Low birth-weight and pre-term delivery in South-east Asia. The WHO International Collaborative Study of Hypertensive Disorders of Pregnancy.

Population-based data from 10 centres in Burma, Thailand, China and Vietnam, have been analysed to assess those social and environmental factors which may be associated with pre-term delivery and low birth-weight. The philosophy behind the study was that if the same association was found in different countries, then the association may be causal; if associations differed in the various countries, then causality was unlikely. The major findings were as follows: a strong association between low birth-weight and pre-term delivery and the unmarried state (RR 1.64, 1.39), a consistent reduced risk

A highly polymorphic locus cloned from the breakpoint of a chromosome 11p13 deletion associated with the WAGR syndrome.

Children with constitutional deletions of chromosome 11p13 suffer from aniridia, genitourinary malformations, and mental retardation and are predisposed to develop bilateral Wilms tumor (the WAGR syndrome). The critical region for these defects has been narrowed to a segment of band 11p13 between the catalase and the beta-follicle-stimulating hormone genes. In this report, we have cloned the endpoints from a WAGR patient whose large cytogenetic deletion, del(11)(p14.3::p13), does not include the catalase gene. The deletion was characterized using DNA polymorphisms and found to originate in the

Molecular genetics of Turner's syndrome.

Recombinant DNA technology now allows an analysis of sex chromosomal abnormalities at the molecular level. X chromosomes differ in respect of their pattern of cutting sites for particular restriction enzymes; these differences can be used to determine which X chromosomes or which part of an X chromosome has been inherited from a parent. Furthermore, these techniques can be used to define the presence or absence of particular regions of the X chromosomes with a higher level of resolution than it is possible to achieve using light microscopy. Thirty-eight families in which there was a child or f

A variant of von Willebrand's disease characterized by recessive inheritance and missing triplet structure of von Willebrand factor multimers.

A 10-yr-old girl had bleeding symptoms of moderate severity; her mother and maternal aunt had milder bleeding symptoms, and other members of the kindred were asymptomatic. In the child, factor VIII coagulant activity (VIII:C) and von Willebrand factor antigen (vWF:Ag) were normal, ristocetin cofactor very low, and the bleeding time (BT) markedly prolonged. These values were normal in the rest of the kindred, but the mother and maternal aunt had prolonged BT and a high VIII:C/vWF:Ag ratio. Crossed immunoelectrophoresis (CIE) showed a vWF:Ag peak migrating more anodally in the propositus, two di

Diploid-triploid mixoploidy: clinical and cytogenetic aspects.

A patient with 46, XY/69,XXY mixoploidy (a mixture of cell populations whose component cells differ in their chromosome numbers, irrespective of whether these numbers are euploid or aneuploid) who had some clinical manifestations of the Russell-Silver syndrome is reported. Triploidy results in a clinically recognizable lethal syndrome with hydatidiform placental changes, severe intrauterine growth deficiency, dysplastic cranial bones, eye defects, cleft lip and/or, palate malformed ears, micrognathia, syndactyly, genital anomalies and, rarely, spina bifida. Less severe are instances of diploid

Inherited X chromosome linked thyroxine-binding globulin (TBG) deficiency in a homozygous female.

Of the 37 families with TBG deficiency so far described, 31 were shown to be compatible with X chromosome linked mode of inheritance, and in 5 of the remaining 6 this mode of transmission was suspected. Difficulties in proving X chromosome linkage in some families was usually due to the inability to identify the heterozygous female carriers when affected males were only partially TBG deficient. This work describes a new family with inherited TBG deficiency which on first glance showed inconsistencies with X chromosome linked inheritance. More specifically, there was an apparent male to male tr

Familial Creutzfeldt-Jakob disease.

A Finnish family is described with 9 cases of presenile dementia in 3 generations. The mean age at onset was 52 years (range 46--62 years). Progressive dementia, upper motor neuron signs, muscular rigidity, and twitching, irregular tremors were consistent features in the 6 clinically investigated patients and were associated with spongiform change in the cerebral cortex of one autopsy and two brain biopsy cases. The EEG showed progressive slowing without the occurrence of repetitive high-voltage complexes at any stage of the disease. The average duration of the disease (21 months, range 11--36

On the mutation rate of neurofibromatosis.

A genetic study of 124 cases of neurofibromatosis was performed. The contingent of probands was mainly represented by a Russian population, most of the individuals being born in the European part of the RSFSR. Both parents of the probands were examined in only 58 cases, the proportion of sporadic cases in this group being 0.79, as compared to 0.77 for the whole group under study. The existing data evaluated by a direct method are not yet sufficient for a decisive estimation of the penetrance, which, however, cannot be under 80%. Maternal mental health and risk of child protection involvement: mental health diagnoses associated with increased risk.

BACKGROUND: Previous research shows that maternal mental illness is an important risk factor for child maltreatment. This study aims to quantify the relationship between maternal mental health and risk of child maltreatment according to the different types of mental health diagnoses. METHODS: The study used a retrospective cohort of children born in Western Australia between 1990 and 2005, with deidentified linked data from routine health and child protection collections. RESULTS: Nearly 1 in 10 children (9.2%) of mothers with a prior mental health contact had a maltreatment allegation. Alte

Prenatal and postnatal cocaine exposure predict teen cocaine use.

Preclinical studies have identified alterations in cocaine and alcohol self-administration and behavioral responses to pharmacological challenges in adolescent offspring following prenatal exposure. To date, no published human studies have evaluated the relation between prenatal cocaine exposure and postnatal adolescent cocaine use. Human studies of prenatal cocaine-exposed children have also noted an increase in behaviors previously associated with substance use/abuse in teens and young adults, specifically childhood and teen externalizing behaviors, impulsivity, and attention problems. Household chaos--links with parenting and child behaviour.

BACKGROUND: The study aimed to confirm previous findings showing links between household chaos and parenting in addition to examining whether household chaos was predictive of children's behaviour over and above parenting. In addition, we investigated whether household chaos acts as a moderator between parenting and children's behaviour. METHOD: The sample consisted of 118 working- and middle-class two-parent English families with two children aged 4-8. Parents provided reports of the parent-child relationship, the level of chaos in their home and the children's problematic behaviour. The chi

Parental imprisonment: effects on boys' antisocial behaviour and delinquency through the life-course.

BACKGROUND: Prisoners' children appear to suffer profound psychosocial difficulties during their parents' imprisonment. However, no previous study has examined later-life outcomes for prisoners' children compared to children separated from parents for other reasons. We hypothesise that parental imprisonment predicts boys' antisocial and delinquent behaviour partly because of the trauma of separation, partly because parental imprisonment is a marker for parental criminality, and partly because of childhood risks associated with parental imprisonment. METHOD: This study uses prospective longitu

Prenatal and postnatal histories of very low birthweight infants who developed hepatoblastoma.

BACKGROUND: Hepatoblastoma in children of very low birthweight (< 1500 g) is increasing in Japan and this has suggested the presence of either a genetic or environmental etiology. This study was aimed at revealing common prenatal and postnatal histories, including family history of hepatoblastomas in children of very low birthweight. METHODS AND RESULTS: The medical records of 15 patients, nine boys and six girls, were reviewed. The patients were diagnosed at the age of 6-77 months (median 16 months). Their birthweight ranged from 560 to 1380 g (median 826 g) and the gestational age was 23-33

Factors related to infant apnoea and cyanosis: a population-based study.

OBJECTIVES: To examine the relationship between infant and parental characteristics and parental report of infant cyanosis and also hospital admission for apnoea/cyanosis. METHODS: A prospective cohort study was conducted. It involved the one-fifth of Tasmanian live births who were assessed, using a perinatal score, as being at higher risk of sudden infant death syndrome (SIDS). From 1 May 1988 to 30 April 1993, 6213 infants (89% of eligible infants) participated in the hospital (4 days postnatal age) and home interview (5 weeks postnatal age). Data on usual sleep position and infant history

Panic-phobic patients and developmental trauma.

BACKGROUND: Several studies suggest high rates of developmental trauma among adult anxiety disorder patients. We attempted to replicate these findings in patients with panic disorder, agoraphobia, and/or social phobia in comparison with a nonclinical population and to evaluate possible relationship of traumas and phobic subtypes. METHOD: Fifty-one patients with panic disorder with agoraphobia and/or social phobia were assessed for lifetime diagnoses using interviews and rating scales and for developmental trauma by the Life Experience Questionnaire (LEQ). Fifty-one demographically similar non

[Abortion on demand. Descriptive and quantitative study of psychological and psychosocial aspects].

A pilot study investigated 66 women (mean age: 27 years) before or after abortion with a standardised questionnaire in a center for family planning in Bobigny, a northern suburb of Paris. The results were compared to a former study about spontaneous abortion (n = 156) in Regensburg (Germany). More than half of the women were from the oversea departments, Africa and Northern Africa. 30% were foreigners and 75% unmarried. 48% had repetitive abortions. However gynaecological and obstetrical complications were low. The undesired pregnancies were in link with discontinued contraceptive protection o

Life events and depressive disorder reviewed. I. Events as predisposing factors.

I reviewed studies examining the hypothesis that life events that occur during childhood or early adolescence may predispose a person to a depression in adulthood. Of studies that compared the incidence of childhood bereavement or other childhood loss events among depressed patients and controls, the majority found an increased incidence among the depressives. Although discrepant negative findings exist, it seems that the childhood loss of a parent by death generally increases depressive risk by a factor of about 2 or 3. In addition, early loss events also seem to be related to the severity of

Genetic and family studies in Friedreich's ataxia.

This study consists of two parts: 1. A detailed genetic analysis of 35 sibships in which 58 individuals were affected with Friedreich's ataxia; and 2. Clinical and laboratory examinations of parents and siblings, in an attempt at carrier detection and diagnosis of the pre-clinical state. The increased parental consanguinity, the lack of affected individuals in other generations, and the lack of significance of extrinsic etiological variables, all suggested an autosomal recessive mode of inheritance, and this was confirmed by formal genetic analyses, employing several different methods.
