Down syndrome and juvenile rheumatoid arthritis have been associated with ligament laxity, specifically the atlantoaxial ligament, which maintains the proper positioning of the cervical first and second vertebrae. Angiokeratoma of the vulva is relatively uncommon in the general population. We present two cases of angiokeratoma of the vulva and review the literature. The two patients presented with the complaint of vulvar pruritus. They underwent wide local excision of the lesions. Histopathology revealed angiokeratoma of the vulva. The women remain well with no evidence of recurrence 48 and 32 months after initial surgery. Although it is a rare disease, angiokeratoma of the vulva should be included in the differential diagnosis of a vulvar tumor.

This article presents a cohort review of anesthesia-related perioperative outcomes of children undergoing ambulatory urologic surgery using a combination of general and regional anesthesia. We analyzed the charts of 123 patients who underwent hypospadias repair and circumcision between July 1,2006, and January 2, 2009, for cases of postoperative nausea and vomiting. We found the incidence to be quite low. We believe the low incidence may have been related to the prophylactic use of antiemetics along with an opioid-sparing technique for anesthesia care.

A laryngocele is a rare benign dilatation of the anterior part of Ventriculus Laryngis. The symptoms are either hoarseness, a tumor on the neck or a combination. In the literature a link between laryngocele and carcinoma of the larynx is well described. For this reason a careful clinical examination together with MRI or CT scans is of outmost importance. None of the patients described had larynx cancer. Of the two patients described, one underwent surgery, the other was treated conservatively due to general health.

Early results (12 weeks after surgery) of the lumbar spinal stenosis operative treatment were assessed in the group of 36 patients (aged 40-65 years). In 72% of patients posterior fusion was applied. In the results evaluation lumbar pain, low extremities pain, neurogenic claudicat ion, sensory disturbances and functional status were taken in account. A significant decrease in low extremities and lumbar pain was observed. Some sensory disturbances after operative treatment did not affect general high patients assessment of the performed surgical procedures (positive 80.5% cases).

We present a case of peri-orbital surgical emphysema following functional endoscopic sinus surgery (FESS), due to extubation with simultaneous manual inhalation with a closed expiratory valve. This complication has not been previously reported in association with general anaesthetic and FESS. Patients undergoing FESS are advised against nose blowing. However, we recommend that the anaesthetist working with a surgeon who performs FESS should also be warned not to use manual inhalation with a closed expiratory valve while extubating, as this can cause peri-orbital surgical emphysema.

The 60-year-old women who complicated by multiple sclerosis was referred to our hospital under diagnosis of mitral valve stenosis and tricuspid valve regurgitation. The mitral valve replacement and the tricuspid valve annuloplasty were performed under cardiopulmonary bypass. In the state of multiple sclerosis, even an operation by general anesthesia could become a cause of abrupt change, but by appropriate management during and after surgery, the post operative course was uneventful without any complications, and the patient discharged on the 35th post operative day.

Post-traumatic enophthalmos has a complex pathogenesis, resulting from injuries to the bony walls of the orbit or to the intraorbital contents. Computed tomography allows specifying this pathogenesis by locating displaced bone, fat leakage or muscle incarceration, as well as calculating the volumes of the orbit and of the soft tissues. This assessment guides surgery (most often bone grafts), but correction remains limited by scar retraction, especially around the orbit. The prognosis of enophthalmos mainly depends on a general initial treatment after the trauma.

We describe a 14-month-old male patient with congenital agranulocytosis who received general anaesthesia on three separate occasions during a 6-week period for minor surgery. Granulocyte colony stimulating factor was commenced after the second anaesthetic. Each anaesthetic was followed by profound reductions in neutrophil numbers, irrespective of the agent used. Even the third anaesthetic, which avoided all the common agents thought to have a marrow suppressant effect and given during granulocyte colony stimulating factor therapy, was associated with a marked decrease in neutrophil numbers.

Following inadvertent dural puncture during epidural catheter placement, a 20 gauge polyethylene catheter was placed in the intrathecal space, and continuous spinal anesthesia with hyperbaric bupivacaine was administered intraoperatively to supplement general anesthesia. Tuberculosis is a disease that has plagued humankind for centuries. The "white plague" is not only treatable and curable but also preventable. Initially, tuberculosis fell in the province of the general physician. With the advent of technologic advances in thoracic surgery, surgical management of tuberculosis was brought to the forefront. Effective bactericidal drug therapy became available by 1954 after the development of streptomycin in 1945 and isoniazid in 1952. Following a brief overview of the general features of the horseshoe kidney, we report on ten patients diagnosed as having this anomaly and renal calculi. One patient had bilateral renal calculi. The clinical cases are described and treatment with extracorporeal shock wave lithotripsy (ESWL1) and its advantages and disadvantages relative to other treatment modalities (PCN, open surgery) are discussed. Although we do not advocate the use of a single therapeutic approach, our results show the usefulness of ESWL1 in the treatment of this frequently recurring condition.

The frequency of postoperative venous thrombosis and the prophylactic effect of dextran 40 and 70 were studied in 90 otherwise healthy patients undergoing cholecystectomy. The 125-I-fibrinogen test was employed as the diagnostic method. The frequency of thrombosis was very low (1.1%) compared with other investigations on patients undergoing elective general surgery. Thus nothing can be concluded about the prophylactic effect of dextran from this series, but it can be presumed that this type of patient does not need routine prophylaxis against deep venous thrombosis.
