Improving the safety net for single mothers who face serious barriers to work.

Rebecca Blank explores a weakness of the welfare reforms of the mid-1990s--the failure of the Temporary Assistance to Needy Families program to address the plight of so-called "hard to employ" single mothers and their children. TANF has moved many women on the welfare caseload into work, but the services it provides are not intensive or flexible enough to meet the needs of women with multiple disadvantages who find it difficult to get and keep full-time employment. Blank notes that many of these women have lost welfare benefits because of their failure to find work. Increasingly, studies show

Screening for Depression and Suicide Risk in Children and Adolescents: US Preventive Services Task Force Recommendation Statement.

IMPORTANCE: Depression is a leading cause of disability in the US. Children and adolescents with depression typically have functional impairments in their performance at school or work as well as in their interactions with their families and peers. Depression can also negatively affect the developmental trajectories of affected youth. Major depressive disorder (MDD) in children and adolescents is strongly associated with recurrent depression in adulthood; other mental disorders; and increased risk for suicidal ideation, suicide attempts, and suicide completion. Suicide is the second-leading ca

Developmental dysplasia of the hip in children with Down syndrome: comparison of clinical and radiological examinations in a local cohort.

Guidelines for children with Down syndrome (DS) suggest to perform an annual hip screening to enable early detection of developmental dysplasia of the hip (DDH). How to perform this screening is not described. Delayed detection can result in disabling osteoarthritis of the hip. Therefore, we determined the association between clinical history, physical, and radiological examination in diagnosing DDH in children with DS. Referral centers for children with DS were interviewed to explore variety of hip examination throughout the Netherlands. Clinical features of 96 outclinic children were retrosp

Feasibility of a remote-care-system-based sibling peer event for young carers: a longitudinal pilot study.

Siblings of children with chronic conditions or disabilities (young carers) often take on caregiving roles yet remain largely outside family-support frameworks, and intervention research targeting them is scarce, particularly in Japan. This prospective longitudinal pilot study examined the feasibility of delivering an in-person sibling peer event (a one-day group gathering combining recreational activities, peer interaction, activities facilitating emotional expression, and crafts) embedded within a continuously operating online remote care system (RCS) and explored preliminary outcome pattern

Comparing the evidence for botulinum neurotoxin injections in paediatric anterior drooling: a scoping review.

Paediatric anterior drooling has a major impact on the daily lives of children and caregivers. Intraglandular botulinum neurotoxin type-A (BoNT-A) injections are considered an effective treatment to diminish drooling. However, there is no international consensus on which major salivary glands should be injected to obtain optimal treatment effect while minimizing the risk of side effects. This scoping review aimed to explore the evidence for submandibular BoNT-A injections and concurrent submandibular and parotid (i.e. four-gland) injections, respectively, and assess whether outcomes could be c

High Maternal Total Cholesterol Is Associated With No-Catch-up Growth in Full-Term SGA Infants: The Japan Environment and Children's Study.

OBJECTIVES: Infants born small for gestational age (SGA) with no catch-up growth (No-CU) are at high risk of intellectual and developmental disabilities. However, factors leading to No-CU among SGA infants are unclear. This study aimed to examine the association between maternal total cholesterol (TC) in mid-pregnancy and No-CU at 3 years among full-term SGA infants. STUDY DESIGN: The Japan Environment and Children's Study (JECS) is a nationwide prospective birth cohort study. We extracted a total of 2,222 mothers and full-term SGA infants (length and/or weight <-2 standard deviation [SD]) wi

Sleep problem screening of young children by speech-language pathologists: A mixed-methods feasibility study.

BACKGROUND & AIMS: Poor sleep in young children imperils language learning and use. Both sleep and language problems are prevalent in early childhood. Speech-language pathologists are in a unique position to expand surveillance of sleep problems, which in turn may contribute to communication difficulties. We conducted a feasibility study of speech-language pathologist screening for behavioral sleep problems and sleep-disordered breathing symptoms at a multidisciplinary evaluation and treatment center. METHODS: Speech-language pathologists administered screeners to parents of 2-6-year-olds: th

Three Malagasy Cases of Severe Bone Complications Revealing Primary Hyperparathyroidism.

Primary hyperparathyroidism (PHPT) is a metabolic disorder characterized by hypercalcemia with elevated or unsuppressed parathyroid hormone (PTH). It is rare in children but common in adults, particularly in women around the age of 50. In 85% of cases, PHPT is asymptomatic and is diagnosed following hypercalcemia and elevated PTH levels during routine examinations. PHPT occurring before the age of 25 and the normocalcemic phenotype are unusual situations that may delay diagnosis. These nonclassical forms expose patients to severe bone complications that can cause disability. Our objective is t

Neuroeducation and Genetics.

Nowadays, more and more educators are turning to neuroscience to be able to discover how children's brains and cognitive development are shaped by their learning experiences. This article discusses the link between neuroeducation and genetics. Several learning disabilities arise from specific genetic characteristics. The contribution of neuroscience is remarkable as it helps to understand the working mechanisms and organization of the brain, from the nerve cell to the integrated nervous system, to bridge the organic bases of behavior with their potential applications. In addition, research in

A history of oculogyric crises during the encephalitis lethargica pandemic.

A pandemic of what came to be known as encephalitis lethargica spread starting in the winter of 1916-1917 and continued into the 1930s. Neurological after-effects, namely permanent parkinsonian syndromes and various abnormal movements, permanently disabled the survivors of the one or two million victims, often children or young adults. Among them, a small proportion developed a symptom that was little known up to that point and that is currently exceptional: oculogyric crises; that is, a lateralised, dystonic upward movement of the eyes known as a tonic eye fit. This paper proposes a history o

Severe recurrent aphthous stomatitis treated with adalimumab: A case report in a teenage patient.

Recurrent aphthous stomatitis (RAS) is a chronic disorder of unknown etiology characterized by recurring ulcers involving the oral mucosa in patients with no other manifestations. It is a common cause of oral ulcers in children. In its major form, RAS can be a severe and disabling disorder due to pain and scarring, and poses a therapeutic challenge. Therapies for major RAS include prednisone, thalidomide, colchicine, and dapsone. However, many patients do not achieve adequate control of the disease with them (J Clin Diagn Res, 10, 2016 and ZE08). We report a case of severe RAS in a teenager wi

Syphilis in Children.

Syphilis, caused by Treponema pallidum, is transmitted both sexually and transplacentally. Untreated syphilis is a progressive disease that may result in death or disability in children and adults. Syphilis diagnosis requires 2-stage serologic testing for nontreponemal and treponemal antibodies. Congenital syphilis diagnosis requires careful review of maternal testing and treatment, comparison of maternal and neonatal nontreponemal antibody titers, and clinical evaluation of the neonate. In this review, we present the current epidemiology of syphilis, and the clinical manifestations, diagnosis

Health challenges in Tanzania: context for educating health professionals.

In this commentary we introduce the population age and gender distributions, the birth rate, and life expectancy in Tanzania's largely agricultural society and highlight the vulnerable status of mothers and children. We present underlying causes of poor health, the leading causes of Disability-Adjusted Life Years and review threats from exposure to disease, toxic substances, and injuries that require protection of populations and control efforts. We summarize health challenges posed by malaria, tuberculosis, and HIV/AIDS, non-communicable diseases, and by new threats that may change the pictur
