Proximal row carpectomy for scapholunate dissociation.

Thirty-one patients underwent proximal row carpectomy for static scapholunate dissociation without degenerative arthritis. We report the radiographic and clinical results including wrist range of motion, grip strength, use of pain medication, pain severity and patient satisfaction. Mean postoperative flexion/extension was 74° (range 5°-150°) compared with 137° (range 70°-175°) for the uninvolved side. Mean grip strength was 22 kg for the involved side and 39 kg for uninvolved side. Fifteen of 31 patients reported moderate or severe pain after proximal row carpectomy, requiring wrist arthrodesi

Subscapularis function following the latarjet coracoid transfer for recurrent anterior shoulder instability.

The Latarjet procedure may be performed with both subscapularis splitting and subscapularis transecting approaches. The subscapularis splitting approach may better preserve subscapularis function and anatomy. The goal of this study was to determine the functional status of the subscapularis after the Latarjet procedure with a subscapularis splitting approach using the quantified belly press test. Thirty patients with traumatic anterior shoulder instability were prospectively enrolled in the study. All patients underwent a Latarjet procedure through a subscapularis splitting approach. Both oper

Osteochondral autograft transfer for treatment of osteochondritis dissecans of the caudocentral humeral head in dogs.

OBJECTIVE: To establish whether osteochondral autograft transfer (OAT) procedures for osteochondritis dissecans (OCD) of the canine caudocentral humeral head would restore articular contour, resurface osteochondral defects, and resolve lameness in dogs with OCD. STUDY DESIGN: Case series. ANIMALS: Dogs (n=14) with caudocentral humeral head OCD (16 shoulders). METHODS: After arthroscopic assessment, the humeral head was exposed by arthrotomy. The OCD lesion was debrided and OATS(™) instrumentation used for resurfacing the defect with osteochondral core grafts collected from the stifle. Recip

Does conventional practice prevent occular complications in prone position spinal surgery?

Postoperative visual loss following spinal surgery in the prone position may be the most limiting to the quality of a patient's life and the most likely to entail medico-legal consequences for medical and theatre personnel. We analyse the incidence of occular complications after 181 consecutive spinal surgery in the prone position in a typical district general hospital setting in the United Kingdom. No patient undergoing spinal surgery in the prone position lasting over 2 hours developed postoperative transient or permanent visual loss or any other occular complication (incidence 0%). Perioper

[Case of thyroid crisis with persistent tachycardia diagnosed postoperatively].

A 35-year-old man with multiple bone fractures underwent an emergency operation. On arriving at the operating room, his heart rate was 160 beats x min(-1), and blood pressure was 100/50 mmHg. We anesthetized him with oxygen, sevoflurane, fentanyl and remifentanil. We suspected hypovolemia, and treated him with crystalloid and transfused red cells and fresh frozen plasma so that heart rate and blood pressure could be stabilized. Tachycardia of 140 beats x min(-1) persisted, and landiolol was continuously administered at a rate of 5-10 mg x hr(-1) after a 2.5 mg bolus injection. Heart rate becam

Polymyoclonus seizure resulting from accidental injection of tranexamic acid in spinal anesthesia.

We present a case of accidental injection of tranexamic acid instead of bupivacaine during spinal anesthesia. One minute after intrathecal injection of 3.5 mL of solution, the patient developed myoclonus of his lower extremities. Accidental intrathecal injection of the wrong drug was suspected and a used ampule of tranexamic acid discovered in the trash can. The ampules of tranexamic acid (500 mg/5 mL) and bupivacaine (5 mg/mL, Merck, Darmstadt, Germany) were similar in appearance. General anesthesia was induced. Ten hours later, the patient developed myoclonus of his upper extremities and fac

Surgical correction of Haglund's triad using complete detachment and reattachment of the Achilles tendon.

UNLABELLED: Haglund's triad consists of the simultaneous presentation of the following clinical entities: Achilles tendinosis, Haglund's deformity, and retrocalcaneal bursitis. In this retrospective study, we review outcomes following the surgical treatment of Haglund's triad by means of complete detachment and reattachment of the Achilles tendon, removal of the retrocalcaneal exostosis, and excision of the retrocalcaneal bursa through a medial J-shaped approach. Patients were identified via chart review, and sent consent forms and a questionnaire consisting of a visual analog scale (VAS), a s

Localized hypertrophic neuropathy: a case report and review of the literature.

The case is presented of a 71-year-old man with a 6-year history of symptoms suggestive of carpal tunnel syndrome, which did not improve despite two surgical procedures. On further investigation, a fusiform enlargement of the median nerve was found above the elbow, which was found on biopsy to be localized hypertrophic neuropathy (LHN). This case is the first to be described affecting the median nerve. The literature regarding LHN is reviewed, with discussion of the differential diagnoses and possible etiology of this rare lesion.

Posterior dislocation of the hip associated with ipsilateral trochanteric fracture--a very rare case.

Hip dislocations are a frequent consequence of traffic accidents. Commonly, the knee hits the car board and the force of the impact leads to either a fracture of the femur or a dislocation of the hip. The dislocation occurs if the femur does not break and the force of the impact is transmitted towards the hip joint leading to a posterior dislocation of the femoral head. We present the case of a 41 year old patient involved in a traffic accident that suffered a knee injury. Clinical and imagistic (radiography, CT) examination revealed a trochanteric fracture associated with a posterior hip disl

Giant cell tumour of the distal radius: wide resection and reconstruction by non-vascularised proximal fibular autograft.

INTRODUCTION: Giant cell tumours of the bone are aggressive and potentially malignant lesions. Juxtaarticular giant cell tumours of the lower end radius are common and present a special problem of reconstruction after tumour excision. Out of the various reconstructive procedures described, non-vascularised fibular autograft has been widely used with satisfactory functional results. MATERIALS AND METHODS: Ten patients with a mean age of 33.4 years, with either Campanacci grade II or III histologically proven giant cell tumours of lower end radius were treated with wide excision and reconstruct

Open carpal tunnel release--still a safe and effective operation.

BACKGROUND: Carpal tunnel syndrome is a common cause of neurological symptomatology. Surgical decompression remains the treatment of choice in patients not responding to conservative therapies. The aim of this study was to assess the effectiveness of standard open decompression by analysis of symptomatic and functional improvement and to assess whether a general surgeon can still perform this operation safely. PATIENTS AND METHODS: Patients undergoing standard open carpal tunnel release by a single general surgeon were recruited. A self-administered Boston questionnaire was used to assess sym

Extreme eccentric canal type pectus excavatum: morphological study and repair techniques.

OBJECTIVE: Extreme eccentric canal type pectus excavatum (Grand Canyon type, GC, Type 2A3), is a distinct morphological variation, characterized by an eccentric longitudinal canal. Due to the extent of depression and asymmetry, repair is often challenging with the minimally invasive technique. Following, we present its morphologic characteristics and evaluate repair techniques according to morphology type. METHODS: Extreme eccentric canal type pectus excavatum is an eccentric, long canal-like chest wall depression from the infra-clavicle to lower chest. Among 851 patients who underwent pectus

After rotator cuff repair, stiffness--but not the loss in range of motion--increased transiently for immobilized shoulders in a rat model.

Although rotator cuff repair is often successful at relieving pain, the repaired insertion site frequently fails. Mechanical properties of the repair improved when the shoulder was immobilized in an animal model, but joint stiffness and range of motion were not evaluated. The objective of this study was to measure rotational mechanics before and after shoulders were immobilized after cuff injury and repair, not immobilized after cuff injury and repair, and immobilized without injury and repair. Humeral rotation was significantly less 4 and 8 weeks after injury and repair but did not decrease s

Intrasynovial lipoma causing trigger wrist and carpal tunnel syndrome.

Triggering of the flexor tendon at the wrist is rare. We report a case of intrasynovial lipoma that caused a trigger wrist. As far as we know it is unique in that the intrasynovial lipoma simultaneously caused carpal tunnel syndrome. The massive tenosynovitis and adhesion of flexors tendons after the locking of the intrasynovial lipoma may have resulted from inflammation caused by attrition within the carpal tunnel.

Acute and chronic management of posterolateral corner injuries of the knee.

Isolated injury to the PLC of the knee is rare. More commonly seen is injury to the PLC combined with other ligamentous and bony knee injuries, such as ACL and PCL tears and tibial plateau fractures. Prompt recognition and treatment of PLC injuries is important as failure to do so can compromise the success of PLC repair or reconstruction, as well as the success of associated ACL or PCL reconstructions. Clinical management of the PLC is dictated by the severity of injury and timing of diagnosis. A general treatment algorithm can be followed based on grade of injury, but individualized treatmen

Revision of loosened iliac screws: a biomechanical study of longer and bigger screws.

STUDY DESIGN: The present study compared the biomechanics of 2 revision iliac screws: longer and bigger screws, on human cadaveric pelves. OBJECTIVE: To determine if a bigger screw resists loosening under cyclic loading better than a longer screw in revising loosened iliac screws. SUMMARY OF BACKGROUND DATA: Iliac screws have been used in treating spinal deformity, spondylolisthesis and many other spinal diseases. Because of the cancellous bone along the screw trajectory, screw loosening over cyclic loading has been experienced in clinical applications. Two popular revision choices are: a lo
