Optimising the dose of oral midazolam sedation for dental procedures in children: a prospective, randomised, and controlled study.

BACKGROUND: Midazolam sedation poses a significant dilemma in paediatric dentistry, which is to find out the optimal dosing with minimal undesirable adverse events. In this study, we aimed to compare the effect of three doses of oral midazolam (0.5, 0.75, and 1 mg/kg) on the sedative state and cooperative behaviour of children during dental treatment. We further compared completion rates, parent satisfaction, and all adverse events. DESIGN: Ninety children aged 3-10 years were randomised to three equal groups. Groups A, B, and C received 0.5, 0.75, and 1 mg/kg of oral midazolam, respectively.

Predicting 5-Year Breast Cancer Risk From Longitudinal Digital Breast Tomosynthesis: A Single-Center Retrospective Study.

Background: Imaging-based breast cancer risk prediction models primarily use full-field digital mammography (FFDM). Although digital breast tomosynthesis (DBT) has become a predominant screening modality in the United States, its potential for long-term breast cancer risk prediction remains underexplored. Objective: The aim of this study was to develop and evaluate a deep learning model that uses longitudinal DBT examinations to predict long-term breast cancer risk. Methods: This retrospective study included 313,335 DBT examinations from 161,077 women (mean age, 58.5 ± 11.7 years) between Janu

Functional health index of intrinsic capacity: multi-domain operationalisation and validation in the Singapore Longitudinal Ageing Study (SLAS2).

BACKGROUND: ad hoc approaches are used to create composite indexes of intrinsic capacity (IC) based on five domains recommended by the World Health Organization for healthy ageing. We examined how combinations of domain-specific measures determine measurement performances of composite IC indexes. METHODS: in this population-based prospective cohort study, community-dwelling older persons (N = 2,906) aged 55 years and above were recruited. We used 12 domain-specific measures: cognition (Mini-Mental State Examination, MMSE), psychological (Geriatric Depression Scale, GDS), locomotion (Timed Up-

Pharmacokinetics, safety, and efficacy of a single co-administered dose of diethylcarbamazine, albendazole and ivermectin in adults with and without Wuchereria bancrofti infection in Côte d'Ivoire.

BACKGROUND: A single co-administered dose of ivermectin (IVM) plus diethylcarbamazine (DEC) plus albendazole (ALB), or triple-drug therapy, was recently found to be more effective for clearing microfilariae (Mf) than standard DEC plus ALB currently used for mass drug administration programs for lymphatic filariasis (LF) outside of sub-Saharan Africa. Triple-drug therapy has not been previously tested in LF-uninfected individuals from Africa. This study evaluated the pharmacokinetics (PK), safety, and efficacy of triple-drug therapy in people with and without Wuchereria bancrofti infection in W

Aging with Traumatic Brain Injury: Effects of Age at Injury on Behavioral Outcome following Diffuse Brain Injury in Rats.

Development and aging are influenced by external factors with the potential to impact health throughout the life span. Traumatic brain injury (TBI) can initiate and sustain a lifetime of physical and mental health symptoms. Over 1.7 million TBIs occur annually in the USA alone, with epidemiology suggesting a higher incidence for young age groups. Additionally, increasing life spans mean more years to age with TBI. While there is ongoing research of experimental pediatric and adult TBI, few studies to date have incorporated animal models of pediatric, adolescent, and adult TBI to understand the

Inside a Programmer's Brain: Dissociating Code and Language Comprehension Networks Using a Novel fMRI-ESM Protocol.

Code comprehension and natural language share structural similarities but engage partially distinct neural systems. While language predominantly recruits the classical perisylvian language network, code comprehension has been linked to the multiple-demand system, associated with mathematical thinking and problem solving. We report the case of a professional software engineer with an IDH-mutant astrocytoma in the left middle frontal gyrus, a region where these networks converge. To address the potential risk of impairing computer programming abilities during surgery, we implemented a personaliz

Dogma Under the Knife: Challenging the Status Quo in Primary Total Knee Arthroplasty.

BACKGROUND: Primary total knee arthroplasty (TKA) is among the most successful procedures in orthopaedic surgery, reliably providing pain relief, functional improvement, and durable implant survivorship. Despite these achievements, approximately 10 to 20% of patients report persistent dissatisfaction following primary TKA, even in the absence of technical failure. This disconnect between implant longevity and patient-perceived success has prompted renewed examination of the foundational principles that have historically guided TKA practice. Advances in implant design, biomaterials, surgical te

Exploring the Mechanisms of in-Hospital Emergency Department Diversion: A Qualitative Single-Case Study.

Healthcare workers (HCWs) are at risk of substance use disorders (SUD) due to factors such as burnout, stress, and occupational trauma, which may contribute to drug diversion (theft or unauthorized redirection of controlled substances), placing HCWs at risk of overdose, death, professional discipline and legal consequences. Within healthcare settings, workplace social relationships may facilitate access to controlled substances. However, these dynamics are complex, and such relationships may reflect genuine personal, professional, and peer connections rather than solely attempts to obtain cont

Epitranscriptomics in Breast Cancer: The Unveiled Role of RNA Modifications.

Epitranscriptomics, the study of dynamic chemical modifications on RNA molecules, has emerged as a pivotal layer of gene expression regulation with profound implications for cancer biology. This review centers on three well-characterized RNA modifications, N6-methyladenosine (m6A), pseudouridine (Ψ), and 5-methylcytosine (m5C), and highlights their diverse roles in the pathogenesis of breast cancer. These modifications modulate critical post-transcriptional processes such as RNA splicing, stability, translation, and degradation, thereby influencing tumor initiation, progression, metastasis, th

Enhanced Post-Prandial Glycemic Response Prediction in Type 2 Diabetes with Microbiome Data and Deep Learning.

Nutritional intervention can improve glycemic control for type 2 diabetes mellitus (T2DM), and thus accurately predicting post-prandial glycemic responses (PPGRs) to each meal is essential. PPGRs can vary significantly between individuals, even when consuming the same foods, due to the diverse and complex nature of individual characteristics. However, to date, system-scale studies investigating the variability of PPGRs in people living with T2DM are scarce. This research collected meal logs, continuous glucose monitoring records, clinicodemographic profiles, and gut microbiota data comprising

Multi-omic characterization of nasopharyngeal carcinoma delineates the subtype-specific landscape of response to induction chemotherapy.

Gemcitabine and cisplatin (GP) serve as first-line induction chemotherapy (IC) for nasopharyngeal carcinoma (NPC), yet predictive markers are lacking. Here we performed multi-omics profiling, including proteomics, phosphoproteomics, genomics and transcriptomics, on 240 patients with NPC who were receiving GP-IC or concurrent chemoradiotherapy (CCRT) alone. Through multi-omic integration, we identified three proteomic subtypes with distinct therapeutic vulnerabilities. The S1 subtype showed a predominant interferon-γ response and had favorable outcomes with CCRT alone. The S2 subtype featured c

Expert review and recommendations on the diagnosis and therapeutic management of eosinophilic granulomatosis with polyangiitis.

Eosinophilic granulomatosis with polyangiitis (EGPA) is a rare and heterogeneous immune-mediated disorder in which eosinophilic inflammation, vasculitis, and airway disease coexist in highly variable combinations. In clinical practice, diagnosis remains challenging because no single biomarker reliably captures disease complexity, and therapeutic decisions must still be based on careful integration of clinical, laboratory, imaging, and histopathological findings. In recent years, EGPA management has evolved from a largely glucocorticoid-based approach toward a more phenotype-oriented strategy t

A 3D Human Bone and Bone Marrow-on-a-Chip Model for In Vitro Bone Remodeling and Immune Cell Maintenance.

We developed a human Bone-on-a-Chip (BOAC) model that integrates autologous primary immune and bone cells on native human bone substrates within a xeno-free, perfused environment. In contrast to existing models, which focus primarily on hematopoietic or stromal compartments, our system recapitulates key aspects of functional bone remodeling and enables the maintenance of mature immune cells (up to 42 days), with preserved functional responsiveness at defined time points during culture. Dynamic flow and sequential cell seeding facilitated osteoclast-mediated resorption, osteoblast-driven matrix

Students' Use of Mindfulness During Full-Scale Immersive Simulation Training: A Qualitative Analysis.

BACKGROUND: This qualitative secondary analysis explored how medical students used mindfulness strategies during a high-stress, multi-day military simulation. Mindfulness training has been increasingly incorporated into medical education to enhance stress management, emotional regulation and performance under pressure, but its application in high-intensity simulation environments remains underexplored. METHODS: Ninety-six medical students completed a single open-ended survey question following the simulation exercise. Responses were analysed using a phenomenologically informed thematic approa

TriOmicNet: A Novel Multi-Layer Network Diffusion Approach Integrating Three Types of Scores to Identify Cancer Driver Genes.

Cancer is a complex disease driven by genetic mutations, and identifying driver genes is crucial for discovering key biomarkers and developing personalized therapies. In this study, we propose TriOmicNet, a multi-layer network diffusion method that integrates gene mutation, gene expression, and miRNA expression data. Unlike traditional methods that rely on a single data source, TriOmicNet combines three scoring mechanisms: regulatory potential, control ability, and multi-network diffusion. In addition, a node-scoring strategy is used to determine the seed nodes of the random walk algorithm. Co

A Surface-Enhanced Raman Scattering and Fluorescence Seesaw Nanosensor for the Detection of Cathepsin B Activity in Breast Cancer Cells.

Cathepsin B (CTSB) plays a key role in several processes that promote breast cancer progression, making its activity detection crucial for cancer analysis. In this study, we developed a surface-enhanced Raman scattering (SERS) and fluorescence seesaw nanosensor for probing CTSB activity in breast cancer cells. The nanosensor, termed Au-pep-TAMRA, was fabricated by conjugating carboxytetramethylrhodamine (TAMRA)-labeled peptide substrates (pep-TAMRA) to gold nanoparticles (Au NPs). The peptide substrates served dual functions: (1) as recognition units specifically cleavable by CTSB and (2) as l
