A simple and reliable low dose radiologic method developed by the author was used to measure leg length inequality of 798 patients with chronic and therapy resistant low-back and/or unilateral hip symptoms and 359 symptom free subjects. Statistically highly significant correlations of the symptoms and leg length inequality were observed. In the majority (79 and 89%), the chronic or recurrent sciatic pain and unilateral hip symptoms occurred on the side of the longer lower extremity. When correcting the leg length inequality simply with an adequate shoe lift, a permanent and mostly complete all

From a retrospective study of 412 thoracolumbar injuries, the author introduces the concept of middle column or middle osteoligamentous complex between the traditionally recognized posterior ligamentous complex and the anterior longitudinal ligament. This middle column is formed by the posterior wall of the vertebral body, the posterior longitudinal ligament and posterior annulus fibrosus. The third column appears crucial, as the mode of its failure correlates both with the type of spinal fracture and with its neurological injury. Spinal injuries were subdivided into minor and major. Minor inj

Both Down's syndrome and juvenile rheumatoid arthritis can cause instability of the cervical spine. Patients with Down's syndrome usually have instability of the atlantoaxial joint. With juvenile rheumatoid arthritis, cervical instability is a manifestation of a more generalized process involving ligamentous laxity, posterior facet ankylosis, and growth disturbances of the vertebral bodies. When the two syndromes occur in the same patient, the potential exists for serious cervical instability. In the 14-year-old girl reported here, the two conditions coexisted, and more severe cervical spine i…

An experimental computer program capable of reformatting stored display data from a CT scanner into true cross-sectional images of the spine has been clinically tested over a 1 year period. With this program, tomographic planes exactly parallel to the vertebral end plate can be imaged at the lumbosacral level even in patients who are markedly rotated or have scoliotic deformities. The reformatted image planes are tilted in the dorsoventral and mediolateral directions to compensate for lordosis or scoliosis. The reformatting can also produce images in coronal and sagittal planes on axes other t…

In 188 patients with rheumatoid arthritis 585 radiological examinations of the cervical spine were performed. Subluxation of C1-2 was found more often than expected; it was often the only radiological change in that segment, and its degree was not related to the duration of the disease. Lateral radiographs taken in full flexion were important for the diagnosis. Age over 40 years at the onset of RA, increased ESR at least on one occasion and seropositivity were related to more severe degrees of involvement. The presence of subcutaneous nodules, high titres of the Rose-Waaler reaction and the pr…

This retrospective study assesses the complications affecting the cervical spine after halo-pelvic traction in 83 patients who were followed up for a minimum of five years. Forty-four patients (53 per cent) had significant cervical complications such as radiological degenerative changes, avascular necrosis of the dens, loss of movement, pain or spontaneous fusion. The most important predisposing factors were a long period in the halo-pelvic apparatus, tuberculous kyphosis, stiffness of the spinal deformity and an age of 15 years or more at the time of application.

The operative results of 79 patients subjected to posterior fusion of the lumbosacral spine were evaluated on the basis of a follow-up examination performed on average 5.4 years after operation. A method of evaluation was developed which measured the patients' subjective improvement and working capacity. The method proved appropriate for clinical use and gave a more correct picture of the operative result than methods relying on the patients' own opinion or the radiographic assessment as the only criterion. Sixty per cent of the patients were subjectively improved, 40 per cent had returned to …

An attempt was made to ascertain all the dwarfs in the State of Victoria. The incidence of achondroplasia proved to be approximately 1 in 26,000 live births in the period 1969 to 1975 when ascertainment was nearly complete. This indicates a mutation rate of 1.93 X 10(-5) per generation in this locus. Paternal age was shown to influence mutation. Ascertainment in earlier years of the study was low despite the very great effort made to find all cases. Patients with hypochondroplasia were particularly difficult to find. However, 25 cases were found for study. Overlap between hypochondroplasia and

Ossification of the posterior longitudinal ligament in the cervical region is generally regarded as a rare disease, though a Japanese series of 185 cases has been reported. The main symptoms arise from a myelopathy, the degree of which varies from moderate to severe, due to stenosis of the spinal canal. In order to clarify the relationship between the severity of symptoms and the cross-sectional area of ossification, axial transverse tomography of the cervical spine has been carried out on twenty-six patients at intervals of 5 millimetres over the full extent of the ossification. In this way t…

Two cases of bilateral sacroiliac joint dislocation with intrapelvic intrusion of the intact lumbosacral spine and sacrum are reported. This condition is a distinct traumatic entity. The essential features of this condition are the disruption of the pelvic ring with bilateral sacroiliac dislocation. The spine as a whole descends into the depths of the pelvis. Disruption of the anterior structures but not always the pubic symphysis is apparently a prerequisite for the development of the condition. Extensive soft-tissue damage is part of the injury. The etiology is a direct crushing violence to …

This report reviews the outcome of surgery for back pain in 50 patients who had had previous back surgery. All were treated by exploration and posterolateral fusion of the spine. Those with nerve root irritation had thorough nerve root decompression including wide laminectomy when needed. Back pain was relieved in 44 of 50 patients, and sciatic pain was relieved in 38 or 43 patients. Postoperative functional activity (return to work) was regained in 24 of the 30 patients in whom this could be assessed. In six patients with back pain and in five patients with sciatic pain surgical benefits were…

Two complications of Paget's disease are presented : they include an osteogenic sarcoma and a giant-cell tumour (GCT) of the lumbar spine with a radiculo-medullary compression syndrome. The rarity of sarcomatous degeneration of the spine affected by Paget's disease is emphasized and also the unusual character of the spinal GCT which develop in the presence of Paget's disease. As in the present case, it is often difficult to confirm the benign or malignant character of a GCT and the principal criteria of this classification are discussed.

Observation over clinical features distinguishing the course of traumas involving the sacro-lumbar vertebral column and spinal cord in 251 patients permitted it to recognize a number of points determining the orientation of a complex restorative treatment. Thanks to the use of a set of medico-rehabilitating measures it was possible to achieve positive results in the treatment of 222 (89 per cent) of the patients and 178 (75 per cent) of them could move without any outside help.

A brief review is presented of the Danish medieval skeletons with leprosy changes, particularly in the skulls, where the pathologic specific changes found are termed facies leprosa. The clinical aspect of facies leprosa is termed Bergen Syndrome I (nasal leprosy) and Bergen Syndrome II (the leprogenic changes of the alveolar process of the maxilla superior with loosening and/or loss of the frontal incisors). An account is given of the interim result of a clinical examination of 333 inpatients at the Prapradaeng Leprosarium in Bangkok, Thailand. In the group of 258 patients with lepromatous lep
