Congenital non-progressive peripheral neuropathy with arthrogryposis multiplex.

A family is described in which a hereditary peripheral neuropathy occurs, inherited as an autosomal dominant character. The syndrome is present at birth and does not show any significant progression thereafter. Three of the cases have suffered from arthrogryposis multiplex congenita. A secondary myopathy is also present. The condition appears to be intermediary between peroneal muscular atrophy and Friedreich's ataxia. Appearance at birth, presence of arthrogryposis multiplex congenita, and the non-progressive nature separate it sharply from the other heredofamilial ataxias and peripheral neur

[Combination of multiple displacement amplification with short tandem repeat polymorphismin preimplantation genetic diagnosis].

OBJECTIVE: To explore the application of multiple displacement amplification (MDA) combined with short tandem repeats (STRs) in preimplantation genetic diagnosis (PGD). METHODS: MDA was applied to amplify the whole genome of a single cell and to retrieve and assemble the highly heterogeneous STR loci among human population. Haplotype analytic system was established with aiming at diagnosis of the single gene diseases by selecting the STR loci located within the pathogenic genes or on both bounding sides of the pathogenic genes. At the same time, allele specific amplification, PCR-reverse dot-

Pathomechanisms of ALS8: altered autophagy and defective RNA binding protein (RBP) homeostasis due to the VAPB P56S mutation.

Mutations in RNA binding proteins (RBPs) and in genes regulating autophagy are frequent causes of familial amyotrophic lateral sclerosis (fALS). The P56S mutation in vesicle-associated membrane protein-associated protein B (VAPB) leads to fALS (ALS8) and spinal muscular atrophy (SMA). While VAPB is primarily involved in the unfolded protein response (UPR), vesicular trafficking and in initial steps of the autophagy pathway, the effect of mutant P56S-VAPB on autophagy regulation in connection with RBP homeostasis has not been explored yet. Examining the muscle biopsy of our index ALS8 patient o

The role of vascular pathology in SMA: implications for pathogenesis and treatment.

Spinal muscular atrophy (SMA) has become a paradigm for precision medicine following the development of transformative therapies targeting the disease-causing deficiency of survival motor neuron (SMN) protein. As more than 98% of SMA cases arise from mutations in the SMN1 gene, restoration of SMN expression through nusinersen (Spinraza; Biogen, Cambridge, MA), risdiplam (Evrysdi; Roche, Basel, Switzerland) and onasemnogene abeparvovec (Zolgensma and Itvisma; AveXis, Novartis, Chicago, IL) has produced unprecedented clinical benefits and established SMA as the first treatable motor neuron disea

Selective Neuromuscular Denervation in Taiwanese Severe SMA Mouse Can Be Reversed by Morpholino Antisense Oligonucleotides.

Spinal muscular atrophy (SMA) is an autosomal recessive motor neuron disease caused by deficiency of the survival of motor neuron (SMN) protein, which leads to synaptic defects and spinal motor neuron death. Neuromuscular junction (NMJ) abnormalities have been found to be involved in SMA pathogenesis in the SMNΔ7 SMA mouse model. However, whether similar NMJ pathological findings present in another commonly used mouse model, the Taiwanese SMA mouse, has not been fully investigated. To examine the NMJs of the Taiwanese severe SMA mouse model (Smn-/-; SMN2tg/0), which is characterized by severe

Muscle cramp in Machado-Joseph disease: altered motor axonal excitability properties and mexiletine treatment.

Machado-Joseph disease is one of the most common hereditary spinocerebellar degenerative disorders with a wide range of clinical manifestations. Pathology studies have shown mild to moderate loss of anterior horn cells and, in terms of spinal pathology, Machado-Joseph disease is regarded as a type of lower motoneuron disease. Muscle cramps are often associated with lower motoneuron disorders, but features of cramps in Machado-Joseph disease patients have never been studied. We investigated the incidence and nature of muscle cramps in Machado-Joseph disease patients, the excitability properties

SMN deficiency-induced alternative splicing dysregulation in cardiac defects.

AIMS: Spinal muscular atrophy (SMA) caused by reduced survival motor neuron (SMN) protein, is increasingly recognized as a multisystem disorder with cardiac involvement. We aimed to determine how SMN deficiency disrupts postnatal cardiac development and to explore the potential molecular mechanism linking SMN loss to cardiomyocyte dysfunction. MATERIALS AND METHODS: A severe Taiwanese SMA mice model was used to evaluate postnatal cardiac growth and function. Cardiac phenotypes were characterized by histological analysis, transmission electron microscopy, and echocardiography. Transcriptomic p

Longitudinal Analysis of Superoxide Dismutase 1 Seeding Activity in Amyotrophic Lateral Sclerosis Cerebrospinal Fluid.

Twenty percent of familial amyotrophic lateral sclerosis (fALS) cases are linked to mutations in the Superoxide Dismutase 1 ( SOD1) gene and accumulation of misfolded SOD1 aggregates. SOD1 misfolding from the broader ALS population without SOD1 mutations is less clear. Here, we report SOD1 seeding activity in antemortem cerebrospinal fluid (CSF) from ALS participants with and without SOD1 mutations during ALS progression. Antemortem CSF from controls, SOD1- ALS, and sporadic ALS (sALS) patients was subjected to SOD1 seed amplification real-time quaking induced conversion (RT-QuIC) assays. SOD1

Improving Severe Combined Immunodeficiency and Cystic Fibrosis Newborn Screening by Replacing Current Tests with First-Tier Targeted Gene Sequencing.

Newborn screening benefits children with rare diseases by enabling early detection and intervention, which improves health outcomes. False-positive results are one harm of screening, with infants subject to further testing to resolve a positive screen and parents susceptible to psychosocial distress while awaiting the results. Newborn bloodspot screening (NBS) for severe combined immunodeficiency (SCID) using quantitative polymerase chain reaction (qPCR) and cystic fibrosis (CF) screening using immunoreactive trypsinogen (IRT) have high false-positive rates, with more than five infants screeni

Challenging the more-is-better dogma: A precision-optimized AAV gene therapy for SMA.

Spinal muscular atrophy (SMA) is a severe neurogenetic disorder and the leading inherited cause of infant mortality. Although the available gene therapy has shown substantial efficacy in treating SMA, safety concerns, including hepatotoxicity, underscore the need for optimization. In this study, our findings challenge the prevailing "more-is-better" paradigm, demonstrating that both insufficient and excessive transgene expression are suboptimal. To maximize therapeutic benefit while minimizing risk, we extensively optimized the expression cassette in adeno-associated virus (AAV)-based construc

Adeno-associated virus serotype 9 antibodies in neonates and young children: Seroprevalence and kinetics.

Gene therapies such as onasemnogene abeparvovec for spinal muscular atrophy (SMA) utilize adeno-associated virus 9 (AAV9) for targeted gene delivery, which requires an AAV9 antibody (AAV9-Ab) immunoglobulin G (IgG) ≤1:50 titer threshold. This retrospective cohort study evaluated age-related AAV9-Ab IgG seroprevalence for patients with SMA (part 1) and AAV9-Ab IgG kinetics and time to 1:50 titer threshold in newborns with elevated AAV9-Ab IgG titers (≥1:100) (part 2). A semi-quantitative ELISA assay was used in part 1 (N = 1,323 patients). For patients aged <12 months, 3.9% (n/N = 31/795) had e

Optimization of Long-Term Human iPSC-Derived Spinal Motor Neuron Culture Using a Dendritic Polyglycerol Amine-Based Substrate.

Human induced pluripotent stem cells (hiPSCs) derived from healthy and diseased individuals can give rise to many cell types, facilitating the study of mechanisms of development, human disease modeling, and early drug target validation. In this context, experimental model systems based on hiPSC-derived motor neurons (MNs) have been used to study MN diseases such as spinal muscular atrophy and amyotrophic lateral sclerosis. Modeling MN disease using hiPSC-based approaches requires culture conditions that can recapitulate in a dish the events underlying differentiation, maturation, aging, and de

Intravenous immunoglobulin prevents peripheral liver transduction of intrathecally delivered AAV vectors.

Gene therapy using neurotropic adeno-associated virus vectors represents an emerging solution for genetic disorders affecting the central nervous system. The first approved central nervous system-targeting adeno-associated virus gene therapy, Zolgensma®, for treating spinal muscular atrophy is administered intravenously at high doses that cause liver-associated adverse events in 20%-30% of patients. Intrathecal routes of vector administration, such as the intra-cisterna magna route, provide efficient gene transduction to central nervous system cells while reducing off-target liver transduction

Premature delivery in the domestic sow in response to in utero delivery of AAV9 to fetal piglets.

Numerous pediatric neurogenetic diseases may be optimally treated by in utero gene therapy (IUGT); but advancing such treatments requires animal models that recapitulate developmental physiology relevant to humans. One disease that could benefit from IUGT is the autosomal recessive motor neuron disease spinal muscular atrophy (SMA). Current SMA gene-targeting therapeutics are more efficacious when delivered shortly after birth, however postnatal treatment is rarely curative in severely affected patients. IUGT may provide benefit for SMA patients. In previous studies, we developed a large anima
