A new tool for testing ossicular mobility during middle ear surgery: preliminary report of four cases.

OBJECTIVE: We developed an ossicular vibration tester for the objective and quantitative assessment of ossicular mobility, which is one of the most critical factors affecting postoperative hearing after tympanoplasty. METHODS: Our device consists of three components: a probe shaft with a curved tip to be attached to the target ossicle, a vibration exciter to activate the probe, and a piezoelectric sensor to detect vibrations of the probe. These components are encased in a stainless steel holder, allowing easy hand manipulation during ear surgery. The probe is activated with an electric signal…

Middle ear tumours with brainstem extension treated by ventral bulla osteotomy and craniectomy in two cats.

Two geriatric domestic shorthaired cats (DSH) were treated surgically with a ventral bulla osteotomy and craniectomy for middle ear tumours that invaded the calvarium. Both cats had a history of vestibular disease. One cat had a normal neurological examination. Both cats underwent computed tomographic imaging of the head. After intravenous injection of an iodinated contrast material, one cat had a ring-enhancing intracranial lesion and the other had a uniform contrast-enhancing intracranial lesion, which either communicated with the bulla or was associated with bulla osseous lysis/production. …

Three-dimensional fast recovery fast spin-echo imaging of the inner ear and the vestibulocochlear nerve.

The aim of this study was to assess the performance of three-dimensional fast recovery fast spin-echo (3DFRFSE) for imaging of the inner ear as well as the facial and vestibulocochlear nerves. We evaluated 3DFRFSE sequences, comparing it with 3D fast spin-echo (3DFSE) in a water phantom and in 12 normal volunteers. We also examined 66 patients using 3DFRFSE sequence and assessed the visualization of their pathologies. In a water phantom study, signal intensity (SI) on 3DFRFSE was higher than that on 3DFSE at the same TR ranging from 1,500 to 6,000 ms. In normal volunteers, 3DFRFSE with TR of 2…

Function-morphological investigations of fish inner ear otoliths as basis for interpretation of human space sickness.

In man, altered gravity may lead to a vestibular dysfunction causing space motion sickness. A hypothesis was developed, according to which asymmetric inner ear statoliths might be the morphological basis of space sickness. The animal model, fish, revealed further information: inner ear "stone" (otolith) growth is dependent on the amplitude and the direction of gravity, regulated by a negative feedback mechanism. The present study was focused on the question, where the regulation centre of adaptive otolith growth may be situated. Therefore, the vestibular nerve was unilaterally transected in ne…

Targeted mutagenesis of the POU-domain gene Brn4/Pou3f4 causes developmental defects in the inner ear.

Targeted mutagenesis in mice demonstrates that the POU-domain gene Brn4/Pou3f4 plays a crucial role in the patterning of the mesenchymal compartment of the inner ear. Brn4 is expressed extensively throughout the condensing mesenchyme of the developing inner ear. Mutant animals displayed behavioral anomalies that resulted from functional deficits in both the auditory and vestibular systems, including vertical head bobbing, changes in gait, and hearing loss. Anatomical analyses of the temporal bone, which is derived in part from the otic mesenchyme, demonstrated several dysplastic features in th…

Moraxella catarrhalis-induced purulent otitis media in the rat middle ear. Structure, protection, and serum antibodies.

To study the effects of viable and heat-killed Moraxella catarrhalis bacteria on the middle ear mucosa and to evaluate the protection after whole-cell immunizations, Sprague-Dawley rats were challenged and rechallenged with four different M. catarrhalis strains. The animals were monitored by clinical observations, bacterial and histological samples from middle ears, and serum IgG levels. Only viable bacteria at a high concentration induced purulent otitis media, which was culture positive in 58% of the cases on day 4. The infection was characterized by a mild acute reaction lasting otomicrosco…

[Hair cell regeneration in a chicken's inner ear after damage due to exposure to industrial noise].

In mammals, the damage to the hair cells of the inner ear due to exposure to noise or other ototoxic agents is irreversible. In fish, reptiles and birds, however, the hair cells may regenerate, probably from the supporting cells. This regeneration process in the inner ear is being intensively examined in animals in the hope of curing the sensorineural hearing loss in human subjects in future. The aim of the study was to assess hair cell regeneration in the inner ear of chicks after exposure to industrial noise, depending on the level of exposure. The birds were exposed either to the noise at t…

[Group B streptococcal meningitis revealing an inner ear congenital malformation. A case report in an adult].

INTRODUCTION: Group B streptococcal meningitis is unusual in adults. It occurs in patients with a chronic disease. EXEGESIS: We report a case of group B streptococcal meningitis that occurred in a 32-year-old woman, uncovering inner ear congenital malformation associated with cerebrospinal fluid fistula. There was no recurrent episode of meningitis during a 2-year period following surgical correction of the malformation. CONCLUSION: Clinical signs suggesting cerebrospinal fluid fistula should be considered in any adult patient with severe group B streptococcal infection.

Feedback control of the auditory periphery: anti-masking effects of middle ear muscles vs. olivocochlear efferents.

Both MEM and MOC systems are sound-evoked reflexes to the auditory periphery which can be elicited by sound in either ear. Both MEM and MOC systems can increase thresholds in the auditory periphery: the MEM system acts by stiffening the ossicular chain, the MOC system by decreasing outer hair cell amplification of sound-induced motion in the inner ear. MEM-induced attenuations are largest for low frequency stimuli, MOC-induced attenuations are largest for mid- to high-frequency sounds. Both MEM and MOC systems can have anti-masking effects. The MEM reflex can decrease the masking of high-frequ…

Inner ear blood flow in the rat after unilateral arterial occlusion in the vertebrobasilar arterial system.

It is generally accepted that certain kinds of vertigo and hearing disturbances are caused by blood flow insufficiency in the vertebrobasilar arterial system. Using the microsphere method we investigated whether unilateral vertebral artery or unilateral posterior inferior cerebellar artery occlusion could cause an imbalance between right and left inner ear blood flow in rats. We also studied the differential vulnerability between blood flow in the cochlea and in the ampullae of the three semicircular canals. We counted the numbers of microspheres distributed to the cochlea (CO) and microsphere…

[First implantation of a totally implantable electronic hearing aid in patients with inner ear hearing loss].

For the majority of patients with sensorineural hearing loss (SNHL) many available hearing aids often do not achieve satisfactory results. For these patients partially implantable hearing devices have been developed, allowing distortion-free hearing and speech intelligibility that may be superior to conventional hearing aids. The external parts of partial implants, however, may result in a patient's stigmatization. Furthermore, they do not use the acoustic properties of the external auditory canal. Recently, we published the successful development of the first totally implantable hearing devic…

High resolution 3D imaging of the inner ear with a modified fast spin-echo pulse sequence.

A fast spin-echo pulse sequence is described that produces high resolution images of the inner ear without susceptibility artifacts. It uses thin overlapping slices that can be reformatted in multiple projections to provide a view of the 3D geometry of inner ear structures, such as the cochlea, vestibule, semicircular canals, and internal auditory canal. It has proven useful in screening adults for the presence of acoustic schwannoma and in identifying structural congenital lesions in children with sensorineural hearing loss.
