Effects of hormonal contraceptive use on HIV acquisition and transmission among HIV-discordant couples.

BACKGROUND: The risk of HIV associated with hormonal contraceptives is controversial. We assessed hormonal contraceptive use and HIV incidence in HIV-discordant couples in Rakai, Uganda. METHODS: HIV-discordant couples were retrospectively identified from a cohort between 1999 and 2009. Hormonal contraception included oral contraception, depomedroxyprogesterone acetate (DMPA), and implants (Norplant). Poisson regression estimated adjusted incidence rate ratios (adjIRRs) associated with hormonal contraceptive methods. A case-control subanalysis estimated odds ratios (ORs) of HIV associated wit…

[Long latency is characteristic for "imported" cases of malaria in children at a municipal hospital in Vienna].

BACKGROUND: Malaria is the most frequent tropical disease and the most important parasitic infectious disease in the world. Due to high mobility by travelling and migration also in Central Europe malaria has to be considered also in children and youths. METHODS: We report four cases of malaria being diagnosed and treated at the department of paediatrics, Vienna Danube Hospital, a municipal centre. RESULTS: In all cases the latency time or incubation period respectively was long with up to one year which made the diagnosis of malaria unlikely at first glance. CONCLUSIONS: Taking into account

[Ebola and Marburg hemorrhagic fever viruses: update on filoviruses].

The Ebola and Marburg viruses are the sole members of the Filoviridae family of viruses. They are characterized by a long filamentous form that is unique in the viral world. Filoviruses are among the most virulent pathogens currently known to infect humans. They cause fulminating disease characterized by acute fever followed by generalized hemorrhagic syndrome that is associated with 90% mortality in the most severe forms. Epidemic outbreaks of Marburg and Ebola viruses have taken a heavy toll on human life in Central Africa and devastated large ape populations in Gabon and Republic of Congo.

Regular vitamin C supplementation during pregnancy reduces hospitalization: outcomes of a Ugandan rural cohort study.

BACKGROUND: Vitamin C or ascorbic acid is a hydro-soluble lactone (synthesized from glucose) essential to human body and available from diet. Despite its broad availability in fruits and vegetables, in many developing countries the incidence of clinical symptoms due to the vitamin deficiency is still very high. Also, pregnant women in the developing countries are frequently hospitalized for several preventable reasons such as anemia in pregnancy, mostly iron-deficient anemia (IDA) and the upper/lower respiratory tract infections (RTI). The aim of the study was to investigate, in a Ugandan rura…

Treatment of Schistosoma mansoni infection increases helminth-specific type 2 cytokine responses and HIV-1 loads in coinfected Ugandan adults.

BACKGROUND: Studies showing that helminths stimulate type 2 cytokine responses and influence responses to unrelated antigens suggest that helminths may accelerate human immunodeficiency virus type 1 (HIV-1) disease progression in coinfected individuals and that antihelminthic therapy may be beneficial. By the same logic, however, the increase in type 2 cytokines occurring immediately after antischistosomal treatment might increase viral replication and be detrimental. METHODS: To assess the effect of antischistosomal therapy on immune responses and HIV-1 replication, a cohort of 163 Ugandans …

Formalising remote consulting in primary care services for underserved populations with long-term conditions in sub-Saharan Africa: a process evaluation and stakeholder engagement.

INTRODUCTION: Remote consulting can improve healthcare access and equity. When delivered by trained health workers, it is safe, trustworthy and efficient. Alongside two independent clinical trials, we studied the implementation of a cascaded health worker training intervention (REaCH), plus airtime, on remote consulting in primary healthcare facilities in Nigeria (n=35) and Tanzania (n=21). Additionally, we assessed health service readiness for sustainable remote consulting in Ghana, Kenya, Nigeria, Malawi, Sierra Leone, Tanzania and Uganda. METHODS: We explored what worked for whom, how, and…

Incidence and Associated Factors of Cord Infection Among Neonates at Fort Portal and Hoima Regional Referral Hospitals.

INTRODUCTION: Untreated cord infections can result in life-threatening effects including necrotizing fasciitis. In Uganda, these effects due to cord infections are high. Proper cord care is crucial for preventing infections. This study aimed to determine the incidence of cord infection and associated factors among neonates at Fort portal and Hoima Regional Referral Hospitals. METHODS: This was a prospective cohort study in which 334 neonates born at FRRH and HRRH and their parents consented to the study were consecutively enrolled and followed up for 14 days. Neonates with abdominal wall defe…

Local habitual movement as a mechanism for Schistosoma mansoni transmission resurgence: a causal analysis of cross-sectional survey data.

BACKGROUND/AIMS: The WHO aims to eliminate schistosomiasis as a public health problem (EPHP) across 78 endemic countries, with interruption of transmission (IoT) as the goal in selected African countries by 2030. However, for low-prevalence settings that reach EPHP, guidance on managing transmission to maintain EPHP or move towards IoT is limited, partly due to insufficient knowledge of the drivers of persistence and/or re-emergence. In Uganda, some communities inland from Lake Victoria have achieved EPHP for Schistosoma mansoni but not progressed to IoT. This study aimed to assess whether rou…

Cardiac Tamponade Secondary to Massive Pericardial Effusion in Severe Primary Hypothyroidism: A Case Report.

BACKGROUND: Pericardial effusion is a recognized manifestation of overt hypothyroidism, but progression to cardiac tamponade is rare. In tuberculosis-endemic settings, diagnostic focus often favors tuberculous pericarditis, risking delayed recognition of reversible endocrine causes. CASE PRESENTATION: A 65-year-old Black African woman from Northern Uganda presented with five months of progressive dyspnea, orthopnea, and generalized edema, followed by acute pleuritic chest pain. She had a history of hypertension and type 2 diabetes mellitus. No prior thyroid disease, tuberculosis, malignancy, …

Ebola Virus Disease in the Era of One Health and Global Preparedness: Evolving Epidemiology, Genomic Surveillance, and Future Challenges.

Ebola virus disease (EVD) remains one of the most severe viral hemorrhagic fevers, with recurrent outbreaks challenging global healthcare systems. This narrative review traces the evolving epidemiology of EVD from the first recognized outbreaks in 1976 to the ongoing 2026 Bundibugyo virus public health emergency in the Democratic Republic of the Congo (DRC) and Uganda. Over nearly five decades, the recognized range of Ebola outbreak contexts and amplification mechanisms has broadened considerably: rural zoonotic spillovers remain the predominant mode of emergence, but the scale and reach of su…

Given that sequencing was performed on only two samples, a causal role for CVA24v in this outbreak cannot be definitively established; however, these findings provide the first genomic evidence of CVA24v …
