PURPOSE: Sino-orbital fungal infections are serious complications of diabetes and immunosuppression. Standard treatments include surgical debridement of the involved tissues with possible orbital exenteration, intravenous antifungal therapy, and improvement of the host's immunocompetence and metabolic state, when possible. This study retrospectively reviews infections over a 7-year period in 60 consecutive adults (median age 25 years) undergoing their first unrelated donor bone marrow transplant (UD-BMT). T-cell depletion was employed in 93%. More than half the patients had one or more severe, potentially life-threatening, infections. OBJECTIVE: To assess skin positivity to extracts of Alternaria in a wide population of Italian patients suffering from respiratory symptoms using a biologically standardized extract.

A natural anti-sulphatide antibody was found to be present in the serum of every normal individual studied. The reactivity of the antibody was assessed by its interaction with galactosylceramide-I3-sulphate. A 14-year-old girl with a rare form of fungal brain abscess involving the dura and brain parenchyma is reported. No portal of entry of the infection was found. Histological findings and fungal culture both indicated that the causative agent was Xylohypha bantiana, an uncommon dematiaceous fungus. A review of the literature suggests that this infection, which affects primarily young male patients, exhibits distinct neurotropism. Despite therapy, the prognosis is generally poor.

Because of donor organ unavailability, staged cardiac transplantation has been performed in eight patients with The Pennsylvania State University pneumatic total artificial heart (two patients) or with the Pierce-Donachy ventricular-assist pump (six patients). The microbiology of 12,174 symptomatic ears presenting over an eight-year period to one hospital was examined in detail. 1,061 cases of fungal infection were found. The incidence and pattern of these fungi and the accompanying bacteria, in cases of mixed infection, do not appear to have been affected by the widespread use of powerful topically-applied antibiotic/steroid preparations over more than two decades. Otomycosis remains an uncommon, often overlooked but important cause of morbidity in a temperate climate.

Based on extensive personal experiences during the past three decades, a review is given on infections and infestations of the central nervous system, which are found in India and in other tropical and subtropical areas.
