Thromboembolic prophylaxis in orthopaedic trauma patients: a comparison between a fixed dose and an individually adjusted dose of a low molecular weight heparin (nadroparin calcium).

In a prospective, randomized multicentre trial the safety and efficacy of two regimens of a low molecular weight heparin (nadroparin calcium) were compared for the prevention of deep vein thrombosis (DVT) and pulmonary embolism (PE) in orthopaedic trauma patients. Two hundred and eighty-three patients with a spinal fracture, a pelvic fracture, or a lower limb injury were randomized to receive either a fixed dose (7500 anti-Xa IC units = 3075 anti-Xa IU) or a variable dose depending on body weight and time since operation (100 to 150 anti-Xa IC units/kg approximately equal to 40 to 60 anti-Xa I

Incomplete, intrasubstance strain injuries of the rectus femoris muscle.

Rectus femoris muscle strain injuries commonly occur at the distal muscle-tendon junction of the quadriceps tendon. However, we have recently recognized a pattern of strain injury that consists of an incomplete intrasubstance tear at the muscle-tendon junction formed by the deep tendon of the muscle's indirect head and those muscle fibers originating from this tendon. These injuries are found more proximally within the thigh than the "classic" distal rectus femoris muscle strain. We reviewed 10 athletes with these intrasubstance tears, all of whom had diagnostic imaging performed using compute

Crush injury to the lower limbs. Treatment of the local injury.

We treated fifteen patients who had been trapped under the masonry of collapsed buildings for various periods of time. In one group of patients who had been buried for twelve hours, treatment commenced only twenty-four hours after the injury, and the other group was treated more promptly. In the second group, the success of forced diuresis and alkalinization of the urine in preventing the renal complications of the crush syndrome was evident. A reassessment of the treatment of the local lesion was made and a scheme of treatment is proposed. The injury is essentially closed, and incomplete exci

[One-stage osteosynthesis of 2 segments in multiple fractures of bones of the limbs].

From the experience with one-stage osteosynthesis of two segments in 71 patients with multiple fractures of the extremities and their sequelae the author considers that up-to-date achievements of anesthesiology favour such interventions. He believes that the patients tolerate well one-stage operation which has some advantages as compared to multi-stage operations: only one exposure to narcosis, psychic trauma and unpleasant sensations of the postoperative period; this type of intervention affords motility of the patients and makes it possible to start the functional treatment early and to prev

[Rheumatoid arthritis and trauma. 5 cases].

The authors report 5 cases of post-traumatic rheumatoid polyarthritis. After the traumatism had initiated the first stages of ordinary inflammation, chain reactions, perhaps by means of an "immune" field, intervened to transform it into a self-maintained rheumatoid inflammation. In fact, the etiopathogenic problems of post-traumatic rheumatoid polyarthritis have not yet been elucidated and it cannot be said either that the question of legal compensation for it is any nearer solution. The growing importance of traumatisms in everyday life and of compensation for them, and the gravity of the con

Comparison of Treatment Methods for Syndesmotic Injuries With Posterior Tibiofibular Ligament Ruptures: A Cadaveric Biomechanical Study.

BACKGROUND: Studies on ankle syndesmosis have focused on anterior inferior tibiofibular ligament (AITFL) and interosseous membrane injuries; however, the characteristics of posterior inferior tibiofibular ligament (PITFL) ruptures remain unclear. PURPOSE/HYPOTHESIS: This study evaluated the biomechanical characteristics of syndesmotic instability caused by PITFL injury and compared various treatment methods. We hypothesized that PITFL injury would lead to syndesmotic internal rotational instability and that the stability would be restored with suture tape (ST) PITFL augmentation. STUDY DESIG

Outcomes Following Below Knee Arterial Trauma.

INTRODUCTION: Lower extremity vascular injuries have significant implications for trauma patients with regards to morbidity from limb loss. There is limited evidence on outcomes for patients with injuries to tibial arteries. Our study focuses on defining outcomes of traumatic vascular injury to vessels below the knee. METHODS: A retrospective review using ICD-9 and 10 codes of all patients with below knee vascular injuries was performed at a Level 1 trauma center from November 2014 to June 2022. Interventions, outcomes, and complications were assessed. RESULTS: Seventy-six patients were iden

Free Flaps in Lower Extremity Reconstruction.

Free tissue transfer to the lower extremity for limb salvage remains challenging. A comprehensive approach includes patient selection, flap selection, selection of the recipient vessels, flap dissection, flap preparation, microvascular anastomosis, flap inset, immediate postoperative care, intermediate postoperative care, and further follow-up care. Each step in this comprehensive approach has its unique considerations and should be executed equally to ensure an optimal outcome. Once acquired, some clinical experience along with adequate microsurgical skill, good surgical judgment, well instru

Risk factors for microvascular free flaps in pediatric lower extremity trauma.

PURPOSE: There is a dearth of literature dedicated to specifically evaluating the use of free flap reconstruction in pediatric lower extremity traumas. This study aims to identify specific risk factors for flap failure in pediatric lower extremity trauma reconstruction. METHODS: Retrospective review of 53 free flaps in our lower extremity database (1979-2017) identified all free flaps performed for traumatic reconstruction in children <18 years of age at our institution. RESULTS: Fifty-three free flaps (11.1%) were performed in 49 pediatric patients. The majority of patients were male (69.8%

Knee flexion not hip extension strength is persistently reduced following hamstring strain injury in Australian Football athletes: Implications for Periodic Health Examinations.

OBJECTIVES: Determine whether clinically feasible tests of isometric lower limb strength and range of motion (ROM) in Australian Football (AF) athletes are useful in Periodic Health Examinations to identify persistent deficits following hamstring strain injury (HSI). DESIGN: Case-control. METHODS: Thirty-seven male, semi-professional AF athletes (mean±SD; age, 20.2±2.9years; height, 1.7±0.5m; mass, 81±9.2kg) participated, ten athletes (37%) reported HSI within the previous three seasons of competition. Cost-Utility Analysis of Reconstruction Compared With Primary Amputation for Patients With Severe Lower Limb Trauma in Colombia.

BACKGROUND: To establish, from the health system perspective, the cost-utility relationship of limb reconstruction compared with primary amputation for patients older than 32 years with grade IIIB and IIIC severe lower limb trauma in Colombia, S.A. METHODS: A Markov model was built including different short-term and long-term states that represent the main events that a patient could experience after a lower limb amputation or a reconstruction. A 42-year time horizon was considered for the base case. Transition probabilities were obtained from a systematic review of the clinical literature. T

Induced Membrane Technique: Pearls and Pitfalls.

The induced membrane technique for reconstruction of bone defects has proved its effectiveness and is now widely accepted. The 2 stages procedure, implying a cement spacer in the first stage and a huge bone graft in the second stage, responds to rigorous technical details involving each phase of the procedure. Prerequisites for the technique, cement spacer, bone fixation, time between the 2 stages, grafting, and postoperative course are reported and discussed.

Biceps Femoris Architecture and Strength in Athletes with a Previous Anterior Cruciate Ligament Reconstruction.

PURPOSE: This study aimed to determine whether limbs with a history of anterior cruciate ligament (ACL) injury reconstructed from the semitendinosus display different biceps femoris long head (BFlh) architecture and eccentric strength, assessed during the Nordic hamstring exercise, compared with the contralateral uninjured limb. METHODS: The architectural characteristics of the BFlh were assessed at rest and at 25% of a maximal voluntary isometric contraction (MVIC) in the control group (n = 52) and in the group who had previous ACL injury (n = 15) using two-dimensional ultrasonography. Ultrasonography and radiography: a comparison.

Distal limb fractures are common presentations to emergency departments and minor injury units (MIUs). The authors conducted a study of the usefulness and efficiency of portable ultrasound in detecting the presence of minor fractures in patients presenting to Cirencester Hospital's MIU. Patients above two years of age about whom there was a high clinical suspicion of a closed fracture of the distal forearm or wrist, or the lower limb, were included in the study. After initial clinical assessments, the patients were referred for X-ray, as is usual for such patients, and also for ultrasound imag

Magnetic resonance neurography evaluation of chronic extraspinal sciatica after remote proximal hamstring injury: a preliminary retrospective analysis.

OBJECT: Extraspinal sciatica can present unique challenges in clinical diagnosis and management. In this study, the authors evaluated qualitative and quantitative patterns of sciatica-related pathology at the ischial tuberosity on MR neurography (MRN) studies performed for chronic extraspinal sciatica. METHODS: Lumbosacral MRN studies obtained in 14 patients at the University of California, San Francisco between 2007 and 2011 were retrospectively reviewed. The patients had been referred by neurosurgeons or neurologists for chronic unilateral sciatica (≥ 3 months), and the MRN reports describe

External fixator kickstands for free soft tissue flap protection: case series and description of technique.

BACKGROUND: Protected elevation represents a critical component of postoperative care, particularly in posteriorly located flaps, to prevent pressure on the flap's vascular pedicle and ensure a successful skin graft. Although several short case series and technique papers have described kickstand placement to prevent heel ulcers as an adjuvant to fixator placement for fracture management, there remains a paucity of reports describing external fixator placement solely for extremity elevation and pressure alleviation in the postoperative care of flap coverage procedures. METHODS: Patients who u

The distally based adipofascial sural artery flap: faster, safer, and easier? A long-term comparison of the fasciocutaneous and adipofascial method in a multimorbid patient population.

BACKGROUND: Complex wounds of the distal third of the leg are challenging to treat. Despite free tissue transfer, local pedicled flaps such as the distally pedicled sural artery flap remain a valuable option. METHODS: The authors conducted a retrospective comparison of the classical harvesting technique of the sural artery flap with a simplified method without a skin island. RESULTS: We report on 148 patients who were treated with 104 adipofascial flaps and 44 fasciocutaneus flaps from 1997 to 2010. The adipofascial group showed a shorter operative time and a better aesthetic outcome. Compli
