HIV preventive behavior scale for Thai men who have sex with men (MSM): development and psychometric properties.

Background In foreign countries, many scales have been developed to measure HIV prevention behavior. However, there are only a few developed scales among MSM in Thailand and they are not up to date. The objective of this study is to examine the reliability and validity of the HIV preventive behavior measure among Thai men who have sex with men (MSM). Methods The sample comprised 424 Thai MSM aged 25 years or older who had at least one sexual encounter in the past six months. The total sample was randomly divided into two equal subsamples: one used for exploratory factor analysis (EFA) to ident

The cost of a late-detected outbreak among people who inject drugs. A modeling study.

BACKGROUND: People who inject drugs (PWID) are at risk for human immunodeficiency virus (HIV) and hepatitis C virus (HCV). In 2009 and 2011, Athens, Greece experienced an HCV and an HIV outbreak among PWID, respectively. Of these, only the 2011 HIV outbreak was detected. However, the public health interventions implemented in response to the HIV outbreak tackled also indirectly the undetected HCV outbreak. The aim of this study is to highlight the potential benefits of an efficient notification system using as a case study the undetected 2009 HCV outbreak among PWID of Athens. More specificall

Genotypic Characterization of Cryptosporidium Species in Humans and Peri-Domestic Animals in Ekiti and Oyo States, Nigeria.

Cryptosporidiosis is one of the leading causes of diarrhea in humans and several other vertebrate species. Because surveys of Cryptosporidium genotypes from animals and humans living in the same region are rare, our understanding of the importance of zoonotic transmission in the epidemiology of cryptosporidiosis remains superficial. PCR was used to amplify a portion of the Cryptosporidium 18S small subunit ribosomal RNA gene from fecal DNA from humans and livestock living in Ekiti and Oyo states, Nigeria. PCR-positive samples were further analyzed using PCR targeting the heat-shock protein HSP

Evaluation of a diagnostic algorithm for sputum smear-negative pulmonary tuberculosis in HIV-infected adults.

BACKGROUND: The Revised National TB Control Program bases diagnosis of tuberculosis (TB) on sputum smear examination and response to a course of antibiotics, whereas World Health Organization recommends early chest radiography [chest x-ray (CXR)] for HIV-infected symptomatic patients. We evaluated the utility of initial CXR in the diagnostic algorithm for symptomatic HIV-infected patients with negative sputum smears. METHODS: HIV-infected ambulatory patients with cough or fever of ≥2 weeks and 3 sputum smears negative for acid-fast bacilli were enrolled in Chennai and Pune, India, between 200

A mathematical model for temperature effects on virus-mediated cell fusion.

Syncytia are formed by fusion of multiple cells into a multi-nucleated cell body. Viral-induced syncytia formation is used by certain viruses to propagate without leaving the cell. The ability to propagate without leaving the cell allows viruses to enhance replication and evade some of the host's immune responses. Currently, it is not known how temperature affects syncytia formation rate. This study uses a cell-to-cell fusion mathematical model to analyze data from previously published cell-cell fusion assays of HIV and SARS-CoV-2 at various temperatures. Parameters were estimated via minimiza

A late HIV diagnosis in the age of COVID-19: Urging screening awareness.

Global health emergencies such as the COVID-19 pandemic have revealed the fragility of medical infrastructure in the US and elsewhere. Specifically, the pandemic insidiously targeted the relationship between public health and medicine by affecting the screening for and prevention of communicable diseases, such as HIV. This case report focuses on a patient with a problematic and late HIV diagnosis amid the pandemic and discusses HIV history, pathogenesis, epidemiology, and current screening guidelines. Although the negative implications of the pandemic are yet to be fully unraveled, health prof

The outcomes of cryptococcal disease in HIV-positive individuals following COVID-19 infection: A systematic review and meta-analysis.

BACKGROUND: Cryptococcal disease is considered a major cause of morbidity in individuals with HIV in resource-limited settings. The long-term effects of COVID-19 and cryptococcal coinfection among people living with HIV (PLWHIV) have not been thoroughly investigated. This study examined the incidence of cryptococcosis among HIV-positive individuals following COVID-19. METHODS: A thorough search was conducted across five databases on November 14, 2023, and updated on May 7, 2024. Observational and case reports on the clinical and pathological outcomes of cryptococcosis in HIV-positive individu

Leveraging PEPFAR- and Global Fund-Supported Laboratory Network for Integrated Disease Testing: Lessons From Integrated HIV, Tuberculosis, and COVID-19 Testing in Nigeria.

The Nigeria Centre for Disease Control and Prevention, as part of the strategic approach to the COVID-19 outbreak response in Nigeria, coordinated the implementation of an integrated disease testing (IDT) strategy with support from multiple partners. This involved adaptation of existing US President's Emergency Plan for AIDS Relief (PEPFAR)- and Global Fund-supported molecular laboratory networks and integration of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) testing into these. This enabled the country to provide large-scale testing and rapid return of results for SARS-CoV-2 f

Management of Psoriasis Patients with Serious Infectious Diseases.

The management of patients affected by moderate-to-severe psoriasis may be challenging, in particular in patients with serious infectious diseases [tuberculosis (TB), hepatitis B and C, HIV, COVID-19]. Indeed, these infections should be ruled out before starting and during systemic treatment for psoriasis. Currently, four conventional systemic drugs (methotrexate, dimethyl fumarate, acitretin, cyclosporine), four classes of biologics (anti-tumour necrosis factor alpha, anti-interleukin (IL)12/23, anti-IL-17s, and anti-IL-23], and two oral small molecules (apremilast, deucravacitinib) have been

COVID-19 immune signatures in Uganda persist in HIV co-infection and diverge by pandemic phase.

Little is known about the pathobiology of SARS-CoV-2 infection in sub-Saharan Africa, where severe COVID-19 fatality rates are among the highest in the world and the immunological landscape is unique. In a prospective cohort study of 306 adults encompassing the entire clinical spectrum of SARS-CoV-2 infection in Uganda, we profile the peripheral blood proteome and transcriptome to characterize the immunopathology of COVID-19 across multiple phases of the pandemic. Beyond the prognostic importance of myeloid cell-driven immune activation and lymphopenia, we show that multifaceted impairment of

[Issues of morphological diagnosis and pathogenesis of tuberculosis].

Tuberculosis remains a serious global problem of our time. The epidemiological situation regarding tuberculosis in the Russian Federation and in Moscow is quite favorable, however, many manifestations of diseases and morphological changes in tuberculosis require a serious approach to the timely diagnosis of the disease, especially intravital morphological verification of the process. The article outlines the main aspects of the pathogenesis of tuberculosis, including deep immunosuppression associated with HIV. A typical microscopic picture of tuberculosis inflammation is described with an algo

Pandemics and Burden of Stroke and Epilepsy in Sub-Saharan Africa: Experience from a Longstanding Health Programme.

Eighty percent of people with stroke live in low- to middle-income nations, particularly in sub-Saharan Africa (SSA) where stroke has increased by more than 100% in the last decades. More than one-third of all epilepsy-related deaths occur in SSA. HIV infection is a risk factor for neurological disorders, including stroke and epilepsy. The vast majority of the 38 million people living with HIV/AIDS are in SSA, and the burden of neurological disorders in SSA parallels that of HIV/AIDS. Local healthcare systems are weak. Many standalone HIV health centres have become a platform with combined tre

COVID-19 Vaccines for HIV-Infected Patients.

Nearly 40 years have passed since the initial cases of infection with the human mmunodeficiency virus (HIV) were identified as a new disease entity and the cause of acquired immunodeficiency disease (AIDS). This virus, unlike any other, is capable of causing severe suppression of our adaptive immune defense mechanisms by directly infecting and destroying helper T cells leading to increased susceptibility to a wide variety of microbial pathogens, especially those considered to be intracellular or opportunistic. After T cells are infected, HIV reproduces itself via a somewhat unique mechanism in

Lessons From Building a Large, Public HIV-Related Database in Support of the Ending the HIV Epidemic in the US Initiative.

The HIV epidemic remains a national priority in the United States, and the Ending the HIV Epidemic initiative has renewed the call for expanded prevention and treatment strategies capable of reducing new HIV infections by 90% by 2030. Achieving this goal requires robust, integrated data for understanding HIV-related needs, barriers to care, and the effectiveness of interventions. However, despite the existence of numerous publicly available datasets, few integrate multiple domains such as HIV outcomes, social determinants of health, and community-level factors. The lack of unified data and dif

Adaptation of an HIV Prevention Ambassador Program Developed in Africa for Use With Cisgender College Women in the United States: Formative Study.

BACKGROUND: Cisgender college women are vulnerable to HIV transmission but rarely use biomedical prevention tools such as pre-exposure prophylaxis (PrEP). In the Orlando Metropolitan Statistical Area (MSA) of Florida, HIV vulnerability is elevated given the area's high incidence rate (sixth among US cities). Increasing HIV and PrEP knowledge among cisgender college women can curb HIV incidence. One method of providing HIV education to cisgender college women and addressing the HIV and PrEP-related stigma is by training ambassadors through programs such as Maximizing Options to Advance Informed

Building capacity for HIV and implementation science among students in the United States: the stimulating training and access to HIV research experiences (STAR) program.

BACKGROUND: Expanding HIV research capacity among the global majority (individuals identifying as Black/African American, American Indian and Alaska Native, Asian, Native Hawaiian and Other Pacific Islander, Multiracial, and Hispanic/Latino) is important. However, achieving national goals to increase the pool of implementation science and HIV early-stage investigators from underrepresented backgrounds remains elusive, largely due to limited investment in training and mentoring these individuals. To address this issue, we launched the Stimulating Training and Access to HIV Research Experiences

Empowerment through knowledge: Qualitative perceptions of 'undetectable equals Untransmittable' among people living with HIV and tuberculosis in South Africa.

INTRODUCTION: Undetectable equals untransmittable (U=U) is an education campaign promoting science that people living with human immunodeficiency virus (HIV) who maintain an undetectable viral load cannot transmit HIV to others. Researchers theorize that undetectable equals untransmittable messaging will decrease HIV stigma by reducing fears of HIV transmission and providing evidence to dismantle discriminatory policies. However, little is known about how people with HIV in South Africa interpret the results of their viral load tests, undetectable equals untransmittable messaging, or its impac

Electron tomography visualization of HIV-1 virions trapped by fusion inhibitors to host cells in infected tissues.

HIV-1 delivers its genetic material to infect a cell after fusion of the viral and host cell membranes, which takes place after the viral envelope (Env) binds host receptor and co-receptor proteins. Binding of host receptor CD4 to Env results in conformational changes that allow interaction with a host co-receptor (CCR5 or CXCR4). Further conformational rearrangements result in an elongated pre-hairpin intermediate structure in which Env is anchored to the viral membrane by its transmembrane region and to the host cell membrane by its fusion peptide. Although budding virions can be readily ima

Elevated Plasma Ceramides Are Associated With Antiretroviral Therapy Use and Progression of Carotid Artery Atherosclerosis in HIV Infection.

BACKGROUND: Ceramides have been implicated in the pathophysiology of HIV infection and cardiovascular disease. However, no study, to our knowledge, has evaluated circulating ceramide levels in association with subclinical cardiovascular disease risk among HIV-infected individuals. METHODS: Plasma levels of 4 ceramide species (C16:0, C22:0, C24:0, and C24:1) were measured among 398 women (73% HIV+) and 339 men (68% HIV+) without carotid artery plaques at baseline from the Women's Interagency HIV Study and the Multicenter AIDS Cohort Study. We examined associations between baseline plasma ceram
