Interlocking nailing is a widely accepted and performed treatment for tibial shaft fractures. The addition of percutaneously placed transfixation screws increases the stabilisation provided by intramedullary nailing; however, the technical complexity associated with the procedure has introduced new potential complications. We report a pseudoaneurysm of the anterior tibial artery caused by a proximal interlocking screw after intramedullary nailing surgery to repair a tibial shaft fracture. The patient experienced complete relief of symptoms following removal of the nail and the screws, excision

The purpose of this study was to use multivariate analysis to evaluate contributing factors affecting deep infection and nonunion of open femoral fractures treated with locked intramedullary nailing (IMN). We examined 89 open femoral fractures (88 patients) treated with immediate or delayed locked IMN in static fashion at the Kitasato University Hospital from 1988 to 2001. Multiple regression models were derived to determine predictors of deep infection and nonunion. The following predictive variables of deep infection were selected for analysis: age, sex, Gustilo type (I+II or III), fracture

PURPOSE OF THE STUDY: Fractures of the proximal humerus are increasingly frequent. Conservative treatment is most often proposed, but surgery must be performed when the displacement is significant and/or when the fracture is unstable. Osteoporosois and comminution are two essential elements for deciding on the surgical technique.
MATERIAL AND METHODS: This retrospective study included 31 patients who underwent pinning from the deltoid V according to the Kapandji procedure. There were 19 females and 12 males. Mean age was 61 years. There was a fracture of one of the tuberosities in 12 cases an

Palmar fracture dislocation of the proximal interphalangeal joint is uncommon. Thirteen patients treated for this injury were retrospectively reviewed. There were 9 acute injuries. Seven were treated by closed reduction and percutaneous pin fixation and 2 were treated by open reduction and internal fixation. The 4 chronic injuries (more than 1 month after injury) were treated with open reduction and soft tissue reconstruction. The length of follow-up averaged 55 months. Eight patients were free from pain. Postoperative proximal interphalangeal motion averaged 91 for the acute injuries and 70 d

A retrospective review of 162 displaced supracondylar fractures in children at Los Angeles County Harbor-UCLA Medical Center from 1975-1981 revealed 23 neural injuries. These injuries occurred in 19 patients whose ages ranged from 5-11 years of age. There were 12 radial, six ulnar, and five median neuropathies detected. Four of the ulnar nerve injuries and one radial nerve injury were iatrogenic, resulting from both percutaneous pinning and open reduction and internal fixation. All of the deficits resolved spontaneously within a range of 2-6 months. The average time to resolution for traumatic

Forearm fractures in children are among the most common fractures however isolated distal ulnar physeal fractures are rare, especially Salter-Harris type IV fracture involving epiphysis, physis and metaphyseal parts. Growth disturbances are not uncommon in such fractures. Also, certain anatomical variants, including cleft ulna epiphysis, may mimic fracture lines on radiographs, unnecessary operational intervention may result from misinterpretation. We describe 2 cases the first of a 12-year-old child who had sustained a bilateral wrist trauma after falling from a bicycle. A left distal forearm

Chronic osteomyelitis of the distal tibia following failed fracture fixation is a challenging condition because of limited soft-tissue coverage, compromised vascularity, persistent infection, and difficulty achieving stable reconstruction. We report the case of a 34-year-old female who developed chronic post-traumatic osteomyelitis of the distal tibia after failed initial management of a distal tibial fracture. She presented with persistent pain, swelling, inability to bear weight, and a discharging sinus. Radiographic evaluation showed persistent distal tibial fracture pathology and failure o

INTRODUCTION: Major orthopedic procedures place patients at risk for Deep venous thrombosis (DVT) and pulmonary embolism (PE). DVT has a 10-40% incidence after isolated fractures of the tibia and distal bones of the lower extremity. Diagnostic techniques are venous compression ultrasonography, venography, and pulmonary angiography. Prevention methods for venous thromboembolism (VTE) include mechanical prophylaxis and pharmacological prophylaxis. Inferior vena cava filter (IVCF) intercepts thrombus in inferior vena cava and prevents it from reaching the pulmonary artery.

PURPOSE: The purpose of this study was to compare healing time for diaphyseal tibia fractures (OTA/AO 42 A, B, C) treated with intramedullary nailing (IMN) in one geographic cohort using nonsteroidal anti-inflammatory drugs (NSAIDs) for post-operative pain control to that of another geographic cohort using opioid medications. The groups represent differing cultural approaches to post-operative pain control. We hypothesized there would be no difference in healing time.
METHODS: Tibia fractures presenting at two level I trauma centers located in different countries between January 1, 2010 and D

BACKGROUND: Favorable reports on the use of massive allografts to reconstruct intercalary defects underline their place in limb-salvage surgery. However, little is known about optimal indications as reports on failure and complication rates in larger populations remain scarce. We evaluated the incidence of and risk factors for failure and complications, time to full weight-bearing, and optimal fixation methods for intercalary allografts after tumor resection.
METHODS: A retrospective study was performed in all four centers of orthopaedic oncology in the Netherlands. All consecutive patients r

This article is intended to deliver three messages. First, minimally invasive methods of fracture repair are successful; when coupled with advances in implant design, these techniques yield higher union rates with fewer complications than prior methods of direct open fracture repair. Fracture repair has shifted away from direct, anatomic reconstruction of osseous surfaces and now emphasizes restoration of length, rotation, and alignment, with preservation of local soft tissues. The use of percutaneous plating and nailing techniques has expanded to many regions of fracture care. Although minima

The development of microvascular surgical techniques during the last quarter century has advanced the ability of the hand surgeon to reconstruct the traumatically amputated thumb. The use of tissue from the foot has become the mainstay of therapy for this previously exceedingly difficult reconstructive problem. Although numerous minor variations of thumb reconstruction with use of the toes from the foot are available, three main techniques--the complete great toe transfer, wraparound flap, and second toe transfer--provide a predictable outcome. With multiple donor sites available, the surgeon

BACKGROUND: Elbow fractures are among the most common types of fractures in children; an incision is inevitable when open surgery is performed. Prophylactic antibiotics was widely used in paediatric elbow fractures. This study is to investigate factors associated with postoperative infection in children undergoing open reduction and internal fixation with percutaneous pin fixation of supracondylar humeral fractures (SHFs). All patients did not receive prophylactic antibiotics during perioperative period.
METHODS: The clinical data of children with SHF who underwent open reduction and percutan

BACKGROUND: Subtrochanteric femur fractures (SFF) are uncommon, but have a high complication rate concerning non-union and mechanical complications. There is ongoing discussion about risk factors for delayed fracture healing after SFF. The purpose of this study was to evaluate potential risk factors for delayed fracture healing after SFF.
METHODS: This retrospective radio-morphometric case control study compares 61 patients after SFF in two groups (uncomplicated healing within 6 months postoperatively vs. delayed union) concerning radiographical properties. The patients were analyzed concerni

Distal tibial fractures with soft tissue damage are relatively difficult to treat. We assessed the outcomes of patients with these fractures treated with the Expert Tibial Nail® (DePuy Synthes, Raynham, MA) from March 2012 to December 2014. At 6 months postoperatively, the general health quality of patients was assessed using operative time, interval to return to work, American Orthopaedic Foot and Ankle Society ankle scale score, pain measured using a visual analog scale, and short-form health outcomes 36-item survey physical functioning and mental health dimension scores. Of 11 cases, 7 (63.
