The ROBODOC system was designed to address potential human errors in performing cementless total hip replacement. The system consists of a preoperative planning computer workstation (called ORTHODOC) and a five-axis robotic arm with a high speed milling device as an end effector. The combined experience of the United States Food and Drug Administration multicenter trial and the German postmarket use of the system are reported. Eleven femurs with press-fit titanium hip components were retrieved at autopsy for clinical, radiographic, and histologic evaluation. Back-scattered electron microscopy (BEM), bone densitometry, and appositional bone index studies also were performed. We retrospectively reviewed a series of 40 cementless total hip arthroplasties in 31 patients with advanced avascular necrosis. The average follow-up was 58 months (range, 24 months to 108 months). The clinical failure rate was 20% (8 hips in 7 patients). Three of 7 threaded cups failed, and 2 of 32 porous coated hemispherical cups failed. Of the 5 stem failures, 1 was due to technical error, and 4 were well ingrown porous stems that succumbed to progressive osteolysis.

We have followed the natural progress of newborn infants in whom ultrasound examination showed abnormalities in hips which appeared to be clinically normal. Over six years we saw 306 such children out of 9952 examined (31 per 1000 live births). The examination was repeated at two to three months and those who still showed an abnormality were followed up further. We implanted 300 uncoated cementless PM prostheses into 271 patients and followed 251 (92.6%) of them for four to seven years. By then 37 had already been revised for aseptic and three for septic loosening. The survival rate with implant failure as the endpoint was 88.8% for the cup and 85.3% for the stem after six years. Twenty patients who had chronic lymphocytic leukemia underwent 25 elective primary total hip arthroplasties at the Mayo Clinic. No patient had a postoperative prosthetic infection. No patient had an intraoperative or postoperative hemorrhagic complication. One patient had a revision for aseptic loosening. Traumatic hip dislocation is a rare emergency situation in childhood. Long-term follow-up (10 1/12 years [10/12-20 years]) of 9 patients treated at our institution between 1971 and 1992 revealed no late sequelae. There was no case of necrosis of the femoral head. One patient had a recurrent dislocation of the hip 17 and 18 years after the first dislocation. For post-reduction treatment we propose bedrest until pain relief and mobilisation with crutches for 4 weeks. Regular clinical and radiographic controls, including bone scan and/or MRI, are mandatory during the first 2 years.
