First-line treatment of osteoporosis with osteoanabolic therapy: a new opportunity.

Osteoporosis is a national health priority, and over six million Australians over the age of 50 years have poor bone health. Fragility fractures due to osteoporosis are associated with an increased morbidity and mortality risk and a high economic cost to the community. It is a chronic condition requiring long-term management. Despite notable advances in pharmacotherapy, large treatment gaps remain. Antiresorptive drugs have been the foundation of treatment; however, their efficacy wanes and rare adverse effects accumulate with prolonged use. Osteoanabolic drugs form new bone and can also resto

Snakebite envenomation-associated acute kidney injury: a South-Asian perspective.

Snakebite is a neglected public health problem in tropical countries. Snakebite envenomation-associated acute kidney injury (SBE-AKI) is a major complication accounting for significant morbidity and mortality. The pathogenesis of SBE-AKI may be multifactorial, including prerenal AKI secondary to hemodynamic alterations, intrinsic renal injury, immune-related mechanisms, venom-induced consumptive coagulopathy and capillary leak syndrome. Epidemiological factors include snake species, duration and severity of snakebite, traditional healers and native medication and accessibility to modern health

Aromatase inhibitors and medication-related osteonecrosis of the jaw: Friends, foes, or bystanders?

Since the 1980s, aromatase inhibitors (AI) have been a cornerstone in the management of hormone receptor-positive breast cancer. However, because of their frequent co-prescription with antiresorptive agents, concerns have been raised regarding the potential contribution of AI to medication-related osteonecrosis of the jaw (MRONJ). We conducted a scoping review to investigate the possible association between AI use and MRONJ. Electronic searches were performed across five databases and supplemented with manual screening and gray literature. Observational studies and clinical trials were include

Acute Concurrent Cardiocerebral Infarction Associated With Trousseau Syndrome: A Case Report.

This case report describes a 66-year-old woman presenting with acute aphasia and right hemiplegia caused by multiple cerebral infarctions. Diagnostic evaluation confirmed an asymptomatic myocardial infarction (MI), as evidenced by elevated troponin I levels (3.1 ng/mL); ST-segment elevation in leads II, III, and aVF on electrocardiography; and coronary angiography showing a thrombotic occlusion in the distal portion of the left anterior descending artery. Elevated tumor markers and imaging identified stage IB endometrioid uterine carcinoma. She had a malignant tumor and multiple thrombotic eve

Severe Rhabdomyolysis in an Elderly Patient With Diabetes and Vascular Disease: Interplay of Statin Therapy, Sodium-Glucose Cotransporter-2 (SGLT2) Inhibition, and Thiazide-Induced Hypokalemia.

Rhabdomyolysis is a potentially life-threatening complication of drug therapy, particularly in elderly patients exposed to polypharmacy. Statins are a well-known cause, while thiazide-induced hypokalemia and volume depletion can further predispose to muscle injury. Sodium-glucose cotransporter-2 (SGLT2) inhibitors such as empagliflozin provide cardio-renal benefits but may potentiate statin toxicity and exacerbate dehydration and electrolyte imbalance, amplifying rhabdomyolysis risk. We report a 78-year-old woman with diabetes, hypertension, hyperlipidemia, and carotid artery disease, hypothyr

Optimal care for mother and baby in severe postpartum mental illness.

BACKGROUND: Severe postpartum mental illnesses (PPSMI), including postpartum psychosis, bipolar disorder, and major depression, affect approximately 1-2 per 1000 deliveries and can lead to serious consequences including suicide, infanticide, and long-term disruptions in the mother-infant relationship. In many low-resource settings, the absence of specialized mother-baby units (MBUs) necessitates alternate models of care. METHODS: This narrative review synthesizes current evidence and clinical guidelines on managing PPMI in outpatient and inpatient settings. Key focus areas include rational ps

Permanent Congenital Hypothyroidism due to Rare Thyroglobulin Gene Variant (p.Cys1476Arg): A Delayed Diagnosis of Thyroid Dyshormonogenesis.

Thyroid dyshormonogenesis is an inherited hypothyroidism caused by a monogenic defect, in the vast majority of cases, in thyroid hormone biosynthesis. It is commonly associated with thyroid enlargement which is vulnerable to nodule formation. We present a Qatari patient with an overlooked diagnosis of thyroid dyshormonogenesis due to thyroglobulin gene mutation. A 10.5-year-old boy has been following up for congenital hypothyroidism since the age of 4 years. He was diagnosed by newborn screening that was confirmed by laboratory thyroid function testing; however, no further workup was done to u

Should echocardiogram be undertaken routinely when a child has severe iron deficiency anaemia?

Iron deficiency anaemia (IDA) is common in children. Treatment usually consists of oral iron therapy and, if severe, inpatient hospitalisation with blood transfusion. Providers may also undertake an echocardiogram, depending on availability and the severity of anaemia. A male toddler with nutritional IDA, haemoglobin of 1.7 g/dL (the lowest level in the literature) and hypertension had left ventricular hypertrophy (LVH) on the initial echocardiogram. He was managed acutely with judicious blood transfusion, followed by oral iron supplementation and anti-hypertensive medication at discharge. Rep

Conservative treatment of corpus callosum hemorrhage due to a falling coconut in Indonesia: a case report.

The potential for traumatic brain injury resulting from falling coconuts is frequently overlooked. These incidents can cause focal lesions in the form of brain hemorrhage. Corpus callosum hemorrhage due to blunt trauma from a falling object is rare and typically associated with poor prognosis. The purpose of this report is to detail a case of corpus callosum hemorrhage caused by a coconut fall and to discuss the conservative management approach employed. We report the case of a 54-year-old woman who was admitted to the hospital with symptoms of unconsciousness, headache, and expressive aphasia

Considerations when prescribing opioid agonist therapies for people living with HIV.

INTRODUCTION: Medications for opioid use disorder (MOUD) include opioid agonist therapies (OAT) (buprenorphine and methadone), and opioid antagonists (extended-release naltrexone). All forms of MOUD improve opioid use disorder (OUD) and HIV outcomes. However, the integration of services for HIV and OUD remains inadequate. Persistent barriers to accessing MOUD underscore the immediate necessity of addressing pharmacoequity in the treatment of OUD in persons with HIV (PWH). AREAS COVERED: In this review article, we specifically focus on OAT among PWH, as it is the most commonly utilized form of

Acute kidney injury in neurocritical care.

Approximately 20% of patients with acute brain injury (ABI) also experience acute kidney injury (AKI), which worsens their outcomes. The metabolic and inflammatory changes associated with AKI likely contribute to prolonged brain injury and edema. As a result, recognizing its presence is important for effectively managing ABI and its sequelae. This review discusses the occurrence and effects of AKI in critically ill adults with neurological conditions, outlines potential mechanisms connecting AKI and ABI progression, and highlights AKI management principles. Tailored approaches include optimizi

Emerging factors affecting peri-implant bone metabolism.

Implant dentistry has evolved to the point that standard implant osseointegration is predictable. This is attributed in part to the advancements in material sciences that have led toward improvements in implant surface technology and characteristics. Nonetheless, there remain several cases where implant therapy fails (specifically at early time points), most commonly attributed to factors affecting bone metabolism. Among these patients, smokers are known to have impaired bone metabolism and thus be subject to higher risks of early implant failure and/or late complications related to the stabil

Resistant hypertension: challenges in everyday practice.

Resistant hypertension is defined as not achieving sufficient control of blood pressure (BP), that is, maintaining BP values equal to or above 140/90 mm Hg when using 3 antihypertensive drugs, including diuretics, properly combined and at maximum doses. The uncontrolled treated hypertension should be confirmed in out‑of‑office BP measurements, preferably with 24‑hour ambulatory BP monitoring. Demographic and clinical characteristics indicate that patients with resistant hypertension are older than the general population of patients with arterial hypertension and more often suffer from comorbid

The Stiff Joint: Comparative Evaluation of Monotherapy and Combination Therapy With Urate Lowering Agents in Managing Acute Gout.

Gout, an extremely painful form of arthritis, is triggered by the innate immune system's response to the accumulation of monosodium urate crystals in specific joints and surrounding tissues. This condition is characterized by recurring episodes of excruciating arthritis flares, interspersed with periods of disease quiescence. Over time, gout can result in disability, tophi formation, and severe pain. The treatment of gout is centered around two main objectives: alleviating inflammation and pain during acute gout attacks and long-term management to reduce serum urate levels and mitigate the ris

Cushing disease in pediatrics: an update.

Cushing disease (CD) is the main cause of endogenous Cushing syndrome (CS) and is produced by an adrenocorticotropic hormone (ACTH)-producing pituitary adenoma. Its relevance in pediatrics is due to the retardation of both growth and developmental processes because of hypercortisolism. In childhood, the main features of CS are facial changes, rapid or exaggerated weight gain, hirsutism, virilization, and acne. Endogenous hypercortisolism should be established after exogenous CS has been ruled out based on 24-hour urinary free cortisol, midnight serum or salivary cortisol, and dexamethasone sup

Perspectives on Mycophenolate Mofetil in the Management of Autoimmunity.

Before becoming a cornerstone in the treatment of numerous immune-mediated diseases, mycophenolate mofetil (MMF) was first introduced as an immunosuppressive agent in transplant immunology and later received the attention of rheumatologists and clinicians involved in the management of autoimmune diseases. MMF is now a widespread immunosuppressive drug for the treatment of several conditions, including lupus nephritis, interstitial lung disease associated with systemic sclerosis, and anti-neutrophil cytoplasm antibody (ANCA)-associated vasculitis while being efficacious also as rescue therapy i

Loperamide Cardiac Toxicity: Pathophysiology, Presentation, and Management.

Loperamide is a nonprescription medication commonly used to treat diarrhea. Although it is an opioid, it is very poorly absorbed and well tolerated, with no systemic toxicity at standard doses. In recent years, however, loperamide has been ingested in very large quantities, sometimes with concomitant medications intended to enhance absorption and/or passage across the blood-brain barrier. Most people who misuse loperamide do so for its euphoric effects or to treat symptoms of opioid withdrawal. In addition to the risks of central opioid toxicity, this practice can result in potentially fatal c

Defibrotide: real-world management of veno-occlusive disease/sinusoidal obstructive syndrome after stem cell transplant.

Hepatic veno-occlusive disease or sinusoidal obstructive syndrome (VOD/SOS) is a life-threatening complication of hematopoietic stem cell transplantation (HSCT). Defibrotide is the only medication approved by the US Food and Drug Administration for the management of severe VOD/SOS after HSCT. We report our center's experience with commercially available defibrotide as treatment of patients with VOD/SOS. We retrospectively identified 28 cases of VOD/SOS, based on the European Society for Blood and Marrow Transplantation criteria, from March 2016 through June 2019. The median day of VOD/SOS onse
