in this article. Sleep apnea syndrome is seen commonly in association with Apert syndrome. The investigation of sleep apnea and various therapeutic modalities used to relieve the airway obstruction are presented. In addition, the otologic aspects of Apert syndrome are discussed.

About 40% of adults are habitual snorers. 2% of women and 4% of men between 30 and 60 years of age present additional sleep disordered breathing. There is a continuous spectrum from simple to disease-causing snoring ranging from merely disturbing noises to pharyngeal obstructions and breathing pauses. Repetitive episodes of upper airway obstructions lead to frequent arousals from sleep. Sleep fragmentation is responsible for non-refreshing sleep and subsequent excessive daytime sleepiness resulting in reduced performance at work, social problems and a higher risk for accidents. Furthermore the

UNLABELLED: Therapeutic decisions in patients with sleep apnea (eg, adjustment of continuous positive airway pressure [CPAP]) depend on differentiating central from obstructive apnea. Obstructive apnea is defined by cessation of airflow in the presence of continued respiratory effort, which is conventionally inferred from chest wall movement or intrathoracic pressure swings. Cardiogenic oscillations in the airflow have been observed during some central apneas, but there is controversy over whether they correlate with airway patency. The present study investigates whether these oscillations are

PURPOSE OF REVIEW: Obstructive sleep apnea is prevalent in children with facial clefts. As there are increasing concerns that sleep disordered breathing and obstructive sleep apnea may lead to cognitive difficulties, it is imperative that the otolaryngologist and cleft surgeon be aware of the concerns for sleep disorders and implement appropriate interventions for the management.
RECENT FINDINGS: Micrognathia associated with Robin Sequence has long been understood to have significant potential for sleep apnea. Positioning, nasopharyngeal airway, tongue-lip adhesion and mandibular distraction

The authors studied the direct and integrated EMG activities of the cricothyroid (CT) and chin muscles in 7 patients with the sleep apnea syndrome. They noted: (1) A tonic activity with phasic inspiratory reinforcements in the CT during wakefulness. (2) A decrease in the tonic activity without any modification of the phasic inspiratory reinforcement during sleep. (3) A decrease or even disappearance of the phasic activity during sleep-induced hypopneas. (4) A complete cessation of both the tonic and the phasic activities of the CT during obstruction apneas: resumption of respiration is marked

UNLABELLED: A 71-year-old male with severe obstructive sleep apnea and nasal septal deviation presented to a positive airway pressure (PAP) alternatives clinic due to persistent obstructive events on both continuous PAP and bilevel PAP therapy delivered via oronasal mask. He underwent drug-induced sleep endoscopy with PAP titration to determine the mechanism of oronasal mask failure. A nasal mask was also applied and titrated for comparison. Drug-induced sleep endoscopy with PAP showed tongue base collapse which resolved at low pressure using a nasal mask. Application of an oronasal mask incre

Sleep apnea is a rare but well-known clinical feature of disorders of the craniocervical junction. It may be obstructive or central in nature, and rarely presents without other neurological symptoms. We report the cases of two children, presenting with isolated central sleep apnea leading to a diagnosis of type I Chiari malformation. Surgical treatment resulted in a dramatic improvement in respiratory parameters during sleep, both clinically and on polysomnography. We discuss this uncommon presentation of type I Chiari malformation and suggest that it be considered in the differential diagnosi

Driving an automobile while sleepy increases the risk of crash-related injury and death. Neurologists see patients with sleepiness due to obstructive sleep apnea, narcolepsy, and a wide variety of neurologic disorders. When addressing fitness to drive, the physician must weigh patient and societal health risks and regional legal mandates. The Driver Fitness Medical Guidelines published by the National Highway Traffic Safety Administration (NHTSA) and the American Association of Motor Vehicle Administrators (AAMVA) provide assistance to clinicians. Drivers with obstructive sleep apnea may conti

Sleep disturbances in chronic peritoneal dialysis (PD) patients are increasingly widely recognized. These include changes in sleep architecture, sleep apnea syndrome, restless legs syndrome, periodic limb movement disorder and excessive daytime sleepiness. Some of these entities will be addressed in this review. The study tools for detecting sleep-related disorders in PD patients range from self-reported questionnaires (subjective) to standard overnight polysomnographic measurements (objective). Both these tools identify a high prevalence (over 50%) of sleep-related disorders among dialysis pa

Wolfram syndrome (WS) is a rare autosomal-recessive genetic disorder. The authors report a case of a patient with WS and undiagnosed/untreated obstructive sleep apnea (OSA) associated with prolonged periods of apnea and hypopnea and nocturnal hypoxemia, which may have predisposed him to the development of a near-fatal event during sleep. Addressing sleep-disordered breathing in patients with WS could improve their quality of life and potentially their longevity.

PURPOSE OF REVIEW: This article provides a framework for the clinical assessment of patients with sleep-related complaints and outlines a systematic approach to a sleep-specific history and physical examination, subjective assessment tools, and diagnostic testing modalities.
RECENT FINDINGS: Physical examination findings may suggest the presence of a sleep disorder, and obstructive sleep apnea in particular, but the clinical history remains the most important element of the assessment for most sleep problems. While nocturnal polysomnography in a sleep laboratory remains the gold standard for

Sleep-related rhythmic movement disorder is characterised by stereotyped and repetitive rhythmic movements involving large muscle groups during sleep with frequencies between 0.5 and 2 Hz. Most of the published studies on sleep-related rhythmic movement disorder have focussed on children. Therefore, we performed a systematic review on this topic focussing on the adult population. The review is followed by a case report. The review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2020 guidelines. A total of seven manuscripts (n = 32 individ
