This study examines the accuracy of a new portable abdominal ultrasound machine (Bladder Scan, BVI 2500) used to measure postvoid urine residual (PVR). Using this machine, we started prospectively measuring residual urine in the first 80 women undergoing uroflowmetry in our urodynamic unit. Conventional oral pharmacotherapy for urge incontinence remains problematic because of limited efficacy and systemic side effects. In this study 27 patients with severe urge incontinence were treated with intravesical drugs (emepron 200 mg or oxybutynin 5 mg) twice a day. OBJECTIVE: To review the results of bladder-neck reconstruction in patients with repaired bladder exstrophy and pubic diastasis.
PATIENTS AND METHODS: Nine girls (mean age 7 years, range 4-17) and four boys (mean age 9 years, range 5-15) underwent a modified Young-Dees-Leadbetter bladder-neck reconstruction with augmentation cystoplasty (YDL-C). Creation of a neourethra by tubularization of the anterior bladder wall during bladder reconstruction at radical retropubic prostatectomy resulted in earlier return of urinary control. The aim of this study was to investigate risk factors for urinary tract infection with Pseudomonas aeruginosa, the course of such infection and the therapeutic approach among doctors. A total av 56 patients were included. Identified risk factors were old age, institutionalization, obstruction or other dysfunction of the urinary tract, use of urinary tract catheters and treatment with broad spectrum antibiotics. Urinary tract infection with P aeruginosa has a tendency to persist or relapse. The therapeutic approach varied widely.

Compared with urethral catheterisation, suprapubic catheterisation is associated with a reduced incidence of urinary infection and urethral stricture and it facilitates a controlled trial of micturition. A prospective comparison of two types of suprapubic catheter was performed in 70 patients, using a specially designed catheter mounted on a trocar for insertion (Suprapubic Ingram Trocar), and a disposable trocar and cannula (Add-a-Cath) and standard Foley catheter. Bladder temperature measured by a thermistor tipped urinary catheter was compared to rectal temperature in 14 ICU patients. During a period of five days 2 x 232 measurements were recorded, and good correlation was found between rectal and bladder temperatures (R = 0.97). There did not seem to be any drift in the bladder thermistor. Measurement was accurate during periods of oliguria. We find that bladder temperature measurement is reliable, safe, convenient and accurate for routine use in the ICU.

Spinal cord injuries in the past were associated with a very high mortality, predominantly from urologic complications and sepsis. Improvements in the urologic and medical management of these patients have significantly decreased the morbidity and mortality. Many complications may still develop during the urologic management, however. Periodic follow-up is thus essential to recognize and treat urologic complications in order to preserve renal function.

This prospective study was conducted in order to evaluate the efficacy of transurethral microwave thermotherapy (TUMT) in the treatment of benign prostatic hyperplasia (BPH). A single TUMT session of 55 min was delivered as an ambulatory procedure without anesthesia to 180 patients. 20 (69%) of the 29 patients who were catheterized due to urinary retention prior to the treatment were able to urinate freely after the treatment. Urinary tract infection of men with spinal cord injuries has been associated with a high incidence of colonization of the patients with gram-negative bacilli. We have examined the factors influencing colonization of 119 patients with Pseudomonas and Klebsiella and studied methods of reducing this colonization. We report on a case of vesicouterine fistula following cesarean section in a 26-year-old patient. A programmable baclofen pump was implanted in 7 patients for relief of severe spasticity. The patients experienced improvement in bladder and sphincter function. This system would seem to be of value in improving lower urinary tract function in patients with diseases that produce marked spasticity.

Two hundred patients treated with phenoxybenzamine for benign prostatic obstruction were reviewed. In 171 of these patients the treatment was given to relieve symptoms in patients not requiring operation, or in patients in whom operation was postponed.
