Loss of hepatic function is the limiting factor in survival of patients with cirrhosis. Fibrogenesis, cytolysis/necrosis and sinusoidal capillarization, altered composition of biomembranes, and hyporegeneration are identified as major factors in loss of hepatic function. Fibrogenesis has been successfully treated with colchicine. Cytolysis/necrosis in autoimmune hepatitis has for years been treated successfully with steroids: in postviral chronic hepatitis the interferons are showing high promise. Treatment of the other factors is the subject of intensive basic and clinical research.

The records of 37 patients with liver injuries operated upon at the Ahmadu Bello University Teaching Hospital, Zaria, Nigeria from 1976 to 1987 were reviewed. There were 31 males and 6 females with a mean age of 22 years. Sixty-two per cent were due to blunt injury from road traffic accidents, 14 per cent were due to gunshot wounds and 5 per cent due to stab wounds as a result of assaults. Left ventricular function is studied in cirrhotic patients and in patients with alcoholic hepatitis by means of isotopic ventriculography (Tc99m) both in basal conditions and after the i.v. injection of a somatostatin bolus (250 mcg). Hydatid cyst of the liver is not uncommon in France. Its various symptoms often include pain, but chance discovery is not exceptional. The diagnosis is helped by two paraclinical examinations, ultrasonography and computerized tomography, which have become indispensable as they provide invaluable information on the precise location and anatomical rapports of the cyst and on possible complications. A retrospective study was performed that analyzed 23 patients who had an orthotopic liver transplantation for the Budd-Chiari syndrome with end-stage liver disease. Patient follow-up was as long as 14 years. The technical considerations relevant to the Budd-Chiari syndrome were discussed. There have been no serious complications of postoperative anticoagulation. Nineteen cases of proven hepatic alveolar echinococcosis were examined by magnetic resonance (MR) and the results were compared with CT. Two patients with situs inversus and biliary atresia were treated with hepatic transplantation, one with an auxiliary liver and the other with an orthotopic graft which was placed using a piggy-back technique. Both transplants functioned well initially. The auxiliary liver was rejected after 1 1/2 months, and the patient died after an attempt at retransplantation many months later. The recipient of the orthotopic liver has perfect liver function 10 months postoperatively.

A case is presented of actinomycosis that involved the lung and the liver, which was diagnosed after a computed-tomographic-guided needle biopsy of a liver mass. The diagnosis was made on histological grounds that were based on the staining and morphological characteristics of Actinomyces spp. Imaging techniques suggested a communication between the lung and liver lesions. The regression of the disease was followed during 12 months of therapy. The patient remained free of disease for three months after the cessation of treatment. The relevant literature is reviewed.
