Two patients with severe liver disease complicated with ascites were recently treated at our institution. Both rapidly developed renal failure. In one patient, liver disease was the result of alcohol abuse, and in the other, was due to malnutrition associated with obesity and acute weight loss. The only reasonable therapeutic approach for these patients was believed to be a course of peritoneal dialysis, along with other supportive measures. In both cases, the management was successful. The effect of sodium nitroprusside (SNP), a vasodilator drug, and chlorpromazine (CPZ), a surface active drug, on the efficacy of peritoneal dialysis was studied in 25 patients with acute or acute on chronic renal failure in a double blind fashion. Fungal infection has become increasingly more important in patients undergoing continuous ambulatory peritoneal dialysis. We report here a case of Histoplasma capsulatum infection in such a Hong Kong Chinese patient who presented with fever and peritonitis. Histoplasma capsulatum was isolated from the dialysis fluid and histoplasma antibody was detected in the serum. The patient responded to the combined treatment of fluconazole, 5-flurocytosine and amphotericin B. This is the first reported case of histoplasmosis in Hong Kong.

It is well recognized that the defect of extrahepatic bile duct close to the hepatic hilum is very difficult to repair by often used method. Since 1980, 3 cases with accidently injured bile duct of such an extent by previous surgery were successfully treated by using isolated peritoneal graft tube. The patients were followed up from one to two years, and they were all doing very well and back to their work. The indications and advantages of the procedure were discussed and the authors believe that it is a useful method to repair of complicated bile duct defect.

We developed a method for avoiding contamination by fibroblasts when cultures of peritoneal cells are initiated.
