To study alterations in glomerular permeability and their relation to immune deposits, anionic and cationic ferritins were systemically inje … lit pores, into the urinary space. When mice with immune complex disease were immunosuppressed with cyclophosphamide or methylprednisolone for 90 days, immune complexes were confined to the mesangium and glomerular structure and barrier (size- and charge-based) function were preserved.

Heat-killed Pseudomonas aeruginosa depresses contact sensitivity to oxazolone in C56BL/6 mice. The draining lymph nodes and spleens of mice … odes and spleen at 3 and 4 days after sensitization, respectively, and have precursors sensitive to cyclophosphamide. It is concluded that P. aeruginosa depresses contact sensitivity to oxazolone by enhancing the suppressor cell activity of the regulatory cells which arise during conventional sensitization.

Wegener's granulomatosis is treatable with steroids with or without cyclophosphamide; polymorphic reticulosis confined to one site responds to irradiation. Immunoglobulin light chain (AL) amyloidosis and POEMS syndrome are plasma cell proliferative disorders involving multiple organs and tissues, often presenting with nonspecific clinical manifestations. The co-occurrence of both conditions in a single patient is clinically rare and can easily lead to misdiagnosis or underdiagnosis. This report describes an elderly male patient who presented primarily with refractory heart failure and was ultimately diagnosed with coexisting AL amyloidosis and POEMS syndrome. The patient received 12 cycles of treatment with bortezomib, cyclophosphamide, and dexam…

Second branchial cleft cyst and lymphoma are two common types of neck masses. The etiology, clinical manifestations, imaging findings, treatment options, and prognosis of these two entities differ. Clinically, the occurrence of both lesions at the same anatomical site is rare. Herein, we report a case of concurrent second branchial cleft cyst and diffuse large B-cell lymphoma in the same anatomical region. The final diagnosis was confirmed by clinical features, ima-ging findings, and ultrasound-guided biopsy. The patient received the R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristin…

ABSTRACT: Immunoglobulin A (IgA) nephropathy is the most common primary glomerulonephritis. Oropharyngeal tularemia is a zoonotic infection … sy confirmed IgA nephropathy in the setting of acute kidney injury. Treatment with prednisolone and cyclophosphamide, in combination with tularemia-specific therapy, led to clinical improvement. This case highlights a possible association between tularemia and IgA nephropathy, warranting further investigation into the underlying mechanisms.

Prevention of graft-versus-host disease (GVHD) is critical to successful allogeneic hematopoietic cell transplantation (HCT), but for many years was difficult to achieve. Advances in the understanding of allogeneic HCT biology and immunology have paved the way for novel clinical approaches to GVHD prophylaxis, highlighted by the broad adoption of posttransplant cyclophosphamide and the approval of abatacept by the US Food and Drug Administration to prevent acute GVHD. Patients undergoing allogeneic HCT are now experiencing severe acute GVHD at historically low rates, and significant improvemen…

We present a case of a 19-year-old who developed nephrotic syndrome with preserved renal function. Renal biopsy confirmed focal segmental glomerular sclerosis (FSGS). No remission was achieved despite 2 years of treatment with glucocorticoids, mycophenolate mofetil, tacrolimus, and cyclophosphamide. After transfer to our center, we performed re-examination of renal pathology by electron microscope (EM), chromosomal karyotype, and gene analysis. EM revealed uneven thickness of the glomerular basement membrane without obvious stratification or fracture. Gene analysis revealed a splice mutation (…

In this case report, we describe a case of double-positive rapidly progressive glomerulonephritis among the Lebanese population. The patient, a 44-year-old female, presented with anemia and a recurrent right middle lobe pneumonia. She rapidly developed renal failure with her autoimmune workup being positive for both anti-glomerular basement membrane antibodies and anti-neutrophil cytoplasmic antibodies. The patient was successfully treated with plasmapheresis, intravenous cyclophosphamide, and oral prednisone. The patient regained her normal renal function and did not depend on dialysis for re…

Ankylosing spondylitis (AS) is a chronic inflammatory disease that typically affects the axial skeleton. Renal involvement is rare in AS, only occurring in 5-13% of AS patients. Membranous nephropathy (MN) in patients with AS is extremely rare. There have been only a few cases showing the association between MN and AS. Herein we report a case of AS-associated MN in a 47-year-old male. He was diagnosed with AS-associated MN after renal biopsy, and he was treated with corticosteroids and cyclophosphamide. His low back pain and oedema disappeared gradually and serum albumin and urine protein excr…

Primary cardiac lymphoma (PCL) is an exceedingly rare extranodal lymphoma type that typically presents with nonspecific symptoms such as arrhythmias or heart failure. Initial chemotherapy with the CHOP (cyclophosphamide, doxorubicin, vincristine, and prednisone) regimen was insufficient, necessitating the addition of rituximab and subsequent mycophenolate therapy to manage immune thrombocytopenia. Sympathetic ophthalmia is a rare and potentially devastating bilateral diffuse granulomatous panuveitis. It is caused by surgical or non-surgical eye injuries and is an uncommon and serious complication of trauma. It is diagnosed clinically and supported by imaging examinations such as ocular ultrasonography and optical coherence tomography. Its treatment consists of immunosuppressive therapy with steroids and sometimes steroid-sparing drugs, such as cyclosporine, azathioprine, cyclophosphamide, and mycophenolate mofetil. Fast and effective management with systemic immunosuppressive agents all…

Autologous haematopoietic stem cell transplantation for systemic scleroderma, developed over more than 25 years, has shown in three randomised controlled clinical trials a significant impact not only in event-free survival, overall survival, cutaneous and pulmonary involvement, but also in the quality of life of patients living with recent severe diffuse cutaneous systemic scleroderma, compared with IV cyclophosphamid despite a transplant-related mortality between 2.4 and 10%. No immunosuppressants or biologics have shown such an impact on mortality in this disease. The risk of relapse is esti

A 55-year-old woman had remarkably generalized telangiectasia. Large atypical lymphoid cells were found within the lumen of small vessels, and CD20 was positive in her third skin biopsy. She was diagnosed with intravascular large B-cell lymphoma (IVLBCL) and went into remission of the skin manifestations after seven courses of R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisolone) and four courses of intravenous high-dose methotrexate (HD MTX). To our knowledge, this is the first case report of IVLBCL with generalized telangiectasia in China. From a review of the lite…
