Effect of a Community-Based Program on Preschoolers' Physical Activity and Nutrition in Chile.

Introduction: Childhood obesity has reached critical levels in Chile, particularly among preschoolers from low-income families who face barriers to nutritious food and physical activity. Early interventions are essential to mitigate long-term health risks. This study evaluates the Chile Active Intervention, a community-based program promoting physical activity and healthy eating among preschoolers attending public daycare centers in Antofagasta, Santiago, and Temuco. Objective: To assess the effectiveness of a structured intervention in improving physical activity levels, dietary habits, and o

Severe community-acquired pneumonia (sCAP): advances in management and future directions.

Severe community-acquired pneumonia (sCAP) is a major global health challenge, with high morbidity and mortality, especially among patients requiring intensive care. Despite advancements in antimicrobial therapies and supportive care, sCAP remains a significant threat, particularly for those needing invasive mechanical ventilation or vasopressor support. Recent progress in diagnostics, therapeutics and management strategies offers hope for improved outcomes. Pathogen-specific management is now central to sCAP care, with molecular diagnostics enhancing pathogen detection accuracy and enabling t

Understanding pathways to recovery from alcohol use disorder in a Black community.

INTRODUCTION: Black Americans suffer a range of health disparities rising from a long history of structural inequities and racism. Black individuals experience alcohol use disorder (AUD) at rates comparable to the general population, yet they suffer more negative consequences due to alcohol use such as illnesses, injuries, criminal-legal involvement, and social problems. The barriers they face challenge their ability to achieve recovery. However, the recovery needs of the Black population and the potential impact of racial disparities on pathways to recovery have not been examined. METHODS: W

Inequity in access to medications among communities in Eastern Ethiopia: a decomposition analysis.

BACKGROUND: Access to affordable essential medicines, including antibiotics, remains a barrier to achieving universal health coverage in poorer nations because of financial and nonfinancial challenges. Despite evidence of disparities in healthcare access and associated poorer health outcomes in Ethiopia, no previous study has thoroughly investigated inequity in access to needed medications. Hence, we aimed to assess inequity in access to needed medications among communities in eastern Ethiopia. METHODS: We conducted a cross-sectional study among 2,256 households, which were sampled using a str

Perspectives on Community Geriatrics: An Applied Proposal From Colombia, South America.

This essay argues for the urgent need to redefine the role of geriatricians, as medical speciality, beyond hospital settings, emphasizing the importance of community geriatrics (CG) in Latin America. Despite global discussions on integrating CG in geriatric practice, its presence remains marginal in low-income countries, where older adults face systemic barriers to health care access. The essay calls for a discussion of CG within geriatric training model, which often prioritizes hospital-based care, and advocates for a community-based approaches to aging. Using a geriatric program in Colombia

Family communication and child behavior problems during the COVID-19 pandemic.

The COVID-19 pandemic posed challenges for many families, with wide-ranging implications for children's mental health. While much prior research has emphasized the developmental harms associated with the pandemic, this study examines how family communication may serve as a protective factor, buffering the impact of COVID-19-related stress on children's internalizing and externalizing problems. Participants included 144 predominantly Black, low-income, and economically marginalized mother-child dyads who were originally enrolled in a longitudinal study of stress and coping (2015-2019). During t

Community engagement in maternal and perinatal death surveillance and response: a realist review.

BACKGROUND: Community engagement in maternal and perinatal death surveillance and response (MPDSR) could support health systems in providing people-centred care and ensure accountability for the prevention of maternal and perinatal deaths. Although community engagement activities in MPDSR have been described, the literature does not adequately explain which community engagement in MPDSR strategies succeed, the contexts in which they work, the outcomes they produce, and for whom. METHODS: We conducted a realist review, which involved the identification and refinement of programme theories. An

Epidemiological Surveillance of Chagas Disease in Rural Communities of a Municipality in Piauí, Brazil.

Chagas disease, caused by Trypanosoma cruzi, remains a neglected tropical disease of public health importance in Latin America, particularly in rural areas. In Floriano, Piauí, Brazil, data on infection prevalence are limited. This cross-sectional population-based study assessed the seroprevalence of T. cruzi infection and analyzed sociodemographic factors associated with seropositivity among residents of 41 rural communities in Floriano. A total of 1212 individuals aged over six years were examined, revealing a seroprevalence of 3.2% (39/1212), with positive cases detected in 17 communities.

How community members in Uganda perceive controlled human infection studies.

Controlled Human Infection studies are a new and uncommon way of conducting clinical research in low resource settings. Regardless of the supposed importance of human infection studies in advancing clinical research, there is still limited literature on community perceptions of this field of research, particularly in low-resource settings. An exploratory qualitative study involving 107 community members organized into 16 focussed groups was conducted in Kampala city and Buikwe district in central Uganda between September 2023 and March 2024. The groups comprised market vendors, secondary schoo

Building Connection and Resident Understanding of Local Resources Through Community Engagement.

INTRODUCTION: Throughout graduate medical education (GME), it is crucial for learners to not only develop the skills necessary to manage a wide variety of medical conditions, but also to foster personal development and to gain a deeper understanding of the complex and multifaceted needs of our patients. We often refer patients to community sites to address needs such as homelessness, hunger, and domestic violence; however, we frequently make these referrals with only a superficial understanding of what each resource entails. METHODS: To address this issue, our department integrated a two-day

Community-based Suicide Interventions in Rural United States: A Scoping Review.

Rural communities in the United States (US) are disproportionately burdened with higher suicide rates than non-rural ones, facing structural and cultural barriers that make it less likely for suicidal individuals to obtain help. Community-based interventions have been called for to address the need for integrative approaches relevant to the rural landscape. To facilitate increased adaptation/implementation of rural suicide prevention programming, we conducted a scoping review of the literature to evaluate the state-of-the-science. Following PRISMA-ScR guidelines, we searched for and identified

Factors Associated With Clinician Confidence in Communication During Goals of Care Discussions.

Background: High-quality goals of care (GOC) discussions should be founded on shared decision-making (SDM) - eliciting patients' values and preferences while providing adequate information for patients to make decisions. One metric that can assess GOC discussion quality is the degree to which patients are adequately informed. Objective: Identify clinician GOC discussion attitudes and behaviors associated with clinician confidence that they conveyed adequate information to patients. Methods: We conducted a secondary analysis of national survey data from Veterans Affairs clinicians assessing att

Artificial Intelligence for Serious Illness Communication: Proactive Approaches to Mitigating Harm.

Serious illness communication is at the core of palliative care, aligning care with patient preferences and improving patient and family experience. Artificial Intelligence (AI) methods have increasingly been applied to palliative care and provide promising opportunities for measuring and enhancing communication (e.g., capturing speech patterns and delivering feedback). Yet, given known disparities in palliative care and the limitations afforded by our natural communication datasets, this task must be approached with caution. Focusing on the study of communication, we consider assumptions that
