Internuclear ophthalmoplegia in tuberculous meningitis. A report of two cases.

Internuclear ophthalmoplegia (INO) is characteristic of lesions of the medial longitudinal fasciculus and usually indicates primary brain stem pathology. Here we describe 2 cases of tuberculous meningitis (TBM) in which INO was a prominent presenting sign. The mechanism of INO in TBM is presumed to be ischemia secondary to involvement of small basilar vessels supplying the MLF in the inflammatory process. In both cases the INO resolved on antituberculous therapy suggesting that the cause of the ischemia is reversible. INO in a patient with meningitis must suggest a basal process probably TBM.

Surgical monitoring of spinal cord function: cauda equina stimulation technique.

Spinal cord and subcortical brain stem evoked potentials had an amplitude at least 2 times higher when the cauda equina rather than bilateral peripheral nerves was stimulated. Cauda equina stimulation is indicated when potentials to peripheral nerve stimulation are absent or are too low in amplitude to permit reliable surgical monitoring. The technique is essentially without risks, but should be performed with a small lumbar puncture needle (21 to 22 gauge), and is contraindicated in patients with general infections, increased cerebrospinal fluid pressure, or a hemorrhagic tendency (thrombocyt…

Intractable facial pain associated with a ganglioglioma of the cervicomedullary junction: report of a case.

A 6-year-old child with a brain stem tumor presented with the unusual complaint of intractable facial pain resembling trigeminal neuralgia in the absence of other symptoms or signs referable to the 5th cranial nerve. The radiological evaluation included a computed tomographic scan with intravenous contrast administration, which demonstrated an enhancing intramedullary lesion extending from the obex to C-4. After radiation and chemotherapy had failed to achieve symptomatic relief, the tumor, later proven to be a ganglioglioma, was radically removed with the ultrasonic aspirator. Postoperatively…

Cerebellar infarction--clinical and computed tomographic correlations.

Fifteen patients with CT evidence of cerebellar infarction were analyzed. In 10 patients, there were premonitory symptoms. In all cases, development to maximal neurological deficit occurred within 4 hr. All patients presented with gait disorder, and they had cerebellar abnormalities on examination. Eight of the 15 had accompanying findings consistent with brain stem dysfunction. CT showed a hypodense lesion which was either wedge, rectangular or ovoid shaped. Only 3 had evidence of fourth ventricular compression with accompanying third and lateral ventricular enlargement. No patient required s…

Total aganglionosis of the colon (Hirschsprung's disease) and congenital failure of automatic control of ventilation (Ondine's curse).

Total aganglionosis of the colon presenting with small intestinal obstruction in the neonatal period was observed in combination with congenital alveolar hypoventilation requiring continuous mechanical ventilation in a boy. The patient died aged 15 months from acute dehydration due to enteritis, long after total resection of the aganglionic bowel had been performed. Pulmonary hypertension was found in the newborn period. There was progressive right ventricular myocardial hypertrophy. This is the fourth case reported with a combination of defects involving nerve cell function of the brain stem …

Cerebral cysticercosis with aqueductal obstruction. Case report.

The authors report a case of cerebral cysticercosis which presented with generalized nonspecific neurological signs and symptoms attributed to acute aqueductal obstruction, with concomitant intracranial hypertension. These were characteristic intracranial calcifications along with angiographically demonstrated signs of hydrocephalus. Contrast encephalography clearly demonstrated aqueductal obstruction. Pathologically, the aqueductal obstruction was shown to be due to parasitic invasion of the brain stem with compression of the aqueduct. The presence of typical intracranial calcification in con…

Progressive dementia with "diffuse Lewy-type inclusions' in cerebral cortex. A case report.

A 69-year-old male suffering from progressive dementia died 3 years after the beginning of his disease. The neuropathology of this case revealed the coexistence of senile changes, typical for Alzheimer's disease, and the characteristics of Parkinson's disease, namely, numerous senile plaques and neurofibrillary tangles in the cerebrum and neuronal loss with depigmentation in the substantia nigra and locus caeruleus. Lewy-type inclusions were distributed not only in the pigmented brain stem nuclei, but also diffusely in the CNS. The close nosological relationship between paralysis agitans and A…

Cricopharyngeal achalasia associated with Arnold-Chiari malformation in childhood.

Six patients with myelomeningocele and the Arnold-Chiari malformation developed cricopharyngeal achalasia and lower cranial nerve deficits. Diagnosis is established by cine-esophagram. Distortion of the brain stem or cranial nerves secondary to the Arnold-Chiari malformation may produce the autonomic imbalance necessary for cricopharyngeal achalasia. Treatment is supportive and includes verification of cerebral spinal fluid shunt function. Suboccipital craniectomy may reverse progressive lower cranial nerve deficits and reduce cricopharyngeus spasm. Cricopharyngeal myotomy may be considered wh…

Central pontine myelinolysis. A clinical reappraisal.

We discuss three patients with clinical findings of central pontine myelinolysis (CPM). Two patients survived with minimal neurological deficits, and one patient died from medical complications while recovering from his neurological deficit. Postmortem examination showed the typical lesion of CPM. Brain stem auditory evoked potential studies in two of our patients indicate that there was impairment of function in auditory pontine pathways that returned to normal as the patients improved clinically. We conclude that CPM can be diagnosed clinically and that patients may recover if intercurrent i…

[Pharmacological effects of the new psychotropic preparation, mebikar].

By its effects mebicar -- a derivative of bicyclic bis-ureas -- may be refered to psychotropic drugs occupying a middle position between tranquilizers and neuroleptics. The drug inhibits the orientation reaction in albino mice, potentiates the action of narcotic hypnotics, abolishes the conditioned reflex reaction of avoidance, displays central adrenolytic activity, interferes with the norepinephrine metabolism in the brain stem and modifies the permeability of the blood-encephalic barrier, like this is done by the known neuroleptics. The substance is little toxic, does not exert any direct sp…

The role of the binaural test in filtered speech audiometry.

The value of the binaural resynthesis test administered according to Matzker's principle can be estimated only when a comparable monaural test can be used as a control. The most common positive finding in the filtered speech test is asymmetrical discrimination. Both parts of the message delivered to either ear must therefore have the same intelligibility in normal material. Binaural intelligibility is highly resistant to degenerative changes in the auditory system, for example. Positive findings in the binaural test are encountered frequently in brain stem lesions with vascular or traumatic ae…

Posterior fossa subdural haematoma associated with anticoagulant therapy.

This is a report of spontaneous posterioir fossa subdural haematoma associated with anticoagulation therapy. The possibility of posterior fossa lesions related to spontaneous haemorrhage is suggested by the combination of severe headache and increasing disturbance of consciousness associated with signs of brain-stem decompensation. A thorough neurological evaluation including appropriate contrast studies will help rule out a supratentorial lesion. This is a neurological emergency which can be successfully treated by early detection and prompt surgical decompression. This is the second reported

Cheiro-oral-pedal syndrome as the presenting symptom of brainstem cavernous malformation: a case report.

The rare cheiro-oral-pedal syndrome (COPS) is characterized by sensory disturbances around the corner of the mouth, and in the hand and foot of the same side. The causative lesion is located in the thalamocortical projections, thalamus or brainstem and is usually due to ischemic or hemorrhagic stroke. We report a case of a patient with brain stem cavernous malformations presented as pure COPS with additional sensory disturbance in the thorax. We report this case to raise awareness of these very rare syndromes and demonstrate that mildly presenting symptoms can be caused by an underlying devast…

[Surgical treatment of meningiomas of the craniovertebral junction].

The results of surgical treatment in 21 patients with meningiomas of the craniovertebral junction are presented. Main clinical diagnostic criteria in relation to the site of a tumor relative to the brain stem are considered. The basic surgical approaches used in the surgical treatment of meningiomas of the craniovertebral junction are described. These include posterior, median, posterolateral, transcondylar approaches. Surgical techniques to remove meningiomas having a matrix of varying areas were concretized, this is also done in relation to the consistency of a tumor. The outcomes of surgica…

Cerebral blood flow deficits in hereditary essential myoclonus.

Hereditary essential myoclonus is a disease in which segmental myoclonus is the sole clinical abnormality and whose cause is unknown. It is characterized by an early onset, a benign course, an autosomal dominant pattern of inheritance, the absence of any other neurologic dysfunction, and normal results of auxiliary tests. Cerebral blood flow studies of a father and son with this disease showed a cortical blood flow reduction contralateral to the myoclonus symptoms. We postulate the cause to be a focal unilateral subcortical cerebral lesion, either in the basal ganglia or in the brain stem, wit…

Rupture of a giant basilar aneurysm after saphenous vein interposition graft to the posterior cerebral artery. Case report.

A patient with a giant aneurysm of the top of the basilar artery presented with severe progressive symptoms of brain-stem compression. There was inadequate collateral circulation to the upper basilar system. She underwent exploration of the aneurysm and, after it was found to be impossible to clip, a tourniquet was placed on the basilar artery for future occlusion with the patient awake. A saphenous vein graft was interposed between the left external carotid and the left posterior cerebral arteries. The previously unruptured aneurysm bled fatally 36 hours after surgery, just before intended oc

[Postoperative courses in space occupying processes of the cranial base].

The postoperative complications of two collectives are compared: 100 patients with space-occupying processes in the base of the skull and 100 patients with mainly space occupying intracerebral processes. The first collective was exclusively, the second mainly operated on microsurgically. The group showing processes of the base of the skull shows a greater number and more frequently occurring complications than the comparative group. The relation to the brain stem as a cause of this fact is discussed. In all studied patients, no cerebellar stupor occurred. This seems to be in relation with the …

Causes of recurrent laryngeal nerve paralysis.

The recurrent laryngeal nerve in its course from the brain stem to the larynx follows a path that brings it in proximity to numerous structures. These structures can interfere with its function by pressure or by disruption of the nerve caused by disease invading the nerve. The various causes of recurrent laryngeal nerve paralysis that I have observed during the past 24 years are reported. In 134 patients with recurrent laryngeal nerve paralysis, the left recurrent nerve was most commonly involved. Malignant neoplasms of the lung and pulmonary tuberculosis were the most frequent causes of the p…

[Hypothalamus: some diagnostic principles].

The hypothalamus is a small group of receptors for visceral control, receiving influences from other parts of the nervous system. In turn, some hypothalamic nuclei send efferent impulses to the neurohypophysis, back along limbic pathways to the cortex and down to the autonomic centers in the brain stem. Not all the experimental observations on lower vertebrates can be applied to hypothalamic nuclear areas and connections in man. Therefore, knowledge of such functions and clinical syndromes as those relating to regulation of cardiovascular system, temperature, feeding, drinking, electrolyte bal…
