Exploring mechanisms of excess mortality with early fluid resuscitation: insights from the FEAST trial.

BACKGROUND: Early rapid fluid resuscitation (boluses) in African children with severe febrile illnesses increases the 48-hour mortality by 3.3% compared with controls (no bolus). We explored the effect of boluses on 48-hour all-cause mortality by clinical presentation at enrolment, hemodynamic changes over the first hour, and on different modes of death, according to terminal clinical events. We hypothesize that boluses may cause excess deaths from neurological or respiratory events relating to fluid overload. METHODS: Pre-defined presentation syndromes (PS; severe acidosis or severe shock, r

[Associated factors for perinatal mortality in gastroschisis].

PURPOSE: To analyze the perinatal mortality rate in cases of gastroschisis and possible associated factors. METHODS: A retrospective cohort study was conducted between 1992 and 2012. All cases of gastroschisis born in Hospital de Clínicas de Porto Alegre (HCPA) during that period were included. The diagnosis of gastroschisis was obtained by morphological ultrasound examination or clinical examination at birth in prenatally unknown cases. The variables of birth (birthweight, gestational age and Apgar score, mode of delivery, type of gastroschisis and associated anomalies) and the surgical ones

[Changes in obstetric management over the past decade (author's transl)].

Over a observation period of 10 years (1969-1979) developments in perinatal control techniques, mostly started in the 1st Department of Obstetrics and Gynaecology, University of Vienna, at the beginning of that period, and their effect on the obstetric management are presented. In contrast to a trend of decreasing birth rate for the whole of Austria the annual numbers of deliveries in this hospital remained -after a temporary increase-unchanged. Subpartial cardiotocographic monitoring is presently applied in about 95% of all cases. The frequency of Caesarean sections has risen from below 4% to

Viral hepatitis and pregnancy.

The management of viral hepatitis in the setting of pregnancy requires special consideration. There are five liver-specific viruses (hepatitis A, B, C, D, E), each with unique epidemiology, tendency to chronicity, risk of liver complications and response to antiviral therapies. In the setting of pregnancy, the liver health of the mother, the influence of pregnancy on the clinical course of the viral infection and the effect of the virus or liver disease on the developing infant must be considered. Although all hepatitis viruses can harm the mother and the child, the greatest risk to maternal h

Examining national and district-level trends in neonatal health in Peru through an equity lens: a success story driven by political will and societal advocacy.

BACKGROUND: Peru has impressively reduced its neonatal mortality rate (NMR). We aimed, for the period 2000-2013, to: (a) describe national and district NMR variations over time; (b) assess NMR trends by wealth quintile and place of residence; (c) describe evolution of mortality causes; (d) assess completeness of registered mortality; (e) assess coverage and equity of NMR-related interventions; and (f) explore underlying driving factors. METHODS: We compared national NMR time trends from different sources. To describe NMR trends by wealth quintiles, place of residence and districts, we pooled

[Perinatal morbimortality].

Between 1972 and 1975 (4 years), the Hospital de Ginecoobstetricia "Dr. Ignacio Morones Prieto" of the I.M.S.S. at Monterrey, N.L., reported a birth rate of 106, 771 with 98.35% (105,004) live borns. The morbidity rate was 262.06/1000 live borns and the perinatal mortality was 31.24/1000, divided into antenatal 16.54/1000 and posnatal 14.70/1000. Within morbidity, prematurity and/or low weight occupies the first place, followed by respiratory pathology and afterwards, jaundice, infections, obstetric trauma and finally, congenital malformations. The neonatal mortality reached 14.70/1000 live bo

Thrombotic Microangiopathy Following Onasemnogene Abeparvovec for Spinal Muscular Atrophy: A Case Series.

Spinal muscular atrophy is treated with onasemnogene abeparvovec, which replaces the missing survival motor neuron 1 gene via an adeno-associated virus vector. As of July 1, 2020, we had identified 3 infants who developed thrombotic microangiopathy following onasemnogene abeparvovec. Early recognition and treatment of drug-induced thrombotic microangiopathy may lessen mortality and morbidity.

[Pulmonary interstitial emphysema in a 15-month-old child treated with high frequency oscillatory ventilation].

Pulmonary interstitial emphysema (PIE) is a known complication of mechanical ventilation of preterm infants. PIE is associated with high mortality and treatment is controversial as both surgery and conservative treatment are possibilities. We present a case with a 15-month-old previously healthy child, hospitalized with severe respiratory distress. A computed tomography (CT) of the chest revealed localized PIE. He was successfully treated.

Listerial rhombencephalitis in an immunocompetent young adult.

Listeria monocytogenes is a common cause of central nervous system infections, especially in immunosuppressed patients, infants and elderly people. Listerial rhombencephalitis is a rare and severe infection of the brainstem that is reported to have high mortality and frequent serious sequelae for survivors. We report the case of a 19-year-old healthy male who presented with listerial brainstem infection due to Listeria monocytogenes.

Bronchiolitis.

Bronchiolitis is a commonly encountered disease of infants and young children. Mortality is low, but morbidity is significant. Treatment requires considerable commitment of time from medical and nonmedical personnel. Decisions about treatment modalities remain controversial. Patients may remain susceptible to pulmonary problems for years and may be predisposed to the development of asthma. Much is known about this disease, but much remains to be learned.

Associations between intake of vitamins E and A and serum concentrations in preterm infants: A retrospective cohort study.

BACKGROUND: Infants in neonatal intensive care are at risk of both deficiency and excess of fat-soluble vitamins, particularly E and A, due to limited reserves and the need for prolonged intravenous nutrition. We aimed to determine the effects of increased intravenous vitamin intake and to characterise the relationships between vitamin intake and serum concentrations. METHODS: Retrospective cohort of infants admitted to neonatal intensive care who had serum vitamin E or A concentrations quantified in the hospital laboratory using high-performance liquid chromatography. Associations between vi

The path to equitable respiratory syncytial virus prevention for infants: challenges and opportunities for global implementation.

The approval for a respiratory syncytial virus (RSV) maternal vaccination programme by Gavi, The Vaccine Alliance, marks substantial progress toward equitable access, with important work still to come. Several countries, most of which are high-income, have introduced RSV immunisation either using a vaccine containing the RSV fusion protein in its prefusion conformation, which is given to pregnant people in late pregnancy, or by using a long-acting monoclonal antibody (mAb) administered directly to infants. Post-implementation real-world effectiveness data show a major impact in reducing medica

Approach to Short Duration Fever in Children in Office Practice.

Fever is the second most common symptom after respiratory complaints in pediatric practice, often causing parental anxiety despite usually resulting from self-limiting viral infections. Without localized symptoms, diagnosing hidden bacterial infections is challenging. Tropical regions like India present additional diagnostic challenges due to endemic diseases like malaria, dengue, and typhoid. This review provides an algorithmic approach to evaluate and manage short-duration fever in children, tailored to the Indian epidemiological context. The approach requires careful clinical assessment and

Perinatal Neuroprotection in Preterm Birth.

Preterm birth is one of the main causes of perinatal morbidity and mortality. The rate of grade III and IV cerebral hemorrhages in infants with a birth weight of less than 1500 g in Germany in 2022 was 2.97% and the periventricular leukomalacia rate was 1.07%. In addition to these severe forms of brain damage which are visible on sonography, recent MRI studies carried out at the calculated due date of affected children also showed diffuse white and grey matter injuries, especially of the basal ganglia and the cerebellum, indicating impaired brain development and function. To offer these childr

Newborn and infant vision screening in primary care: A clinical review.

PURPOSE: Newborn and infant vision screening is an essential component of the health promotion visit, where the provider screens for ocular risk factors and abnormalities that may cause future impairment or vision loss. Providers may underestimate the importance of screening or find neonatal vision assessments difficult due to poor patient cooperation or time-consuming exams, but the reversibility of vision impairment in infants makes early detection paramount to proper treatment. This article provides a clinical review of evidence-based, practical guidance to providers who care for infants fr

Omphalocele prevalence and co-occurring malformations: a nationwide register-based study of Danish live births in 1997-2021.

PURPOSE: Omphalocele is a congenital abdominal wall defect associated with a high risk of morbidity and mortality, with co-occurring congenital malformations often being the most important prognostic factor. High rates of spontaneous and medical terminations have been reported among pregnancies with omphalocele and co-occurring malformations. Few studies have focused on co-occurring malformations, particularly non-gastrointestinal malformations among live births. This study examined birth prevalence of omphalocele and co-occurring major malformations among a 25-year Danish liveborn cohort. ME
