Interpretation of genetic association studies: markers with replicated highly significant odds ratios may be poor classifiers.

Recent successful discoveries of potentially causal single nucleotide polymorphisms (SNPs) for complex diseases hold great promise, and commercialization of genomics in personalized medicine has already begun. The hope is that genetic testing will benefit patients and their families, and encourage positive lifestyle changes and guide clinical decisions. However, for many complex diseases, it is arguable whether the era of genomics in personalized medicine is here yet. We focus on the clinical validity of genetic testing with an emphasis on two popular statistical methods for evaluating markers

Uncontrolled manifold analysis of single trials during multi-finger force production by persons with and without Down syndrome.

Recently, the framework of the uncontrolled manifold (UCM) hypothesis has been used to study multi-finger synergies based on analysis of motor variability across large sets of trials. We introduce a similar method of analysis, which can be applied to single trials, and hence may be more relevant to studies of atypical populations. In one experiment, results of across-trials and single-trial UCM analysis were compared for control participants who performed accurate ramp force production trials by pressing with four fingers of the hand. Both types of analysis revealed selective stabilization of

[Analysis of causes for falls in people with Parkinson's disease].

Falls in Parkinson's disease may pose a significant threat for patients. This is not only a clinical problem, but also an economic one. The effects of falls may cause deterioration of the quality of life for both patients and their caretakers. The causes of falls are not clinically uniform: the falls are caused by various factors and require a differential diagnosis. The purpose of this paper is to analyse the above mentioned causes, as well as to draw clinicians' attention to the possibility of effective therapy for certain disorders that cause such falls in patients with Parkinson's disease.

Incidence and duration of hospitalizations among persons with AIDS: an event history approach.

OBJECTIVE: To analyze hospitalization patterns of persons with AIDS (PWAs) in a multi-state/multi-episode continuous time duration framework. DATA SOURCES: PWAs on Medicaid identified through a match between the state's AIDS Registry and Medicaid eligibility files; hospital admission and discharge dates identified through Medicaid claims. STUDY DESIGN: Using a Weibull event history framework, we model the hazard of transition between hospitalized and community spells, incorporating the competing risk of death in each of these states. Simulations are used to translate these parameters into re

The threat of multidrug-resistant tuberculosis: results of 1 yr of surveillance in the Lombardy region of Italy.

A descriptive multicentre study based on laboratory data was carried out in patients with culture-confirmed tuberculosis (TB) who were cared for between September 1995 and August 1996 in 14 general hospitals with at least one human immunodeficiency virus (HIV) ward, a hospital specializing in TB or a large chest clinic, all in the Lombardy region of Italy. For each culture positive for Mycobacterium tuberculosis, the results of susceptibility tests to the five first-line anti-TB drugs were collected; other information collected included: the patient's name, the ward/service where the patient w

Liver Injury in Immune Stevens-Johnson Syndrome and Toxic Epidermal Necrolysis: Five New Classification Types.

Liver injury in Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) is a multifaceted disorder, lacking cohort homogeneity due to a variety of potential causes, including drugs, arsenic and other heavy metals, glyphosate, infections, and ultraviolet radiation. The goals of this review were (1) to analyze the role of diagnostic algorithms in assessing causality for potential culprits involved in the development of liver injury associated with immune-mediated SJS and TEN, which represent immune-based variant disorders within a continuous spectrum. Milder forms are classified as S

Clinical Characteristics and Therapeutic Aspects of Blaschko Linear Psoriasis.

INTRODUCTION: Blaschko linear psoriasis (BLP) is characterized by the linear distribution of psoriatic skin lesions along the Blaschko lines. BLP can be divided into type I and type II, mainly on the basis of clinical manifestations. BLP can easily cause psychological burdens in patients and clinical confusion for physicians. Here, we summarize clinical cases to provide a better understanding of BLP. METHODS: The subjects included patients with BLP who visited our dermatology departments and those reported in the literature obtained from the PubMed and Wanfang databases. Quantitative data wer

Impact of age at appendectomy on development of type 2 diabetes: A population-based cohort study.

AIM: Diabetes is a complex metabolic disease characterized by chronic low-grade inflammation in which genetic and environmental factors are involved. Growing evidence implicates that alterations of the gut microbiota potentially contribute to the emergence of metabolic diseases. The human appendix has more recently been recognized as a microbial reservoir for repopulating the gastrointestinal tract and an important part of the immune system. Thus, appendectomy may influence microbial ecology and immune function. This study investigated the association between appendectomy and type 2 diabetes r

Assessment of minimal clinically important improvement by using Oral Health Impact Profile-14 in Behçet's disease.

BACKGROUND: Behcet's disease involves several systems in the body. Neurological involvement is identified by different symptoms. Headache is one of the common complaints of patients with Behcet's disease. It might be a part of neurological involvement or may arise independently in the course of disease. Studies with small sample size have resulted in various findings in this field. Since the prevalence of Behcet's disease is relatively high in Iran, this study was carried out to compare the features of headache between an acceptable number of patients with this rare disease and a control group

Outgroup emotion processing in the vACC is modulated by childhood trauma and CACNA1C risk variant.

A high frequency of outgroup contact-as experienced by urban dwellers and migrants-possibly increases schizophrenia risk. This risk might be further amplified by genetic and environmental risk factors, such as the A-allele of rs1006737 within the calcium voltage-gated channel subunit alpha1 C gene and childhood interpersonal trauma (CIT). Both have been related to ventral anterior cingulate cortex (vACC) functioning. We investigated vACC functioning, during ingroup and outgroup emotion perception in relation to rs1006737 and CIT. Group membership was manipulated through a minimal group paradig

Hearing impairment after childhood bacterial meningitis dependent on etiology in Luanda, Angola.

OBJECTIVE: Childhood bacterial meningitis (BM) damages hearing, but the potential of different agents to cause impairment in developing countries is poorly understood. We compared the extent of hearing impairment in BM caused by Haemophilus influenzae type b (Hib), Streptococcus pneumoniae or Neisseria meningitidis among children aged 2 months to 13 years in Luanda, Angola. METHODS: Hearing of 685 ears of 351 (78%) survivors among 723 enrolled patients was tested by brainstem-evoked response audiometry on day 7 of hospitalization. The causative agent was sought by cerebrospinal fluid culture,

Genome-wide association data suggest ABCB1 and immune-related gene sets may be involved in adult antisocial behavior.

Adult antisocial behavior (AAB) is moderately heritable, relatively common and has adverse consequences for individuals and society. We examined the molecular genetic basis of AAB in 1379 participants from a case-control study in which the cases met criteria for alcohol dependence. We also examined whether genes of interest were expressed in human brain. AAB was measured using a count of the number of Antisocial Personality Disorder criteria endorsed under criterion A from the Diagnostic and Statistical Manual of Mental Disorders, 4th Edition (DSM-IV). Participants were genotyped on the Illumi

Kinship, gender, and the social networks of caregivers for older adults.

BACKGROUND AND OBJECTIVES: Caregiving for older adults falls disproportionately on women and family members, but little research addresses how gender and kinship intersect to shape caregiver social support. This study generates an empirical typology of caregiver social networks and examines the types' association with caregiver gender, relationship to the recipient, and their interaction. RESEARCH DESIGN AND METHODS: We collected personal network data from a nationally representative sample of 2,824 caregivers in December 2020 and January 2021. We used zero-inflated negative binomial regressi

Family mass murder in the United States, 1970-2019: Perpetrator mental health characteristics and the role of domestic violence.

Examining perpetrator characteristics of, and risk factors for, different types of mass murder, such as family mass murder-the most common type-could have important ramifications for preventive efforts. Histories of domestic violence and mental illness are of particular interest. We examined 944 cases of mass murder from the Columbia Mass Murder Database that occurred in the United States between 1970 and 2019, identifying 377 incidents (39.9%) in which at least one victim was a family member of the perpetrator. We collected information on histories of domestic violence and characteristics of

Epidemiology and economic burden of selected rare genetic diseases in Germany - a claims database study.

BACKGROUND: Research on rare genetic diseases is challenging due to their low prevalence and complex clinical manifestations. Despite their individual rarity, the cumulative burden of these diseases is substantial. Larger claims databases could enable a quantification and characterization of patients with rare diseases and contribute to determining their specific healthcare needs. This study examines the prevalence, comorbidities, and annual direct costs of Huntington’s disease (HD), Beta-Thalassemia (BT), and Spinal Muscular Atrophy type 1 (SMA type 1) in Germany. METHODS: Utilizing anonymize

Unveiling the Complexities of Hereditary Angioedema.

Hereditary angioedema (HAE) is a rare and potentially life-threatening genetic disorder, constituting approximately 2% of all clinical cases of angioedema, with a global prevalence estimated between 1 in 50,000 and 1 in 150,000 individuals. The condition affects individuals of all genders and ethnic backgrounds without significant variation. HAE is classified into three types. Type I HAE, which accounts for 85% of cases, is characterized by a deficiency of the C1 esterase inhibitor (C1-INH) gene. Type II HAE, making up 15% of cases, involves a dysfunctional C1-INH. Type III HAE, which represen
