Aging and sequelae of poliomyelitis.

OBJECTIVE: We estimate that there are about 50,000 persons who survived poliomyelitis in their childhood in France (mean age estimated between 50 and 65 years). After a few decades of stability, 30 to 65% of individuals who had been infected and recovered from polio begin to experience new signs and symptoms. METHOD: Review of the literature on Pubmed with the following keywords "Poliomyelitis" and "Post-Polio Syndrome (PPS)". RESULTS: These new signs and symptoms are characterized by muscular atrophy (decreased muscle mass), muscle weakness and fatigue, muscle and/or joint pain. All these s

The prevalence of nocturnal hypoxemia in advanced cancer.

Nocturnal hypoxemia is associated with excessive daytime sleepiness in patients with chronic respiratory disease. This relationship has not been explored in patients with cancer. This study examined the prevalence of nocturnal hypoxemia in patients admitted to a specialist palliative care unit, and explored relationships with demographic and physiological parameters, opioid or other sedative drug use, and daytime sleepiness, fatigue, and quality of life. Demographic details, diagnosis, performance status, body mass index, opioid or other sedative drug use, hemoglobin, spirometry, and sniff nas

Sleep characteristics of youth with functional abdominal pain and a healthy comparison group.

OBJECTIVE: To describe sleep problems among youth with and without functional abdominal pain (FAP). METHODS: Participants were 8-15 years of age diagnosed with FAP (n = 67) or healthy pain-free comparisons (n = 80). Parents and participants completed instruments assessing sleep, psychiatric status, and FAP symptoms. Parent and child reports of sleep problems were compared across groups and the association of FAP to behavioral sleep problems was assessed controlling for psychopathology. RESULTS: Children with FAP were reported to have more symptoms of behavioral sleep disorders (BSD), as well

A patient with cerebral Whipple disease with gastric involvement but no gastrointestinal symptoms: a consequence of local protective immunity?

Whipple disease is a granulomatous infectious disease caused by Tropheryma whipplei. The bacteria accumulate within macrophages, preferentially in the intestinal mucosa. Disease manifestation seems to be linked to immunological abnormalities of macrophages. We describe a patient with cerebral Whipple disease who presented with changes in mental status, confusion, inverse sleep-wake cycle, bilateral ptosis and vertical gaze palsy. Endoscopic biopsy sampling revealed Whipple disease in the gastric antrum but not in the duodenum. Whole blood stimulation displayed reactivity to T. whipplei that wa

Radiation somnolence syndrome.

Although initially described 7 decades ago, somnolence syndrome remains a poorly understood subacute effect of cranial irradiation. Despite the relatively transient and benign nature of somnolence syndrome, its symptoms can be distressing for children and caregivers. Anticipatory guidance related to radiation-induced somnolence remains a critical nursing intervention. This article reviews what is known about somnolence syndrome, including its causes, symptoms, and management.

Sleep fragmentation in children with atopic dermatitis.

OBJECTIVE: To characterize the sleep pattern of children with atopic dermatitis in clinical remission. METHODS: Fourteen children with atopic dermatitis, with a mean+/-SD age of 6+/-2 years, were recruited consecutively from a pediatric dermatology clinic. No participant had any other medical or psychiatric illness. The control group (n = 9, mean age 7+/-1.8 years) was composed of children with mild "benign" snoring and no evidence of respiratory disturbance during sleep. All participating children were evaluated by formal all-night polysomnography, scratch electrodes, and self-reported quest

Review of strategies for treating sleep problems in persons with severe or profound mental retardation or multiple handicaps.

During the last 15 years, researchers have shown that a large percentage of persons with severe or profound mental retardation or multiple handicaps have serious sleep problems. These problems can cause emotional and physical burdens for caretakers, interfere with the persons' daily learning and occupational performance, and/or exacerbate deviant behaviors. Several studies have been conducted to assess behavioral and pharmacological strategies (e.g., bedtime fading and response cost, extinction, chronotherapy, and the use of melatonin) for reducing sleep problems. Those studies are reviewed he

Sleep disturbances, fibromyalgia and primary Sjögren's syndrome.

OBJECTIVE: To investigate the association and prevalence of sleep disturbances and fibromyalgia (FM) in a group of patients with primary Sjögren's syndrome (pSS). METHODS: Sixty-five patients with pSS were investigated. A 10-point Mini Sleep Questionnaire (MSQ) was completed focusing on sleep complaints. The same questionnaire was also used in three control groups: Group A-67 patients with rheumatoid arthritis. Group B-53 patients with rheumatoid arthritis (RA) and sicca symptoms; Group C-31 patients with osteoarthritis. All patients with pSS were also studied fro the presence of FM. RESULTS

Low dose famotidine in the prevention of sleep disturbance caused by heartburn after an evening meal.

AIM: To determine whether, in a susceptible population, dosing with 10 mg famotidine 1 h before an evening meal could decrease the interference with sleep caused by heartburn. METHODS: Patients with a history of frequent heartburn (n = 309) were randomized to receive 10 mg famotidine or placebo 1 h before an evening meal likely to induce symptoms. Patients assessed the efficacy of the treatment in preventing heartburn after the meal, at bedtime and during the night. The number of awakenings due to heartburn and the consumption of antacid tablets taken to alleviate symptoms were also recorded.

The pattern of isolated sleep paralysis among Nigerian medical students.

In a cross-sectional study of the patterns of isolated sleep paralysis among 164 Nigerian medical students, 26.1% admitted having experienced this phenomenon. About 31% of the females and 20% of the males had had this experience. Of those with sleep paralysis, 32.6% had hypnogenic hallucinations during the episode, mainly visual. Sleep paralysis was not significantly associated with psychosocial distress or differences in personality profile. Although the rate differs across cultures, the myths associated with sleep paralysis are similar.

[The duodenum : a psychosomatic organ? (author's transl)].

A pressure-captor tube was inserted into the duodenum close to the ampulla of Vater in 15 surgical patients. The authors present a selection of 12 pressure curves under different circumstances : sleep, awake, meal, psychological agression, physical agression, pharmacological agression. The curves indicate that the duodenum reflects a psychosomatic structure in terms of its motricity, which could explain various aspects of gastric or biliary pathology.

Nocturnal sleep panic and depression: relationship to subjective sleep in panic disorder.

BACKGROUND: Patients with panic disorder (PD) often complain of sleep disturbances. PD patients have high co-morbid depression and almost 65-70% reports a history of nocturnal panic attacks. It is possible that both nocturnal-sleep panic attacks and depression contribute to sleep disturbances in PD patients. However, the individual and interactive effects of nocturnal-sleep panic attacks and lifetime depression on subjective sleep in PD are unknown. METHODS: The National Institute of Mental Health Panic Disorder Questionnaire (NIMH-PQ) was administered to 773 individuals who met DSM-IV criter

Ion concentrations in CSF and serum are differentially and precisely regulated.

Fluctuations of extracellular brain ion concentrations have been associated with transitions between brain states such as sleep and wakefulness, and disturbances have been implicated in a variety of neurological conditions, including dementia, epilepsy and migraine. This study aims to define the normal CSF ion profile and identify key factors influencing its regulation. In this cross-sectional study, we analysed samples from 42 individuals (16 men), including 28 healthy participants and 14 patients with medically unexplained neurological symptoms. Age spanned between 20 and 55 years. Using val

The impact of cognitive behavioral group therapy for anxiety on GAD related sleep disturbance during the perinatal period.

BACKGROUND: Generalized anxiety disorder (GAD) is highly prevalent during the perinatal (pregnancy and postpartum) period. Sleep disturbance (SD), a core symptom of GAD, and a common difficulty during the perinatal period, has been overlooked as a specific treatment target This study examined (1) the efficacy of cognitive behavioral group therapy for perinatal anxiety (CBT-PA) in reducing SD among individuals with GAD, and (2) whether SD functions as a mechanism of treatment change. METHODS: Perinatal participants (pregnant or ≤12 months postpartum) with GAD enrolled in a randomized trial (n
