Twenty-six patients with unilateral facet dislocation of the cervical spine were analyzed to determine the best method of treatment of this injury, the incidence of late pain and instability, and the indications for surgical stabilization. This review showed that all patients with a unilateral facet dislocation of the cervical spine should be treated initially with halo traction in an attempt to obtain reduction. The problems are discussed that are encountered in diagnosing instability of the middle and lower cervical spine on the basis of conventional radiographs. Two aspects are considered: 11 elementary radiological signs are presented; the corresponding radiological syndromes, their clinical presentations, and the roentgenographic signs are discussed. Both aspects are illustrated by means of appropriate radiographs.

A prospective evaluation of dynamic lateral lumbar spine radiographs in the erect position in 52 symptomatic patients over the age of 50 years revealed vacuum phenomena in intervertebral discs in 46%. An analysis of the clinical course of the disease in 516 patients with tumors of the spinal cord and vertebral column made it possible to identify 4 types of the disease, namely fulminant, acutely progressive, remittent and continuously progressive. The usefulness of spinal computed tomography (CT) in predicting the presence of epidural tumor was evaluated in cancer patients undergoing CT myelography for suspected epidural tumor. Two hundred ninety two vertebral levels were evaluated in 30 patients. Spinal CT demonstrated cortical disruption surrounding the epidural space from metastatic cancer in 109 vertebrae. There are good results in the functional conservative treatment of the primary stable lesion of the thoracic and the lumbar part of the spine. Late results show that traumatic deformation can be compensated by the spine and by trained muscles. Segmental mobility can be generally neglected. Real late instabilities are very seldom. Static complaints caused by left deformation are important especially if the spine was previously damaged. Then, primary or early secondary operative reposition, replacement and segmental spondylodesis are indicated.

Bone scanning of the thoracolumbar spine and the anterior thorax was performed in 7 girls with recently diagnosed progressive thoracic idiopathic scoliosis. A reference group consisted of a group of 8 patients submitted to bone scanning for other reasons. A new system of operative fixation of thoracolumbar and lumbar spine fractures is presented: the 'fixateur interne' (F.I.). From a posterior approach long Schanz screws are inserted through the pedicles into the body of the two vertebrae just adjacent to the lesion and connected by th threaded F.I. rods. Between 1955 and 1968, we treated five patients who had post-traumatic Charcot's arthropathy of the spine. All of the patients had a neurologically complete spinal-cord lesion. A laminectomy was implicated as the cause in four of the five patients. Spinal arthrodesis was used successfully to correct the deformity, stabilize the spine, restore sitting balance, and prevent complications resulting from neuropathic arthropathy of the vertebrae following complete spinal-cord injury.
