Polysomnography.

Polysomnography refers to a systematic process used to collect physiologic parameters during sleep. A polysomnogram (PSG) is a procedure that utilizes electroencephalogram, electro-oculogram, electromyogram, electrocardiogram, and pulse oximetry, as well as airflow and respiratory effort, to evaluate for underlying causes of sleep disturbances. PSG is considered to be the gold standard for diagnosing sleep-related breathing disorders, which include obstructive sleep apnea (OSA), central sleep apnea, and sleep-related hypoventilation/hypoxia. PSG can also be utilized to evaluate for other sleep

The impact of sleep disruption on executive function in Down syndrome.

The high prevalence of sleep disorders, particularly obstructive sleep apnea, is well established in children with Down syndrome. However, only a few studies have focused on older children and young adults in this population. Given the presence of sleep disorders and the early emergence of Alzheimer's disease, more work is needed to examine the relationship between sleep and cognition in Down syndrome. Twenty-nine adolescents and young adults with Down syndrome participated in the present study. Parents reported on their sleep difficulties using a well-validated measure of sleep problems in in

The role of mouth tape for CPAP use in patients with mouth breathing and OSA.

STUDY OBJECTIVES: We assessed the use of mouth tape in patients with mouth breathing during continuous positive airway pressure (CPAP) therapy for obstructive sleep apnea, focusing on CPAP adherence. Additional outcomes included daytime sleepiness (Epworth Sleepiness Scale), snoring (visual analog scale), night awakenings, mouth/throat dryness, leakage, and adverse effects. METHODS: This randomized crossover study compared CPAP adherence with and without silicone hypoallergenic mouth tape. Participants used each intervention for 30 days, separated by a 1-week washout period. After each period

Posterior fossa decompression in patients with Chiari malformation type 1: effect on sleep apnea and follow-up outcomes.

PURPOSE: Sleep apnea, posing significant health risks, is frequently associated with Chiari malformation (CM), characterized by cerebellar tonsil herniation through the foramen magnum. Central sleep apnea (CSA) in CM results from impaired brain-to-muscle signaling and requires treatment. Conversely, obstructive sleep apnea (OSA), arising from throat muscle relaxation, typically unrelated to CM, often coexists. This study evaluates the effectiveness of posterior fossa decompression (PFD) on sleep apnea. METHODS: A retrospective chart review was conducted of pediatric patients with CM-1 and sle

Influence of pulmonary capillary wedge pressure on central apnea in heart failure.

BACKGROUND: Recent studies suggest that acute pulmonary congestion induces hyperventilation and that hyperventilation-related hypocapnia leads to ventilatory control instability and central sleep apnea. Whether chronic pulmonary congestion due to congestive heart failure (CHF) is associated with central apnea is unknown. We hypothesized that CHF patients with central apnea would have greater pulmonary capillary wedge pressure (PCWP) than patients without central apnea and that PCWP would correlate with central apnea severity. METHODS AND RESULTS: Seventy-five stable CHF patients underwent rig

Polysomnographic values in children 2-9 years old: additional data and review of the literature.

The establishment of normal pediatric polysomnographic parameters is important for both clinical and research interests. Our objectives were to describe respiratory events, paradoxical breathing, periodic limb movements, and sleep architecture of children at the age of peak incidence of obstructive sleep apnea syndrome. We performed a retrospective cross-sectional analysis of a prospective cohort study of 66 children, 2-9 years old, at the Sleep Disorders Center at the Children's Hospital of Philadelphia. Subjects screened by questionnaire underwent a standard polysomnogram. The percent of tot

Dysphagia severity is associated with worse sleep-disordered breathing in infants with Down syndrome.

STUDY OBJECTIVES: Hypotonia, commonly seen in infants with Down syndrome (I-DS), can contribute to masticatory and oropharyngeal muscle weakness, increasing the risk for dysphagia and sleep-disordered breathing. Data describing the occurrence of dysphagia and sleep-disordered breathing in I-DS are limited. This study aims to determine the frequency and severity of dysphagia and its relationship to polysomnogram parameters in I-DS. METHODS: We included I-DS who underwent polysomnography at a single academic center over a 6-year period. Data collected included sex, age, presence of dysphagia (l

Hypervolemia and Sleep Apnea in Kidney Disease.

In end-stage renal disease (ESRD) and heart failure, conditions characterized by fluid overload, both obstructive sleep apnea (OSA) and central sleep apnea (CSA) are highly prevalent. This observation suggests that fluid overload may be a unifying mechanism in the pathogenesis of both OSA and CSA in these conditions. An overnight rostral fluid shift from the legs to the neck and lungs has been shown to contribute to the pathogenesis of OSA and CSA, respectively, in various different patient populations. This article reviews the evidence that supports a role for fluid overload and overnight flu

Performance of wrist-worn home sleep apnea testing (watch-PAT) among individuals with chronic insomnia: a comparative study with polysomnography.

BACKGROUND: While comorbid insomnia and sleep apnea are frequently encountered in clinical practice, the accuracy of home sleep apnea testing in this particular scenario has long been overlooked. Our study aims to evaluate the performance of a wrist-worn device in diagnosing sleep apnea among individuals with chronic insomnia. METHODS: Participants with chronic insomnia suspected of having sleep apnea were consecutively enrolled at a sleep center. Single-night Watch-PAT 200 monitoring was conducted in parallel with attended in-lab polysomnography. RESULTS: Recordings from 44 participants (41

What is central sleep apnea?

Central sleep apnea (CSA) describes a group of conditions in which cessations in air flow occur without respiratory effort. In contrast, obstructive sleep apnea patients have ongoing respiratory effort during respiratory events. However, considerable overlap exists in the pathogenesis and clinical presentation of obstructive sleep apnea and CSA. A good working knowledge of the mechanisms underlying CSA is important for optimal clinical care. In general, CSA can be classified into those with excessive drive (eg, Cheyne-Stokes breathing) versus those with inadequate drive (eg, sleep hypoventilat

Obstructive sleep apnoea in obesity: A review.

Obstructive sleep apnea is a common comorbidity that occurs in individuals with obesity. It classically manifests with excessive daytime sleepiness, resulting in reduced quality of life, workplace productivity, and an increased risk of motor vehicle accidents. Weight gain plays an important role in its pathogenesis through worsening upper airway collapsibility, and current treatment options are targeted towards mechanically overcoming upper airway obstruction and weight loss. Continuous positive airway pressure therapy remains the most widely prescribed treatment for obstructive sleep apnea bu

The correlation between ubiquitin in the brainstem and sleep apnea in SIDS victims.

BACKGROUND: The sudden infant death syndrome (SIDS) is still the main cause of postneonatal infant death and its etiology has stimulated many competing theories, among which is the role of hypoxia and brainstem abnormalities. One report claims an increased in ubiquitin in the liver of SIDS victims, ubiquitin being one of the heat-shock proteins. The correlation between ubiquitin in the brainstem and sleep apnea in SIDS was investigated here. MATERIALS AND METHODS: Among 27,000 infants studied prospectively to characterize their sleep-wake behavior, 38 infants died under 6 months of age, inclu

Effect of CPAP or oxygen on high loop gain sleep apnea and associated blood pressure change- an analysis using respiratory self-similarity.

BACKGROUND: Respiratory high loop gain (HLG) is an important driver of obstructive apnea and may be modified by continuous positive airway pressure (CPAP) or supplemental oxygen. Respiratory self-similarity is a marker of expressed HLG. METHODS: The baseline and on-therapy (CPAP, oxygen, healthy lifestyle and sleep education, HLSE) follow-up home sleep apnea tests (HSAT) from the Heart Biomarker Evaluation in Apnea Treatment (HeartBEAT) study were analyzed using self-similarity based on the respiratory effort signal. Linear regression analysis assessed the role of treatment efficacy in improv

Validation of a deep learning-based system for sleep staging, arousal detection, and respiratory event scoring in atrial fibrillation patients.

PURPOSE: Obstructive sleep apnea is a common yet underdiagnosed sleep disorder in patients with atrial fibrillation (AF). Although polysomnography (PSG) is the gold standard, its costs and complexity limit routine use. Home sleep apnea testing (HSAT) is a more accessible alternative, but it lacks electroencephalography (EEG), limiting direct assessment of sleep stages and arousals. This study evaluates the performance of DeepRESP, an automated scoring solution that infers sleep stages and arousals using respiratory inductance plethysmography (RIP) signals and scoring respiratory events, in a c

[Central sleep apnea syndrome and Cheyne-Stokes respiration].

This overview discusses pathogenesis, clinical presentation, prognostic implications and therapy of central sleep apnea with special reference to Cheyne-Stokes-Respiration or periodic breathing. In contrast to obstructive sleep apnea due to upper airway collapse during sleep, central sleep apnea (CSA) is mainly due to an instability of the breathing control system. Causes of central sleep apnea include alveolar hypoventilation disorders, heart failure, neurologic and autonomic disorders and idiopathic forms of CSA. Patients with idiopathic CSA often complain of insomnia and awakening during sl
