Roundtable discussion: Early labor: what's the problem?

In places where hospital birth is the norm, one of the major contemporary challenges to the organization of intrapartum care is posed by women who are not in established labor. In the United Kingdom, these women have been given a special name, "Category X," and they can account for a substantial percentage of admissions (1). These women are not deemed to be in need of hospital care, but the women themselves may feel otherwise as they struggle to understand the sensations they are experiencing. Until relatively recently, little research effort was expended on early and latent phase labor, refle

Management of adult patients receiving intraventricular chemotherapy for the treatment of leptomeningeal metastasis.

Cancer in the central nervous system can arise from a primary brain tumor and metastasize to the brain or to the leptomeninges, leading to leptomeningeal metastasis (LM). LM also is called leptomeningeal carcinomatosis and carcinomatous meningitis. When LM occurs, signs and symptoms include headache, nausea, vomiting, lumbar back pain, and stiff or painful neck; LM also may lead to mental disturbances and seizures. Nursing care of patients with LM requires an understanding of neurologic anatomy and physiology, along with associated treatments and complications. Treatment of LM may involve intr

Reframing professional development for first-line nurses.

Within a context of healthcare restructuring and a shift toward individualized continuing competency in Canada, this inductive, narrative inquiry explored positive learning experiences of first-line acute care nurses. The written stories of eight self-selected participants were collected and unstructured follow-up interviews were conducted. The stories and interview transcripts were examined using categorical-content and holistic-form analysis, and analyzed in light of literature relating to adult education and professional development in nursing. Emergent themes included life-changing learnin

Wheezing and asthma in early childhood: an update.

Although asthma is the most common chronic disease in children, recent studies show that labeling this condition "asthma," especially in association with chronic wheezing, remains a matter of dispute. Nonetheless, data confirm that wheezing and asthma place great burdens on the caregivers of affected children. A principal source of information on diagnosis and treatment is the practical guide published by the National Asthma Education Prevention Program. The implications for nurses in the management of young children whose wheezing is chronic or who have been diagnosed as having asthma include

Perinatal mental health: midwives and health visitors working together.

The Healthy Child Programme (HCP) focuses on the care offered to pregnant women and children in the first five years of life. It is delivered by a range of professionals, with the health visitor as lead. Effective delivery of the HCP depends on services for children and families being fully integrated, and partnership working between different agencies on a local level seems to be the key to success. This article focuses on how effective implementation of the HCP facilitates the recognition and care of women with anxiety, mild to moderate depression and other perinatal mental disorders during

The care of children and adolescents with type 2 diabetes.

The prevalence of Type 2 diabetes has dramatically increased in children and adolescents over the past 10 years. Type 2 diabetes is characterized by insulin resistance and high insulin levels. Reasons cited for the rise of this condition in children and adolescents are speculated to stem from obesity because of a rise in sedentary behavior, nonnutritious food choices, and genetic predisposition. A high recurrence rate in families shows that therapy for children and adolescents must involve the entire family to be successful. Treatment recommendations vary depending on severity but include nutr

Public health initiative at a young offenders institute.

One in four of prisoners aged 17 to 21 are fathers, often with limited experience of positive parenting themselves. The men in Aylesbury Young Offenders Institute were found to have pride in fatherhood and during parenting classes expressed a desire to become actively involved in the care of their children. They stated that they felt they needed to learn parenting skills with their partner and family rather than in isolation from them. Health visitors were involved with parenting classes in the prison education block and helped the fathers develop joint parenting skills through a family orient

Finding the best fit: a grounded theory of contraceptive decision making in women.

PROBLEM: Half of the pregnancies in the United States that occur annually are unintended, and of these, half occur among women using contraceptives. METHODS: Interviews with 16 women, field notes, and memos were analyzed using constant comparative analysis and the methods of open, axial, and selective coding were used until saturation occurred. FINDINGS: The core category that describes the process of contraceptive decision making in women is "finding the best fit." Women choose a method or methods to prevent pregnancy based on their knowledge, experience, and evaluation of what would be the

Heterogeneity and associated factors of palliative care core competency among oncology nurses: a latent profile analysis.

BACKGROUND: Global cancer incidence is expected to rise by 60% by 2040, intensifying the burden experienced by patients and their families. Palliative care improves quality of life in advanced disease and depends on oncology nurses' competencies in ethical decision-making, symptom management, and psychological resilience. Despite moderate overall proficiency, psychosocial shortcomings and heterogeneity in competency remain insufficiently characterized. This study utilized latent profile analysis (LPA), guided by shared decision-making and symptom management theories, to identify competency sub

Sustainable improvement of interprofessional care for better resident outcomes: protocol for the INTERSCALE hybrid type III effectiveness cluster-randomized trial comparing individualized and collaborative delivery of an evidence-based care model for long-term care.

BACKGROUND: Over recent decades, multifaceted nurse-led care models have been developed to reduce unplanned hospital transfers from long-term care facilities (LTCFs). In Switzerland, the INTERCARE model has demonstrated effectiveness, with core components including deployment of nurses in expanded roles (INTERCARE nurses), evidence-based communication tools, and advance care planning. However, resource-intensive implementation strategies such as 1:1 support meetings for model implementers pose challenges for scale-up, underscoring the need for more scalable implementation support. The INTERSCA

Risk Factors for Enteral Nutrition Intolerance in ICU Patients and Corresponding Optimized Nursing Strategies.

BACKGROUND: Enteral nutrition intolerance (ENI) is common among critically ill patients and may interfere with the effective delivery of enteral nutrition. Identifying patients at increased risk and potentially modifiable care-related factors may help inform more targeted nursing management. AIM: This study aimed to identify independent risk factors for ENI in critically ill patients, evaluate nurse-modifiable protective factors and develop risk-stratified nursing optimization strategies. METHODS: A single-centre retrospective cohort study was conducted, including adults patients receiving E

Do Alarm Cues on ECG Monitors Enhance ICU Nurses' Detection of Abnormal Data? Evidence From an Eye-Tracking Experiment.

BACKGROUND: Most modern electrocardiographic (ECG) monitors include visual alarm cues to flag abnormalities. Whether these cues consistently improve nurses' recognition of abnormal data remains uncertain. AIM: The primary aim is to examine the effects of alarm cues on ICU nurses' behavioural performance when identifying abnormal physiological parameters while using bedside vital-sign monitors. The secondary aim was to investigate how alarm cues influence nurses' visual attention patterns. STUDY DESIGN: A within-subjects experiment with ICU nurses using a simulated monitor under two condition

Educational death café programme as an intervention for coping with death, thanatophobia and readiness for advance care planning: A sequential mixed methods study among final year nursing students.

AIM: This study aimed to develop and evaluate an educational death café programme on coping with death, thanatophobia, and readiness for advance care planning (ACP) among final year nursing students. BACKGROUND: Nursing students often feel unprepared to face death, struggle to cope with death and discussions surrounding ACP. Death cafés have emerged as a potential educational strategy to normalise death conversations in a safe, informal setting. Hence, this study aimed to examine the effects of a new educational death café on nursing students' outcomes within an Asian setting. DESIGN: A sequ

Healthcare providers' perceived acceptability of a warning signs intervention for rural hospital-to-home transitional care: A cross-sectional study.

INTRODUCTION: There is a pressing need for transitional care that prepares rural dwelling medical patients to identify and respond to the signs of worsening health conditions. An evidence-based warning signs intervention has the potential to address this need. While the intervention is predominantly delivered by nurses, other healthcare providers may be required to deliver it in rural communities where human health resources are typically limited. Understanding the perspectives of other healthcare providers likely to be involved in delivering the intervention is a necessary first step to avert

Psychological depletion in physicians and nurses exposed to workplace violence: A cross-sectional study using propensity score analysis.

BACKGROUND: Workplace safety is a key issue in health care. However, workplace violence represents a serious threat to workplace safety and has become a global public health problem. Workplace violence may also lead to the psychological depletion of healthcare workers. Previous studies, although cross-sectional, did not use methods such as propensity score matching to assess the potential causality of workplace violence on mental health. OBJECTIVES: To investigate the effects of workplace violence on depression and anxiety symptoms by propensity score matching, and to explore the prevalence o

Safe Patient Handling Training: One-Year Practice Indicators.

OBECTIVE: To evaluate the implementation and one-year practice-related indicators of a structured safe patient handling training program within a quality improvement framework. BACKGROUND: Safe patient handling is a core component of rehabilitation nursing practice, particularly for patients with reduced autonomy, but it exposes healthcare workers to substantial biomechanical demands and risk of work-related musculoskeletal disorders. Although structured training programs are widely used to promote safer handling practices, evidence on their sustained integration into routine clinical practic
