Recurrent Panuveitis as a Manifestation of a Novel PIK3CD Gene Mutation: A Diagnostic and Management Challenge.

PURPOSE: To present a unique case of recurrent panuveitis in a young patient associated with a novel pathogenic variant in the PIK3CD gene, expanding the clinical spectrum of Activated PIK3 delta syndrome (APDS), a primary immunodeficiency that predisposes individuals to infections, autoimmunity, and malignancies. METHODS: We evaluated a 15-year-old boy with refractory panuveitis, recurrent infections, and lymphadenopathy with Hodgkin lymphoma. Ophthalmic assessment and ultra-wide field fundus fluorescein angiography were conducted. Whole exome sequencing identified a novel heterozygous patho

Recovery of cone-mediated vision in Lebercilin associated retinal ciliopathy after gene therapy: One-year results of a phase I/II trial.

We assessed the preliminary safety of a recombinant adeno-associated virus serotype 8 vector carrying the native human LCA5 cDNA (OPGx-001) in LCA5-associated Leber congenital amaurosis (LCA5-LCA), a congenital blindness. This phase 1b/2a trial (NCT05616793) is a nonrandomized, single ascending, dose-escalation study. Three subjects with LCA5-LCA (ages 19, 26, and 34 years old) received uniocular subretinal injections of 1E10 vector genome per eye of OPGx-001. There were no serious adverse events related to OPGx-001 or the procedure. Retinal microstructure by spectral-domain optical coherence

Varix of the vortex vein ampulla: a case report and imaging correlation.

BACKGROUND: We report a case of varix of the vortex vein ampulla (VVVA) in the right eye. VVVA is relatively rare and often presents as a fundus mass-like lesion during clinical examination, making it easily confused with choroidal tumors. This case report aims to improve ophthalmologists' understanding of this rare lesion and provide a reference for clinical diagnosis and management. CASE PRESENTATION: A 29-year-old male presented with 1 month of decreased vision in the right eye. His best-corrected visual acuity was 20/20. B-scan ultrasonography and optical coherence tomography (OCT) reveal

PEDIATRIC MACULAR HOLE ASSOCIATED WITH VITREORETINAL TRACTION ON EPIRETINAL LESIONS: A CASE REPORT AND LITERATURE REVIEW.

PURPOSE: We describe a case of nontraumatic macular hole in a pediatric patient associated with numerous epiretinal lesions throughout the macula. METHODS: A healthy 9-year-old girl presented to retina clinic with several months of blurry vision in the right eye. Clinically, there was a full-thickness macular hole with serous detachment and white epiretinal tufts. Spectral-domain optical coherence tomography confirmed the presence of the full-thickness macular hole with hyperreflective epiretinal proliferations in the right eye and an unremarkable left eye. Systemic workup was negative. After

UCHL1-Related Dominant Optic Atrophy: Report of Two New Families.

Previously, biallelic variants in ubiquitin carboxy-terminal hydrolase L1 (UCHL1) have been associated with spastic paraplegia type 79, an autosomal recessive early-onset neurodegenerative disorder. Recently, heterozygous variants in UCHL1 have been associated with a neurological syndrome with various combinations of optic atrophy, spasticity, ataxia and neuropathy. We present two independent families presenting with multigenerational optic atrophy. The proband of family A presented in her 60s with worsening gait imbalance and optic atrophy. The proband of family B presented in his 40s with wo

Neural adaptation to the eye's optics through phase compensation.

How does the brain achieve a seemingly veridical and 'in-focus' perception of the world, knowing how severely corrupted visual information is by the eye's optics? Optical blur degrades retinal image quality by reducing the contrast and disrupting the phase of transmitted signals. Neural adaptation can attenuate the impact of blur on image contrast, yet vision rather relies on perceptually-relevant information contained within the phase structure of natural images. Here we show that neural adaptation can compensate for the impact of optical aberrations on phase congruency. We used adaptive opti

Using Computer Vision to Annotate Video-Recoded Direct Observation of Physical Behavior.

Direct observation is a ground-truth measure for physical behavior, but the high cost limits widespread use. The purpose of this study was to develop and test machine learning methods to recognize aspects of physical behavior and location from videos of human movement: Adults (N = 26, aged 18-59 y) were recorded in their natural environment for two, 2- to 3-h sessions. Trained research assistants annotated videos using commercially available software including the following taxonomies: (1) sedentary versus non-sedentary (two classes); (2) activity type (four classes: sedentary, walking, runnin

Inherited Optic Neuropathies: Real-World Experience in the Paediatric Neuro-Ophthalmology Clinic.

Inherited optic neuropathies affect around 1 in 10,000 people in England; in these conditions, vision is lost as retinal ganglion cells lose function or die (usually due to pathological variants in genes concerned with mitochondrial function). Emerging gene therapies for these conditions have emphasised the importance of early and expedient molecular diagnoses, particularly in the paediatric population. Here, we report our real-world clinical experience of such a population, exploring which children presented with the condition, how they were investigated and the time taken for a molecular dia

Case Series on Autosomal Recessive Non-Syndromic Retinitis Pigmentosa Caused by POMGNT1 Mutations with a Report of a New Variant.

Recessive Protein O-linked-mannose beta-1,2-N-acetylglucosaminyltransferase 1 (POMGNT1) mutations can cause early onset muscle-eye-brain disease but have also more recently been associated with non-syndromic Retinitis Pigmentosa. In this case series, we describe three sisters affected by non-syndromic autosomal recessive POMGNT1 retinopathy with a report of a new variant. The three patients received care at West Virginia University Eye Institute, including full ophthalmic examination with additional fundus imaging, optical coherence tomography (OCT), electroretinogram (ERG), and visual field t

Photo-crosslinked hydrogels for tissue engineering of corneal epithelium.

The vast majority of patients with corneal blindness cannot recover their vision due to the serious shortage of donor cornea. However, the technology to construct a feasible corneal substitute is a promising treatment method for corneal blindness. In this paper, methacrylated gelatin (GelMA)-methacrylated hyaluronic acid (HAMA) double network (GHDN) hydrogels were prepared by modifying gelatin and hyaluronic acid with methacrylate anhydride (MA). GHDN hydrogel was compared with GelMA single network and HAMA single network hydrogels through characterization experiments of mechanical properties,

Repurposing of nifedipine loaded in situ ophthalmic gel as a novel approach for glaucoma treatment.

Glaucoma is a chronic eye disease characterized by elevated intraocular pressure (IOP) which causes severe complications to the eyes and may lead to vision loss. The effective treatment of such diseases motivated the search for novel and unique drugs and delivery systems. It has been reported that, nifedipine (NF) is effective in reducing the elevated IOP due to vasodilatation of eye vascular smooth muscles. NF loaded thermo-sensitive in situ gels were prepared by the cold method using poloxamer 407 (P407) and hydroxypropyl methyl cellulose (HPMC) polymers adopting Box-Behnken experimental des

Movement detection thresholds reveal proprioceptive impairments in developmental dyslexia.

Developmental dyslexia is associated with vision and hearing impairments. Whether these impairments are causes or comorbidities is controversial. Because both senses are heavily involved in reading, cognitive theories argue that sensory impairments are comorbidities that result from a lack of reading practice. Sensory theories instead argue that this is sensory impairments that cause reading disabilities. Here we test a discriminant prediction: whether sensory impairments in developmental dyslexia are restrained to reading-related senses or encompass other senses. Sensory theories predict that

Effect of MMP-9 gene knockout on retinal vascular form and function.

Retinal degeneration from inherited gene mutation(s) is a common cause of blindness because of structural and functional alterations in photoreceptors. Accordingly, various approaches are being tested to ameliorate or even cure neuroretinal blinding conditions in susceptible patients by employing neuroprotective agents, gene therapeutics, optogenetics, regenerative therapies, and retinal prostheses. The FVB/NJ mouse strain inherently has a common Pde6b rd1 homozygous allele that renders its progeny blind by the time pups reach weaning age. To study the role matrix metalloproteinase-9 (MMP-9) i
