Primary Systemic Vasculitides as a Cause of Group IV Pulmonary Hypertension.

The primary systemic vasculitides are rare diseases characterized by vessel wall inflammation. Isolated pulmonary vasculitis, large-vessel vasculitis, and Behçet's disease are mimickers of chronic thromboembolic pulmonary hypertension (CTEPH); group IV pulmonary hypertension (PH) can occur as a devastating complication in the course of these diseases. Pulmonary endarterectomy, balloon angioplasty, anticoagulation and pulmonary vasodilator agents are the main treatment options for CTEPH. There is no specific recommendation for the treatment of patients having group IV PH due to primary systemic…

Infant with a rapidly progressing rash.

An infant presented to the emergency department with sudden onset of rapidly progressing erythematous macules and distal oedema. The rash was striking in appearance, covering most of the body, which raised concern for pressing infectious (eg, meningococcaemia) and other serious aetiologies. Nevertheless, the infant remained playful and was overall stable and posed a diagnostic and management dilemma. After an extensive workup and multispecialty consultations, the patient was eventually diagnosed with acute haemorrhagic oedema of infancy, a benign leucocytoclastic vasculitis. The patient receiv…

The central nervous system in systemic lupus erythematosus. Part 2. Pathogenetic mechanisms of clinical syndromes: a literature investigation.

OBJECTIVES: To identify the pathogenetic mechanisms of central nervous system (CNS) syndromes of systemic lupus erythematosus (SLE) as descr … haemorrhages are much less frequent than ischaemia and are presumably in part due directly to SLE. Vasculitis may cause ischaemia or haemorrhage in the CNS and is involved occasionally, as shown by imaging and histological findings. White matter damage is heterogeneous and ill-understood. It includes white matter degeneration and myelin vacuolation of the s …

[Allergic vascularities with clinical skin manifestations].

After describing the cutaneous and histo-pathological symptoms of allergic vasculazities, the author describes: Gougerot's tri-symptomatic disease, its differential diagnoses, and its various aetiologies. The pathology of the latter leads us to consider immune complexes, but our study does not deal with it in detail. Purpura rheumatica produces gammaglobulins A. Urticarial vasculazities may or may not be accompanied by anomalies of the complementary system; they may be transposed auto-immune disorders such as lupus erythematosus; their development is fairly unpredictable., Acute hemorrhagic oe

Histopathological Characteristics of Percutaneous Endomyocardial Biopsy in Heart Transplant Rejection Surveillance: A Single Center Experience.

BACKGROUND: Heart transplantation (HT) remains the ultimate treatment for end-stage heart failure. An endomyocardial biopsy (EMB) is "the gold standard" diagnostic procedure used in HT rejection surveillance. The aim of this study is to provide a detailed analysis of the histopathological characteristics of the EMB and to investigate if there is a correlation between some histopathological changes, such as fibrosis, vasculitis, Quilty effect (Q.E.), myocytes damage, and the presence of episodes of acute rejection. METHODS: In this retrospective study, 200 EMBs were included, coming from 65 pa…

Angioedema: a rare manifestation of Henoch-Schönlein purpura.

Henoch-Schönlein purpura (HSP) is a small vessel immunoglobulin A (IgA) vasculitis that primarily affects children. Characteristic manifestations include purpuric rash, abdominal pain, renal insult and arthralgia. Non-classical manifestations, such as angioedema, may mimic signs and symptoms associated with differential diagnoses including hereditary angioedema, anaphylaxis and urticarial vasculitis. We present the case of a girl in middle childhood who presented with angioedema with a known history of HSP. Familiarity with atypical manifestations of HSP may mitigate the need for invasive diag…

Ocular Manifestations of Pediatric Behçet's Disease.

Behçet's disease (BD) is a multi-systemic, chronic inflammatory disease of unknown etiology, with a multisystemic recurrent course and variable vasculitis. Although the disease onset is usually between the second and fourth decades of life, the preliminary symptoms and signs may appear under the age of 16 years. The prevalence of BD in children is unknown, but it is probably very low. The most common ocular manifestations of pediatric BD include anterior and posterior uveitis, panuveitis, retinal vasculitis, and retinitis. The pathogenesis, treatment, and future directions for pediatric BD are…

Carotid artery aneurysm as the initial presentation of Behçet's disease: A case report.

Behçet's disease is a rare systemic vasculitis that may involve arteries and veins. Carotid artery aneurysms are extremely uncommon and can be the initial manifestation. We report a 48-year-old man presenting with a painless pulsatile cervical mass. Imaging revealed a right common carotid artery aneurysm, surgically resected with a reversed saphenous vein graft. Histopathology suggested vasculitis, and subsequent evaluation confirmed Behçet's disease. This case highlights the importance of considering Behçet's disease in isolated arterial aneurysms and the need for combined surgical and immuno…

Proteinase-3 as an autoantigen and the development of granulomatosis with polyangiitis.

Human proteinase 3 (hPR3) is a lysosomal enzyme of the serine protease type. In autoimmune vasculitis, autoantibodies to hPR3 appear to have a role in the inception of antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV), where this protein is the main autoantigen. Indeed, patients with antibodies against hPR3 have more severe symptoms, relapses, and resistance to immunosuppressive therapies, supporting an important role for this autoantigen in the pathophysiology and severity of AAV. In this review, we describe what is known about the role of hPR3 in pathophysiology, diagnos…

Highlights from the breakout session: cellular and molecular mechanisms of disease (II).

The steps leading to key pathogenetic aspect of ANCA-associated vasculitis, such as lung nodules in granulomatosis with polyangiitis (GPA), onset of glomerulonephritis (GN) in experimental myeloperoxidase (MPO)-ANCA models and the implications of carrying staphylococcal superantigens, such as toxic shock syndrome toxin-1 (tsst-1), in GPA remain to be elucidated. The session 'Cellular and molecular mechanisms of disease (II)' was devoted to close some of these knowledge gaps and underlined that research in ANCA-associated vasculitis leads to more granular understanding of these complex diseases…

Isolated brachial artery aneurysm: a rare presentation of paediatric Behçet's disease.

Behçet's disease is a rare disease characterised by recurrent oral ulcers, with systemic manifestations including genital ulcers, ocular disease, skin lesions, gastrointestinal disease, neurologic disease, vascular disease and arthritis. Most clinical manifestations of Behçet's disease are believed to be due to vasculitis. The heterogeneous clinical spectrum is influenced by sex, ethnicity and country of residence. Vascular manifestation in the form of isolated large brachial artery aneurysm is rare in children. Treatment involves aneurysmorrhaphy to avoid rupture or ischaemic sequelae in addi…

Aortitis with aneurysm formation as a rare complication of Wegener's granulomatosis.

Wegener's granulomatosis (WG) is a systemic vasculitis of medium-sized and small blood vessels. Aortic involvement in WG is very uncommon. We present a 43-year-old patient with an aortitis with aneurysm formation as a manifestation of WG. The patient was operated on and an aortoiliac Dacron inlay graft was inserted. Postoperatively, he recovered uneventfully. Abdominal pain occurring during a WG flare may result from vasculitis of large abdominal arteries with or without aneurysmatic changes for which surgical treatment and immunosuppressive agents are indicated to prevent a possible rupture.

[Central nervous system involvement in Wegener's granulomatosis].

INTRODUCTION: Wegener's granulomatosis (WG) is a systemic necrotizing vasculitis associated with c-ANCA antibodies. The involvement of the central nervous system in WG is uncommon and usually caused by in situ vasculitis, intracranial granuloma formation or contiguous invasion from extracranial sites. CASE REPORT: Here, we report on a tumour-like expansion of a severe nasosinusal WG into the brain, which was confirmed by brain biopsy examination. CONCLUSION: The positivity of positron emission tomography in our observation supports the potential role of such functional imaging in the staging…

[Erythema induratum Bazin. "Tuberculid" or tuberculosis?].

A 66-year-old woman presented with erythematous nodules on both legs and told of a history of lymph node tuberculosis that had been treated with surgery and tuberculostatic therapy years before. A biopsy taken from a nodule showed nodular vasculitis. PCR for Mycobacterium tuberculosis-specific DNA on the paraffin-embedded tissue was positive. Based on the experience with our patient, positive PCR results for Mycobacterium tuberculosis-specific DNA in lesions of nodular vasculitis are discussed critically. The historical background of "tuberculids", such as erythema induratum Bazin, is elucidat…

[Juvenile cutaneous polyarteritis nodosa associated with streptococcal infection].

Polyarteritis nodosa is a rare vasculitis of small and medium arteries. It can occur in a systemic form with multi-organ involvement, or as a limited form confined to the skin, muscles, joints and peripheral nerves called cutaneous polyarteritis nodosa. Both forms are rare in adults and even more in children. The caues of this vasculitis remain unknown but some viruses and bacteria have been implicated, specially, Streptococcus. We present the case of a 6-year-old child who developed cutaneous polyarteritis nodosa following a probable streptococcal infection.

[Aortic rupture in Behçet's disease].

Behçet's disease is a vasculitis of unknown aetiology, which becomes apparent by ocular and mucocutaneous symptoms. In rare cases, owing to vasculitis of the vasa vasorum of large arteries, necrosis of the vascular wall and thus spurious aneurysms occur. The case of a 41-year-old patient is described who presented with a rupture of an infrarenal aortic aneurysm due to Behçet's disease. Therapy consisted of placement of an aortic-aortic Dacron prosthesis. On the basis of the case report, the diagnostic approach, the therapy and the value of secondary prophylaxis of Behçet's disease are discusse…

Acute fulminant symmetrical vasculitic polyneuropathy: need for early biopsy.

We report 2 patients with acute symmetrical axonal motor and sensory polyneuropathy associated with vasculitis. The clinical presentation in each case was not that of a mononeuritis multiplex. Serial electrophysiological studies in 1 of the patients documented the development of confluent axonal degeneration involving several motor and sensory territories over the same period of time. In both patients the diagnosis of necrotizing vasculitis was established by peripheral nerve biopsy and recovery followed treatment with prednisone and cyclophosphamide. Biopsy of an involved nerve should be rout…

Oral methotrexate therapy for chronic rheumatoid arthritis ulcerations.

Eight patients with long-standing rheumatoid arthritis and cutaneous vasculitis ulcerations resistant to conventional therapy were treated successfully with a low-dose intermittent regimen of oral methotrexate. Objective clinical response was prompt and complete resolution was observed at about 12 weeks of therapy. The drug was well tolerated. Mild gastrointestinal side effects were the most common untoward reaction. We conclude that methotrexate therapy is an effective agent for some of the extraarticular manifestations of rheumatoid arthritis including vasculitis, and further clinical evalua…

[Cutaneous manifestations of cytomegalovirus infections].

The cutaneous manifestations that occur commonly in cytomegalovirus (CMV) infections are erythematous maculopapular eruptions or purpuric rashes . But, CMV has been detected in chronic mucosal and skin ulcerations, benign tumour-like lesions and eroded gingivitis. Most of these cutaneous lesions are secondary to vasculitis of the small dermal, subcutaneous tissue or submucosa vessels. The authors discuss the different physiopathological mechanisms which may be involved in the development of these viral-induced vasculitis. Dermal erythropoiesis in neonatal infants and localized granulomatous le…
