Benefits of nitric oxide administration during cardiopulmonary bypass on postoperative outcomes in adult patients: a meta-analysis and systematic review.

BACKGROUND: Nitric oxide (NO) is reported to play several protective roles in the inflammatory response and ischemia/reperfusion injury. This study evaluated the effect of nitric oxide (NO) on postoperative outcomes in adults undergoing cardiac surgery with cardiopulmonary bypass (CPB). METHODS: We systematically searched PubMed, Web of Science, and EMBASE (inception–April 2025) for relevant RCTs. Two reviewers independently screened studies, extracted data, and assessed risk of bias (Cochrane RoB 1) and evidence certainty (GRADE). Meta-analysis was performed using random-effects models. RESUL

Reduced- Versus Extended-Dose Anticoagulation for Venous Thromboembolism in Patients With and Without Cancer: A Systematic Review and Meta-Analysis.

BACKGROUND: Extended-duration anticoagulation reduces recurrent venous thromboembolism (VTE) in cancer and non-cancer patients. Reduced-dose direct oral anticoagulants (DOACs) may be safer if they match efficacy with less bleeding risk. We conducted a systematic review and meta-analysis of trials comparing reduced-dose and extended-dose DOACs. METHODS: Randomized controlled trials comparing reduced-dose with extended-dose anticoagulation for secondary VTE prevention were systematically reviewed. Primary outcomes included recurrent symptomatic VTE, pulmonary embolism (PE), and deep vein thromb

Assessment of adverse effects attributed to statin therapy in product labels: a meta-analysis of double-blind randomised controlled trials.

BACKGROUND: Statin product labels (eg, Summaries of Product Characteristics [SmPCs]) list certain adverse outcomes as potential treatment-related effects based mainly on non-randomised and non-blinded studies, which might be subject to bias. We aimed to assess the evidence for such undesirable effects more reliably through a meta-analysis of individual participant data from large double-blind trials of statin therapy. METHODS: In this meta-analysis of individual participant-level data from double-blind randomised controlled trials, we generated a list of all undesirable effect terms listed in

Tumor treating fields: Will the evidence gap ever be filled?

Tumor Treating Fields (TTFs) deliver low-intensity electric fields through adhesive skin arrays and have been approved in different oncology indications. However, concerns remain regarding the evidence supporting their benefit. Here, we highlight four limitations. First, TTFields utilization includes 24/7 assistance, home deliveries, and regular home-based interactions for array changes. In the absence of sham-controlled randomized trials, which are the equivalent of placebo-controlled trials for devices, the specific contribution of TTFields cannot be distinguished from the additional support

Preconception Interventions in Women at High Risk of Developing Gestational Diabetes: A Systematic Review.

INTRODUCTION: Gestational diabetes mellitus (GDM) is a common pregnancy-related complication. The modest benefits of interventions to prevent GDM in women with high risk during pregnancy has shifted the focus to the preconception period. However, research on the effectiveness of preconception interventions in women who are more likely to develop GDM is lacking. This review aimed to assess the effect of preconception interventions, including behavioural strategies, supplementation, and pharmacological treatments on reducing the incidence of GDM. METHODS: MEDLINE, EMBASE and the Cochrane Central

Extracorporeal membrane oxygenation in cardiogenic shock: evidence, limitations, and patient selection in the contemporary era.

Cardiogenic shock (CS) is a life-threatening condition with severely impaired myocardial contractility, causing systemic hypoperfusion, tissue hypoxia, and organ dysfunction. Despite the advances in revascularisation and pharmacological therapies, CS carries high mortality among patients with acute myocardial infarctions and other acute cardiac syndromes. Veno-arterial extracorporeal membrane oxygenation (V-A ECMO) offers short-term mechanical circulatory support in refractory CS by restoring systemic perfusion and maintaining end-organ oxygenation. This narrative review aims to explain the ph

Early versus late oral anticoagulants after acute ischemic stroke in atrial fibrillation patients: a meta-analysis of randomized controlled trials.

BACKGROUND: Atrial fibrillation (AF) occurs in about 20% of ischemic stroke cases. Although oral anticoagulants (OACs) reduce stroke risk in AF, the optimal timing for starting them after acute ischemic stroke remains uncertain, with varying recommendations across guidelines. METHODS: A systematic review and meta-analysis of randomized controlled trials was conducted using PubMed, Scopus, Web of Science, EMBASE, and Cochrane CENTRAL through October 2024. Studies included adults with AF and recent ischemic stroke, comparing early versus late OAC initiation. The primary outcome was recurrent is

Direct oral anticoagulants versus vitamin K antagonists for the treatment of cerebral venous thrombosis: A systematic review and meta-analysis of randomized clinical trials.

BACKGROUND: Current guidelines consider direct oral anticoagulants (DOACs) a reasonable treatment option for cerebral venous thrombosis (CVT), but randomized evidence comparing DOACs vs. vitamin K antagonists (VKAs) remain limited. OBJECTIVE: To compare the efficacy and safety of DOACs vs. VKAs for CVT treatment. METHODS: We conducted a systematic review and meta-analysis of randomized controlled trials (RCTs) comparing DOACs with VKAs in adults with CVT. Repositioning for pressure injury prevention in adults.

RATIONALE: Pressure injuries (or pressure ulcers) are localised damage to skin or tissue (or both) occurring over bony prominences, resulting from prolonged pressure or shear forces (or both). Adults receiving care in any healthcare setting can develop pressure injuries. Immobility, malnutrition, and reduced sensation are some known risk factors. Regular repositioning is a theoretically sound prevention strategy that is part of standard patient care. This is the second update of a review published in 2014 and updated in 2020. OBJECTIVES: To evaluate the benefits and harms, and cost-effectiven

Viscoelastic Tests in Cirrhotic Patients Undergoing Invasive Procedures: A Systematic Review and Meta-Analysis of RCTs.

BACKGROUND AND AIMS: Bleeding risk in cirrhotic patients undergoing invasive procedures is traditionally assessed using conventional coagulation tests, which poorly reflect the rebalanced haemostatic state of cirrhosis and often lead to unnecessary transfusions. Viscoelastic testing (VET) provides a global assessment of coagulation and may enable more rational transfusion strategies. We performed a systematic review and meta-analysis of randomised controlled trials (RCTs) to evaluate the efficacy and safety of VET-guided transfusion strategies in this setting. METHODS: We systematically searc

Meta-Analysis of Randomised Controlled Trials on the Efficacy of Direct Oral Anticoagulants in Left Ventricular Thrombosis: An Updated Perspective.

BACKGROUND: Left ventricular thrombus (LVT) is associated with significant morbidity and mortality. Current guidelines recommend direct oral anticoagulants (DOACs) and vitamin K antagonists for LVT treatment, although these recommendations are based on limited randomised data. However, a substantial body of new evidence has emerged, prompting the re-assessment of the optimal anticoagulation strategy. AIM: This study aimed to conduct a systematic review and meta-analysis to assess the efficacy and safety profile of DOACs compared with warfarin in the management of LVT. METHOD: We systematical
