Tissue perfusion as the ultimate target of hemodynamic interventions in the perioperative period.

This point-of-view article examines the complex relationship between global hemodynamic parameters and tissue perfusion, emphasizing the limitations of using macrohemodynamic metrics as proxies for tissue-level oxygen delivery. Key topics of the paper include the physiological determinants of tissue perfusion, the influence of anesthesia on perfusion dynamics, and the role of hemodynamic interventions in optimizing perfusion. Furthermore, we explore the application of tissue perfusion monitoring in the perioperative setting, highlighting its potential to guide individualized therapies. By addr

Human motion measurement methods under the background of molecular chain conformation changes.

This study proposes a human motion measurement model combining molecular chain conformation with a silicone rubber strain sensor embedded with carbon nanotubes to enhance signal response stability. An improved least mean square algorithm is used to optimize signal processing. Experimental results show the model achieves 95.12% measurement accuracy, 92.45% F1 score, 35.14 dB SNR, and 60.45 ms latency. Across different age groups and motion states such as gait, running, and jumping, the average detection error remains below 3%, and physiological monitoring errors for heart rate and oxygen satura

Chronic Kidney Disease-Part 2: Management and Its Complications.

Due to the multifaceted aspects of kidney function, the management of CKD not only requires knowledge of kidney physiology and pathophysiology but also a keen understanding of how to diagnose, treat, and monitor the metabolic derangements and complications of reduced kidney function. In this article, we provide questions that cover a broad range of clinical scenarios that may test the clinical acumen of a kidney health professional. Each question starts with a brief clinical scenario to guide the reader. Questions are followed by a selection of answers. As in most clinical situations, a variet

Pleural Diseases in Connective Tissue Diseases.

Connective tissue diseases (CTD) are heterogeneous, immune-mediated inflammatory disorders often presenting with multiorgan involvement. With the advent of high-resolution computed tomography, CTD-related pleuritis-pleural thickening and effusion-is now increasingly recognized early in the disease trajectory. The natural history of CTD-related pleural effusions varies from spontaneous resolution to progressive fibrothorax with ventilatory impairment. Treatment of the underlying CTD is necessary to manage the pleural disease. Depending on the degree of symptom burden and physiological insult, s

Ferret Behavior Medicine.

Domestic ferrets (Mustela putorius furo) are common zoologic companion animals and display specific body language and vocalizations. Social interactions, play behavior, and resting periods are important keystone in domestic ferret behavior. Specific housing and environmental enrichment are recommended to preserve the expression of normal behavior and physiology in ferrets. Presence of abnormal behaviors, including aggression, urination and defecation outside the litter box, stereotypies, and absence of play behavior, should be carefully monitored by veterinarians and ferret owners to assess fe

Antimicrobial Pharmacology for the Neonatal Foal.

Neonatal foals are at high risk of developing sepsis, which can be life-threatening. Early antimicrobial use is a critical component of the treatment of sepsis. Because the neonatal foal has unique pharmacologic physiology, antimicrobial choice and dosing are often different than in adult horses. Broad-spectrum, bactericidal, and intravenous antimicrobials should be considered first-line therapy for septic foals. A combination of aminoglycoside and beta-lactam antimicrobial or third-generation cephalosporin is an excellent empirical first choice for treating septic foals, until culture and sus

Vascular Magnetic Resonance Imaging in Brain Tumors During Antiangiogenic Therapy--Are We There Yet?

Abnormal tumor vasculature is a potent mediator of treatment resistance because it results in heterogeneous perfusion, hypoxia, increased interstitial fluid pressure, and incomplete penetration of cytotoxic chemotherapies. Targeting this abnormal tumor vasculature is a promising therapeutic strategy, but results with antiangiogenic drugs in brain cancer have been mixed. Vasculature's response to treatment is a dynamic physiological process that can change rapidly throughout treatment, so it requires noninvasive techniques to serially monitor these changes in order to improve outcome. We review

Ischemic prevention for stent implantation via brachial artery in children.

An 11-year-old boy received stent implantation for peripheral pulmonary stenosis through the left modified Blalock-Taussig shunt from the left brachial approach because of the hairpin-shaped route from a femoral or carotid approach. A temporary bypass between the left radial and the femoral arteries was established to prevent ischemic complication of the left forearm. There was no ischemic change of the left forearm in the physiological monitoring despite the long procedure (more than 4 hr) and brachial arterial spasms. No complications occurred after the procedure. The temporary bypass will s

Serial plasma BNP levels in assessing inadequate left ventricular decompression on ECMO.

We describe a female infant with complex single ventricle physiology who had undergone median sternotomy for placement of a right-sided systemic-to-pulmonary artery shunt, division of a patent ductus arteriosus, and left pulmonary artery augmentation. Her early postoperative course was complicated by cardiac arrest requiring institution of extracorporeal membrane oxygenation (ECMO) support. The brain natriuretic peptide (BNP) levels acutely improved after left ventricular decompression by insertion of a left atrial cannula. In this setting BNP levels may be an indicator of left ventricular str

Brain death and organ procurement.

Patients with severe brain injuries (as can result from trauma, subarachnoid hemorrhage, or brain tumor) are monitored closely by nursing staff. It's often the nurse who first recognizes clinical signs of decompensation and begins the process of determining whether the patient is a potential organ donor. When a person is declared brain dead, it's the nurse who maintains hemodynamic stability so that donor organs remain viable. It's therefore crucial for nurses to know how brain death is determined in adults and how potential organ donors are identified, and to know the major physiologic change

Intrapartum fetal assessment. A review.

Some of the difficulty in assessing the clinical value of intrapartum fetal-heart-rate (FHR) monitoring stems from lack of uniformity in pattern interpretation and, therefore, in response to perceived FHR abnormalities. This article reviews the physiology and pathophysiology of FHR changes, stresses the new NICHD guidelines for unambiguously defining FHR patterns, emphasizes the avoidance of circumstances stressful to the fetus, and suggests appropriate treatment for the distressed fetus. The strong likelihood that such treatment will be successful in reversing abnormal FHR patterns, as well a

Pregnancy in neuromuscular and skeletal disorders.

Pregnancy often poses a risk to patients with neuromuscular and skeletal disorders when these affect the respiratory muscles or the rib cage. The outlook is determined both by the severity of the underlying condition and the physiological changes during pregnancy. Patients with a vital capacity of less than 1 to 1.5 litres, hypercapnia, severe scoliosis, diaphragm weakness or pulmonary hypertension before pregnancy are particularly at risk. Pregnancy may adversely affect the conducting airways, respiratory pump and gas exchange in the lungs. Close monitoring of high risk patients during pregna

Pharmacology in space. Part 2. Controlling motion sickness.

In this second article in the two-part series on pharmacology in space, Claire Lathers and colleagues discuss the pharmacology of drugs used to control motion sickness in space and note that the pharmacology of the 'ideal' agent has yet to be worked out. That motion sickness may impair the pharmacological action of a drug by interfering with its absorption and distribution because of alteration of physiology is a problem unique to pharmacology in space. The authors comment on the problem of designing suitable ground-based studies to evaluate the pharmacological effect of drugs to be used in sp

Future trends in neurosurgical instrumentation--a speculative approach.

The neurosurgeon is confronted with a cumbersome array of sophisticated instrumentation with which he must cope. A multitude of tubes, wires, and cables spill about his feet and operative field, cluttering his work area and limiting his mobility. In this communication, the consolidation of power sources, improvement in the power delivery systems to the operative site, and the creation of an equipment console incorporating built-in lights, microscope, power sources, vital-sign monitors, and instrumentation are considered. The physiological condition and milieu of the surgeon are also examined.

Circadian instability following mass trauma predicts PTSD risk.

Trauma exposure can lead to posttraumatic stress disorder (PTSD) in a subset of vulnerable individuals. Circadian rhythm disturbances have emerged as a candidate pathway of vulnerability, yet mean circadian levels often show limited discriminability. We tested whether circadian instability at trauma aftermath prospectively predicts long-term PTSD risk, and what behavioral and physiological indices are most predictive. For that, 211 survivors of the Supernova music festival mass-trauma event and 113 matched comparison participants were monitored in real-world settings for one-month, two-to-six

Modernisation of chronic respiratory disease management: The integration of wearable technology.

Chronic respiratory diseases such as Chronic Obstructive Pulmonary Disease, Asthma, Interstitial Lung Disease, Cystic Fibrosis, and Pulmonary Hypertension affect over 500 million people globally and pose significant challenges for diagnosis, management, and long-term care. Traditional approaches often rely on episodic clinical assessments and patient-reported symptoms, which fail to capture the dynamic nature of respiratory health. The integration of wearable technology offers a transformative solution, enabling continuous, real-time monitoring of physiological and behavioural parameters. This

Continuous-Wave Radar and Motion-Derived Biomarkers for Non-Contact Vital Status Classification in End-of-Life Care: A Clinically Validated Machine Learning Approach.

In palliative care, effective communication about anticipated death is critical for aligning therapeutic goals, managing family expectations, and ensuring dignified care. However, prognostic uncertainty - particularly regarding the time of death - remains a challenge due to the limited reliability of current methods. This study explores the potential of radar-derived motion biomarkers as a novel approach to distinguish between living and deceased patients, addressing the need for objective decision-support tools in palliative care. Using continuous-wave radar, we recorded the torso displacemen

Pulse wave analysis for quality-of-life associated early physiological changes during breast cancer chemotherapy: a proof-of-concept pilot study.

BACKGROUND: Breast cancer (BC) treatment, while essential, often impairs quality of life (QoL), typically assessed through -subjective reports. This single‑center, proof‑of‑concept observational study aimed to develop an objective and noninvasive monitoring method based on spectral analysis of the arterial blood pulse waveform (BPW). We investigated whether specific harmonic indices derived from the BPW could track physiological changes in BC patients following chemotherapy and correlate with patient-reported QoL improvements. METHODS: Radial BPW signals were measured noninvasively in 41 BC p

Shedding light on direct and indirect effects of antimicrobial photodynamic therapy: A review update.

BACKGROUND: Antimicrobial photodynamic therapy (aPDT) operates through the light-activated excitation of photosensitizers, generating reactive oxygen species that simultaneously damage multiple essential microbial structures. This broad-spectrum, multi-target mechanism holds strong promise for combating antimicrobial-resistant microorganisms. Beyond these direct effects, other biological events can occur, potentially influencing microbial physiology, host responses, or overall treatment efficacy. These intermediate processes can yield beneficial outcomes or unintended consequences, highlightin
