Double muscle transfer for upper extremity reconstruction following complete avulsion of the brachial plexus.

Recent interest in reconstruction of the upper limb following brachial plexus injuries has focused on the restoration of prehension following complete avulsion of the brachial plexus. The authors use free muscle transfers for reconstruction of the upper limb to resolve the difficult problems in complete avulsion of the brachial plexus. This article describes the authors' updated technique--the double free muscle procedure. Reconstruction of prehension to achieve independent voluntary finger and elbow flexion and extension by the use of double free muscle and multiple nerve transfers following …

Effects of dehydration on isometric muscular strength and endurance.

To examine the effects of rapid dehydration on isometric muscular strength and endurance, seven men were tested at baseline (control) and after a dehydration (dHST) and a euhydration (eHST) heat stress trial. The dHST consisted of intermittent sauna exposure until 4% of body mass was lost, whereas the eHST consisted of intermittent sauna exposure (same duration as dHST) with water replacement. Peak torque was determined for the knee extensors and elbow flexors during three isometric maximal voluntary contractions. Time to fatigue was determined by holding a maximal voluntary contraction until …

[Significance of the radial compression syndrome for the diagnosis and surgical therapy of so-called epicondylitis humeri radialis (Epic. hum. rad.)].

25 patients with Tennis Elbow were examined after surgery (= retrospectively), 25 before surgery (= prospectively). All of the patients were operated on according to the Wilhelm and Wachsmuth method. 5-18 months after surgery (on average 10), both groups underwent clinical examinations. Those patients who still had symptoms were given neurophysiological examinations. Both groups were compared with each other. The following results were obtained from the comparison: 1) 7 patients (28%) in the retrospective group were dissatisfied with the results of surgery and still had symptoms specific to th…

CONGENITAL PROXIMAL RADIOULNAR SYNOSTOSIS: RADIOGRAPHIC FEATURES IN TWO CASES WITH REVIEW OF THE LITERATURE.

BACKGROUND: Congenital radioulnar synostosis (CRUS) is an uncommon developmental defect caused by failure of segmentation between the radius and ulna during early embryonic life. Despite its rarity, it is the most prevalent congenital functional abnormality affecting the elbow. Clinical presentation varies widely, ranging from subtle cosmetic concerns to significant restriction of forearm rotation with consequent limitations in activities of daily living. CASE 1: A 4-year-old girl presented with inability to fully extend her left elbow since birth, accompanied by progressively worsening diffi…

Semi-automated algorithm for rotational axis determination of the ulno-humeral joint.

Preoperative simulation of the elbow joint motion has been proposed for prosthesis alignment, ligament reconstruction or treatment of impingement-inducing osteophytes. However, its daily application remains seldom. This study proposes an algorithm to simulate the ulno-humeral joint motion in flexion/extension. Four observers placed reference points on 3D surface models of elbows. The algorithm generated five spheres representing specific joint surface: medial and lateral trochlea humeri (MT and LT), capitellum (CAP), medial and lateral trochlear notch (MED- and LAT NOTCH). Three rotational axe…

Long-term outcomes and complications of titanium transcondylar screw placement to address humeral intracondylar fissures in Spaniel breed dogs.

OBJECTIVE: To describe the outcome and complications of titanium transcondylar screws to treat humeral intracondylar fissures in Spaniel breed dogs. ANIMALS: 39 dogs (55 elbows). Medical records (2013 to 2021) of dogs treated for humeral intracondylar fissure were reviewed. Dogs presenting with a humeral fracture on the same side or contralateral side were excluded. Collected data included signalment, lameness score, investigations, surgical technique, and complications. Long-term outcomes were assessed via an owner questionnaire, orthopedic examination, measurement of elbow goniometry, and p…

Neuromechanics of finger hangs with arm lock-offs: analyzing joint moments and muscle activations to improve practice guidelines for climbing.

INTRODUCTION: Climbing imposes substantial demands on the upper limbs and understanding the mechanical loads experienced by the joints during climbing movements is crucial for injury prevention and optimizing training protocols. This study aimed to quantify and compare upper limb joint loads and muscle activations during isometric finger hanging exercises with different arm lock-off positions. METHODS: Seventeen recreational climbers performed six finger dead hangs with arm lock-offs at 90° and 135° of elbow flexion, as well as arms fully extended. Upper limb joint moments were calculated usi…

Classification system for partial distal biceps tendon tears: a descriptive 3-Tesla magnetic resonance imaging study of tear morphology.

BACKGROUND: There is minimal literature on the morphology of partial distal biceps tendon (DBT) tears. We sought to investigate tear morphology by retrospectively reviewing 3-Tesla magnetic resonance imaging (3T MRI) scans of elbows with partial DBT tears and to propose a basic classification system. METHODS: 3T MRI scans of elbows with partial DBT tears were retrospectively reviewed by two experienced observers. Basic demographic data were collected. Tear morphology was recorded including type, presence of retraction (>5 mm), and presence of discrete long-head and short-head tendons at the D…

Isolated trochlea osteochondral fracture of the distal humerus in a 6-year-old patient surgically fixed using biodegradable pins: A case study with over 3 years of follow-up.

Isolated humeral trochlea fracture, particularly in skeletally immature children, is extremely rare. The exact mechanism of this injury remains unknown because the humeral trochlea is embraced within the olecranon fossa without any muscular or ligamentous attachment. We report the treatment of a 6-year-old boy who sustained an isolated trochlea osteochondral fracture when he fell with his elbow in a flexed position while skateboarding. The patient had a history of lateral condyle fracture of the ipsilateral humerus one year previously. At the diagnosis, computed tomography (CT) revealed a smal…

Gray-Scale and Power Doppler Ultrasound Findings Predictive of Cubital Tunnel Syndrome Severity.

BACKGROUND: The use of ultrasound in the diagnosis of cubital tunnel syndrome (CuTS) is an attractive alternative to electrodiagnostic (EDX) studies, but its utility is binary with poor severity correlation. We hypothesize that increasing ulnar nerve cross-sectional area (CSA) and power Doppler measurement of intraneural vascularity may predict the extent of disease. METHODS: We identified 20 elbows from patients with a history of CuTS and 20 elbows in 10 asymptomatic controls. Electrodiagnosis was performed for symptomatic patients. Gray-scale ultrasound and power Doppler ultrasound were per…

Tenodesis with bone marrow venting under local anesthesia for recalcitrant lateral epicondylitis: results of 2 years of follow-up.

HYPOTHESIS: We hypothesized that the treatment of recalcitrant lateral epicondylitis requires accurate identification of the painful area to promote remodeling of the degenerated extensor insertion and to stabilize the tendon origin during tendon healing. Thus, we performed tenodesis with bone marrow venting under local anesthesia for recalcitrant lateral epicondylitis. METHODS: Twenty patients (21 elbows) were treated with bone marrow venting at the painful area of the lateral epicondyle of the elbow and tenodesis using 2 soft anchors lateral to the capitellum (immediately distal to the pain…

Three Dimensional Motion Analysis of Hand Tremors During Endoscopic Ear Surgery.

BACKGROUND: Endoscopic surgery is developing in various clinical specialties. During ear endoscopic surgery, a surgeon has to hold an endosc … minant hand and manipulated it using three different stabilization techniques i.e. with resting the elbow on the table, resting the endoscope on the ear canal, both with the elbow on the table and endoscope on the ear canal. For the control, subjects were made to use the endoscope without any stabilization technique. The endoscopic movement was measured …

Simultaneous bilateral ulnar neuropathy: an unusual complication caused by neuroleptic treatment-induced tardive dyskinesia: A Case Report.

RATIONALE: In the past decade, only a few studies have focused on simultaneous bilateral ulnar neuropathy. PATIENT CONCERNS: A 54-year-old Asian male who has suffered from paranoid schizophrenia for 2 years. He reported that flexion contracture occurring over his fourth and fifth fingers on both hands appeared since six months after he started taking the antipsychotic drug. The electromyogram revealed bilateral ulnar neuropathy with chronic axonal degeneration at the elbow level. McGowan classification was performed to evaluate the severity of the ulnar nerve injury, and the patient was diagn…
