Patients with plant-induced dermatitis frequently present to the emergency department. The range of responses varies from irritant to allergic to photo or granulomatous. To a certain extent the clinical presentations will vary; it is important to determine causation because appropriate emergency therapy treatment also will vary. This review considers the most common forms of plant dermatitis and emphasizes advances in the understanding of pathomechanisms and recommended therapy for each type.

The skin is more susceptible to irritation when an active eczematous process is present. This reduced threshold to irritation occurs in skin distant from the site of the eczematous skin. Colour developers are widely used and contact dermatitis and lichen planus-like eruptions from them are well-known. As a result of automation, there have been few recent reports of contact dermatitis from colour developers. We describe here 4 cases of dermatoses from Kodak CD4 colour developer and study their cross sensitivity to other colour developers and PPD. Attempts were made to determine the purity of the patch test allergens to ensure that cross sensitivity was not the result of contamination and decomposition of the allergens.

Systemic absorption of metal or metallic salts from dental and orthopedic surgical implants can produce a cutaneous allergic dermatitis in susceptible individuals. Mercury, nickel and cobalt are the most common metals to elicit such systemic allergic reactions from chronic internal exposure. A case is presented of a generalized eczematoid dermatitis apparently caused by allergy to chromium liberated from a metal dental plate.

During a period of 2 1/2 years, 7 of 2000 patients routinely tested at our laboratory revealed contact allergy to oak moss in perfumes. All reacted to a mixture of different lichens and to some specific lichen compounds. The sensitivity was probably induced by cosmetics containing lichen substances. The following 3 compounds caused reactions in all patients tested: atranorin, evernic and usnic acids. 3 patients were photosensitive, but stronger reactions were elicited by prolonged contact during occlusion of the patches and complete protection against light, rather than by irradiation alone.
