Afferent loop obstruction presenting as acute pancreatitis and pseudocyst: case reports and review of the literature.

Afferent loop obstruction after gastrectomy and Billroth II reconstruction is an uncommon problem. Complete acute obstruction requires emergent laparotomy. However, chronic obstruction may begin insidiously and its symptoms may reflect other gastrointestinal diseases. Two patients are described who developed acute abdominal pain, marked hyperamylasemia, and palpable abdominal masses 5 and 15 years after Billroth II gastrectomy. The masses were initially interpreted as pancreatic pseudocysts. Both patients were found to have chronically obstructed afferent limbs, and in one the obstruction was

Single-stage combined decompression for patients with tandem ossification in the cervical and thoracic spine.

STUDY DESIGN: Retrospective study of clinical outcomes of single-staged combined cervical and thoracic decompression for patients with tandem ossification (TO). OBJECTIVE: To describe primary clinical outcomes of this procedure. TO is introduced to described a double ossification lesion of the posterior longitudinal ligament (OPLL) or the ligament flavum (OLF) at the cervical, thoracic and lumbar spine. In clinical practice, cervical OPLL combined with thoracic OPLL or/and OLF are the most common types of TO. However, little is known about the clinical outcomes of surgical treatment and there

Transforaminal Percutaneous Endoscopic Discectomy and Foraminoplasty after Lumbar Spinal Fusion Surgery.

BACKGROUND: The most common causes of pain following lumbar spinal fusions are residual herniation, or foraminal fibrosis and foraminal stenosis that is ignored, untreated, or undertreated. The original surgeon may advise his patient that nothing more can be done in his opinion that the nerve was visually decompressed by the original surgery. Post-operative imaging or electrophysiological assessment may be inadequate to explain all the reasons for residual or recurrent symptoms. Treatment of failed lumbar spinal fusions by repeat traditional open revision surgery usually incorporates more exte

Late-presenting dural tear: incidence, risk factors, and associated complications.

BACKGROUND CONTEXT: Unrecognized and inadequately repaired intraoperative durotomies may lead to cerebrospinal fluid leak, pseudomeningocele, and other complications. Few studies have investigated durotomy that is unrecognized intraoperatively and requires additional postoperative management (hereafter, late-presenting dural tear [LPDT]), although estimates of LPDT range from 0.6 to 8.3 per 1,000 spinal surgeries. These single-center studies are based on relatively small sample sizes for an event of this rarity, all with <10 patients experiencing LPDT. PURPOSE: This investigation is the large

Frontal Bone Contouring and Forehead Lifting: A Retrospective Study on Aesthetic and Functional Outcomes.

BACKGROUND: Frontal bone contouring and forehead lifting are widely performed for aesthetic purposes. Emerging evidence suggests that these procedures may also alleviate frontal headache syndromes through mechanisms involving neurovascular decompression and soft tissue repositioning. OBJECTIVES: This study aimed to evaluate postoperative changes in headache severity following frontal bone contouring and forehead lifting and to determine the relationship between specific intraoperative maneuvers, including frontal recess widening and supraorbital nerve decompression, and clinical outcomes METH

Sagittal Alignment as a Determinant of Pain and Functional Recovery after Lumbar Surgery: A Systematic Review and Meta-Analysis.

BACKGROUND: Sagittal alignment is increasingly recognized as a key determinant of functional recovery after lumbar spine surgery; however, its role in degenerative lumbar disease remains variably reported. While alignment principles are well-established in adult spinal deformity, their applicability to routine degenerative procedures has not been quantitatively synthesized. METHODS: A systematic review and meta-analysis of 23 original clinical studies (1,837 adults) was performed. Eligible studies reported postoperative sagittal or spinopelvic parameters-pelvic incidence (PI)-lumbar lordosis

Laparoscopic duodenojejunostomy for recurrent acute pancreatitis associated with superior mesenteric artery syndrome in a medically complex patient: A case report and literature review.

RATIONALE: Superior mesenteric artery syndrome (SMAS) is a rare cause of duodenal obstruction, and acute pancreatitis associated with SMAS is extremely uncommon. The causal relationship is usually inferred from persistent duodenal obstruction, exclusion of other etiologies, and resolution after decompression. PATIENT CONCERNS: A 19-year-old male with a history of esophageal atresia repair, previous upper abdominal surgery, refractory epilepsy, severe developmental delay, and a bedridden status presented with recurrent vomiting and acute pancreatitis. Pancreatitis recurred over a period of 4 y

Incidental durotomy should not preclude same-day discharge from lumbar spine surgery.

BACKGROUND CONTEXT: An incidental durotomy is a common surgical complication that can occur during any spine surgery. Although rarely associated with sequelae requiring intervention, keeping patients who sustained durotomy for overnight observation is a common clinical practice. PURPOSE: To determine whether patients who sustained incidental durotomy during lumbar spine surgery can be safely discharged on the day of surgery. DESIGN: Retrospective cohort study. PATIENT SAMPLE: Patients who sustained incidental durotomy during elective lumbar microdiscectomy or decompression. OUTCOME MEASURE

Rates of achieving meaningful outcomes after undergoing minimally-invasive far lateral tubular decompression.

PURPOSE: To evaluate rates of achieving meaningful outcomes among patients undergoing far lateral tubular decompression (FLTD) for neuroforaminal stenosis. Traditional midline approaches are ineffective in treating isolated neuroforaminal stenosis. Direct decompression via a far lateral approach offers a minimally invasive, facet-sparing surgery with minimal surgical dissection. METHODS: Patients who underwent FLTD between January 2014 and January 2019 for isolated foraminal stenosis were included. The study collected patient demographics, perioperative data, and pre- and postoperative patien

Two-year follow-up of unilateral biportal endoscopy assisted extraforaminal lumbar interbody fusion: how to perform indirect decompression and fusion under endoscopy: a retrospective study in Japan.

STUDY DESIGN: Retrospective study. PURPOSE: To compare the clinical and radiographic outcomes of unilateral biportal endoscopy-assisted extraforaminal lumbar interbody fusion (BE-ELIF) and oblique lateral interbody fusion (OLIF). OVERVIEW OF LITERATURE: OLIF is widely recognized for its strong realignment capability, achieved through placing a large interbody cage, and its favorable clinical outcomes with indirect decompression. ELIF, similar to OLIF, does not entail exposure of the spinal canal. At our hospital, BE-ELIF involves removing the superior articular processes on both sides, inser

Comparison of functional outcomes and patient satisfaction in patients with lumbar canal stenosis with and without concomitant obesity following lumbar decompression surgery and their correlation with BMI.

BACKGROUND: Obesity is an important predictor of development of Lumbar canal stenosis (LCS) and its symptoms. LCS further restricts the mobility of the patient and a vicious cycle develops. The operative decision on patients with LCS and obesity is a matter of debate. Hence, we attempted to compare changes in body mass index and patient satisfaction in patients with LCS with and without concomitant obesity, undergoing spinal decompression surgery. METHODS: Patients undergoing decompression for LCS between 1st June 2019 to 31st May 2020 were included. Baseline characteristics including age, se

Lumbar Spinal Steroid Injections and Infection Risk after Spinal Surgery: A Systematic Review and Meta-Analysis.

Lumbar spinal steroid injections (LSSI) are universally used as preferred diagnostic or therapeutic treatment options before major spinal surgeries. Some recent studies have reported higher risks of surgical-site infection (SSI) for spinal surgeries performed after injections, while others have overlooked such associations. The purpose of this study is to systematically review the literature and perform a meta-analysis to evaluate the associations between preoperative LSSI and postoperative infection following subsequent lumbar decompression and fusion procedures. Three databases, namely PubMe

Comparison of intraoperative neuromonitoring accuracies and procedures associated with alarms in anterior versus posterior fusion for cervical spinal disorders: A prospective multi-institutional cohort study.

A prospective multicenter cohort study. To clarify the differences in the accuracy of transcranial motor-evoked potentials (TcE-MEPs) and procedures associated with the alarms between cervical anterior spinal fusion (ASF) and posterior spinal fusion (PSF). Neurological complications after TcE-MEP alarms have been prevented by appropriate interventions for cervical degenerative disorders. The differences in the accuracy of TcE-MEPs and the timing of alarms between cervical ASF and PSF noted in the existing literature remain unclear. Patients (n = 415) who underwent cervical ASF (n = 171) or PSF
