Pharmacotherapy improves treatment outcome in depressed cocaine addicts.

Depression is the most common psychiatric disorder in substance abusers, and results in a poorer prognosis and response to traditional chemical dependence and mental health treatment approaches. This article focuses on the use of pharmacotherapy in the treatment of the cocaine addict with secondary depression and provides general clinical treatment guidelines. It also reports on a study in which depressed, methadone-maintained cocaine addicts were treated in a 12-week placebo-controlled trial that compared two pharmacological agents. Patients were randomly assigned to receive placebo, amantadi

Adverse health consequences of cocaine abuse.

Cocaine creates a strong physical addiction and is becoming recognized as one of the most dangerous illicit drugs abused today. The myth is that cocaine is harmless and nonaddictive. An estimated 30 million Americans have used cocaine, but the number may be as high as 40 million. Five to six million individuals are compulsive users. A review of the current literature revealed multiple reports of acute myocardial infarction and cerebrovascular accident with a temporal relation to cocaine use. Cocaine has also been associated with acute rupture of the aorta, cardiac arrhythmia, and sudden death.

Diagnosing depressive disorders in patients with alcohol and drug problems: a comparison of the SADS-L and the DIS.

The DIS and the SADS-L diagnostic procedures occupy an important position in psychiatric research. We compared these procedures for assessing depressive disorders in patients with substance abuse problems. The two instruments agreed poorly, and the SADS-L produced a considerably higher proportion of cases than the DIS. A number of factors were examined to determine their influence on the agreement obtained for Major Depression. These factors included (1) recency of the disorder, (2) the primary/secondary distinction, (3) clinical experience of the DIS interviewers, (4) numerous subject charact

Reducing the risk: barbiturate substitution with a benzodiazepine.

A study was carried out in general practice in 85 habitual barbiturate takers, many of whom had been using this hypnotic for over 30 years, to assess the clinical practicability and acceptance of barbiturate substitution with flurazepam. Each barbiturate capsule or tablet being taken by patients was replaced by 15 mg flurazepam, either immediately or by gradual substitution. Patients were followed-up over a period of 6 months. The results showed that 5% of the patients were able to stop hypnotic therapy after initial substitution with flurazepam, a further 7% were maintained on occasional flur

Acquired transient CNS hypersensitivity to lithium therapy.

Organic brain syndrome is reported to occur occasionally in patients who are treated with lithium carbonate alone, or in combination with neuroleptics. Such neurotoxicity is often associated with moderate to low serum lithium levels and is usually reversible upon the cessation of lithium treatment. The authors report on a 57 year old manic-depressive and alcoholic patient who, after four years of lithium treatment, developed symptoms of CNS hypersensitivity which reappeared several times when he was challenged with lithium during a two month period. The possible role of an underlying, subclini

Understanding healthcare engagement for people who inject drugs.

People who inject drugs (PWID) are at an increased risk of multimorbid mental health and chronic diseases, which are frequently underdiagnosed and under-treated due to systemic barriers and ongoing substance use. Healthcare engagement is essential to address these conditions and prevent excess morbidity and mortality. The goal of this study was to understand how PWID engage in care for their chronic health conditions and substance use treatment given the known historic and pervasive barriers. We conducted 24 semistructured qualitative interviews informed by the Behavioral Model for Vulnerable

Deployment-related psychiatric and behavioral conditions and their association with functional disability in OEF/OIF/OND veterans.

Understanding the factors that influence veterans' functional outcome after deployment is critical to provide appropriately targeted care. Mild traumatic brain injury (mTBI) and posttraumatic stress disorder (PTSD) have been related to disability, but other psychiatric and behavioral conditions are not as well examined. We investigated the impact of deployment-related psychiatric and behavioral conditions on disability among 255 OEF/OIF/OND service members and veterans. Structured clinical interviews assessed TBI and the psychiatric conditions of depression, PTSD, anxiety, and substance use. S

Neuroimaging and neurophysiologic biomarkers for diagnosis and prognosis of depressive disorders, bipolar disorder, anxiety disorders, obsessive compulsive disorder, posttraumatic stress disorder, and substance use disorder: an evidence map.

BACKGROUND: Advancements in precision medicine, particularly the use of neuroimaging and neurophysiologic techniques, may improve diagnosis, prognosis, and treatment of mental health disorders. Recent efforts to develop large neuroimaging datasets have yielded promising results for identifying mental health biomarkers. This scoping review identifies and characterizes studies of neuroimaging and neurophysiologic techniques used to address a variety of mental health disorders. METHODS: We searched MEDLINE and Embase (January 2010-September 2023). Eligible studies examined neuroimaging and neuro

Bromazolam Tablet Quantification and Analysis of Post-Mortem Cases From the National Programme on Substance Use Mortality (NPSUM).

Bromazolam is a new psychoactive substance (NPS) benzodiazepine commonly identified by drug checking services and in post-mortem toxicological analyses in the United Kingdom, Europe, and North America. At the time of writing, there are no studies that present quantitative analyses of bromazolam in street tablets. Here we describe the first quantitative analysis of bromazolam tablets, from samples submitted by UK drug checking services and police forces between 2022 and 2025. Using validated GC-EI-MS and 1H NMR methods, 47 tablet samples were quantified revealing a median bromazolam dose of 0.4

How Is Food Addiction Addressed at U.S. Substance Use Disorder Treatment Facilities? Perspectives of Service Providers.

BACKGROUND: Substance use disorders (SUDs) are a leading public health concern in the United States. Food addiction (FA), as a common comorbidity or substitute addiction of SUDs, is not currently a fifth edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) diagnosis, with overlaps with SUDs and eating disorders (EDs). How FA is addressed in SUD treatment is understudied. METHODS: Cross-sectional survey interviews with service providers (N = 85) from 68 SUD treatment facilities across 20 U.S. states asked an open-ended question on how FA was addressed. Descriptive stati

Tracking functional recovery in a community-based substance use disorder program: a five-year descriptive evaluation using the brief addiction monitor.

OBJECTIVES: Accessible, evidence-based treatment for substance use disorders (SUDs) remain a public health challenge due to complex clinical and social needs and barriers to long-term engagement. This study describes a five-year evaluation of a low-barrier, outpatient SUD treatment program implemented by the Health Care District of Palm Beach County, Florida, focusing on trends in functional recovery using the Brief Addiction Monitor (BAM) functional assessment. METHODS: Between February 2018 to March 2023, participants with substance use disorders received care through a Federally Qualified H

Drug use pattern of suspected drug-influenced drivers in Hungary (2019-2023).

Between 2019 and 2023, blood and urine samples of 6229 suspected impaired drivers were analysed, with a positivity rate of 91.5 %. Classical illicit drugs were detected in 4693 drivers, medicines in 850 drivers, stimulant new psychoactive substances (sNPS) in 928 drivers and synthetic cannabinoid receptor agonists (SCRAs) in 657 drivers. The consumption of cannabis, N-ethyl-norpentedron, and N-ethyl-hexedrone increased significantly over time, while the consumption of five SCRAs declined. These changes were concordant with the seizure data for the entire country. 74.2 % of drug-positive driver

[Managing benzodiazepine withdrawal using prazepam substitution: Retrospective study of 86 patients].

INTRODUCTION: In France, the consumption of benzodiazepines (BZD) remains significant despite Haute autorité de santé (French National Authority of Health-HAS) recommendations. Various strategies are recommended: gradual dosage reduction of the BZD consumed or switch to diazepam in certain situations. Prazepam may also be an interesting molecule (long half-life, oral solution, less risk of abuse than diazepam). The aim of this study is to describe the efficacy, adverse events and feasibility of a BZD withdrawal program with prazepam. MATERIALS AND METHODS: Data were collected retrospectively
