Nightmare reduction in a Vietnam veteran using imagery rehearsal therapy.

Following exposure to traumatic events, approximately 19% of combat veterans develop posttraumatic stress disorder. One of the main symptoms of this mental illness is reexperiencing the trauma, which is commonly expressed in the form of chronic trauma-related nightmares. In these patients, nightmares can fragment sleep, decrease sleep quality, and even cause fear about going to sleep. One promising psychological treatment for chronic nightmares is imagery rehearsal therapy. Imagery rehearsal therapy presumes that nightmares are a learned behavior and that activating the visual imagery system m

The use of a synthetic cannabinoid in the management of treatment-resistant nightmares in posttraumatic stress disorder (PTSD).

UNLABELLED: This is the report of an open label clinical trial to evaluate the effects of nabilone, an endocannabinoid receptor agonist, on treatment-resistant nightmares in patients diagnosed with posttraumatic stress disorder (PTSD). METHODS: Charts of 47 patients diagnosed with PTSD and having continuing nightmares in spite of conventional antidepressants and hypnotics were reviewed after adjunctive treatment with nabilone was initiated. These patients had been referred to a psychiatric specialist outpatient clinic between 2004 and 2006. The majority of patients (72%) receiving nabilone ex

Long-term psychosocial sequelae of stillbirth: phase II of a nested case-control cohort study.

Stillbirth is associated with increased psychological morbidity in the subsequent pregnancy and puerperium. This study aimed to assess longer-term psychological and social outcomes of stillbirth and to identify factors associated with adverse outcome. We conducted seven-year follow-up of a cohort of women who were initially assessed during and after a pregnancy subsequent to stillbirth, together with pair-matched controls. All women were living with a partner at baseline and none had live children. Measured outcomes at follow-up included depression, posttraumatic stress disorder (PTSD) and par

Psychological services for returning veterans and their families: evolving conceptualizations of the sequelae of war-zone experiences.

The provision of effective and timely behavioral health care for veterans returning from the conflicts in Iraq and Afghanistan has become the focus of national attention. In this special issue, attempts to provide psychological care for service members and their families are examined in light of three key constructs. First, it is contended that at no other time in history has more attention been paid to the psychological consequences of engaging in combat. Second, for the first time in recorded warfare, psychological morbidity is likely to far outstrip physical injury associated with combat. F

Impact of physical abuse on adulthood depressive symptoms among women.

This manuscript provides a systematic review of the literature to examine the relationship between childhood physical abuse and intimate partner physical abuse with adulthood depressive symptoms among women. Thirty-five studies that targeted women, measured depressive symptoms, childhood physical abuse, and intimate partner physical abuse were reviewed. Findings indicated an association between physical abuse experiences and depressive symptoms. However, the association of other risk factors, including other types of abuse, with depressive symptoms confounded this relationship. Recommendations

Child sexual abuse: can anatomy explain the presentation?

This article discusses a 4-year-old girl who displayed behavioral symptoms consistent with posttraumatic stress disorder. She was recently placed in foster care due to emotional and physical neglect. During her clinic visit, she disclosed being sexually abused by her father with a knife. Results of her general and anogenital physical examinations were normal. The case discussion proposes an explanation for how a maltreated child (1) develops behavioral problems, (2) has a normal genital examination despite the history of sexual abuse, and (3) has an implausible disclosure of her father hurting

Autobiographical memory for trauma: update on four controversies.

Empirical research since the year 2000 on trauma and autobiographical memory in adults is reviewed and related to four enduring controversies in the field: Whether traumatic memories are inherently different from other types of autobiographical memory; whether memory for trauma is better or worse than memory for non-traumatic events; whether traumas can be forgotten and then recalled later in life; and whether special mechanisms such as repression or dissociation are required to account for any such forgetting. The review concludes that trauma and non-trauma memories differ substantially, but

Brief cognitive therapy of nightmares and post-traumatic ruminations in a man with a learning disability.

OBJECTIVES: This is a case study of the treatment of two recurrent nightmares, one of which was accompanied by congruent post-traumatic daytime ruminations, in a man with a mild to moderate learning disability. METHOD: Treatment was by a cognitive intervention in which the patient was taught new positive endings to his dream stories. RESULTS: Both of the nightmares, as well as the post-traumatic ruminations, responded to treatment, and gains were maintained at 12-month follow-up. CONCLUSIONS: Cognitive treatment of nightmares can be successful even in a patient with very limited intellectua

PTSD and the law of psychiatric injury in England and Wales: finally coming closer?

With the increase in terrorism in several parts of the world, more people are exposed to traumatic events that could cause psychiatric injury either to them or to members of their families. In Britain, terrorist attacks or other catastrophes are not unknown; indeed, the case law relating to psychiatric injury is vast. However, the intersection between medicine and the law is minimal. The result is a law that lags behind the scientific evidence and, on occasion, may seem unfair.

An emotional Stroop effect to malingering-related words.

This study was designed to elicit an emotional Stroop effect in simulators of malingering. Student participants (mostly women) either did or did not feign mild brain trauma. A modified Stroop test was administered, using neutral and "malingering" words. Controls showed no difference on malingering versus neutral words; simulators performed significantly worse on malingering words. Further research must specify the best method of administration to elicit a malingering Stroop effect and assess the additional discriminative power it may provide when combined with other malingering-detection strat

Factorial validity of the Childhood Trauma Questionnaire in men and women.

In an effort to confirm the factorial validity of the Childhood Trauma Questionnaire (CTQ) across sex, the items from the CTQ for 916 university students were subjected to confirmatory factor analysis. Results indicated that the factor structure for the CTQ was significantly different for men and women. For women, the items from the Physical Abuse subscale did not create a stable factor and thus appear not to be conceptually valid. Conversely, for men, the five-factor model provided a relatively good fit to the data. This investigation provides important information regarding sex differences i

Psychological consequences of road traffic accidents in children.

Questionnaire information about possible psychological changes in the months following their accident was obtained for 45 school age children who had attended an Accident and Emergency Department because of their involvement in road traffic accidents. Central nervous system involvement was rare in this group. Short-term psychological consequences were common, but even after 4-7 months 33% were considered by their parents to still have a moderate reaction and a further 11% were considered to be severely affected. The children had high levels of post-traumatic stress symptoms, particularly avoid

Effects of the 1989 San Francisco earthquake on frequency and content of nightmares.

In a systematic evaluation of the effects of a natural disaster on nightmares, nightmare frequency was found to be about twice as high among 92 San Francisco Bay area college students as among 97 control subjects in Tucson, Arizona, after the 1989 Loma Prieta earthquake. Subjects in California had not only more nightmares in general but substantially more nightmares about earthquakes. Over a 3-week period, about 40% of those in the San Francisco Bay area reported one or more nightmares about an earthquake, as compared with only 5% of those in Arizona. However, nightmares about earthquakes were

[Neuropsychological sequelae of deportation to the Nazi concentration camps during the Second World War].

The essential neuropsychic consequences of the deportation to the nazi concentration camps of adults during the second world war are the "KZ syndrome", the survival syndrome (or persecution syndrome) and reactive schizophrenias. The "KZ syndrome" puts together a psychic asthenia, a progressive intellectual deterioration, anxiety, depressive mood and vegetative disturbances. It is brought about as a consequence of malnutrition and the psychic traumas suffered by those deported. It shows up mainly in the non-jewish deported. The survival syndrome is chronic, puts together anxiety, insomnia and n

Hyperbaric oxygen therapy as a neuromodulatory technique: a review of the recent evidence.

Hyperbaric oxygen therapy (HBOT) has recently emerged as a promising neuromodulatory modality for treating several neurological and psychological disorders. Various studies indicate that HBOT can promote brain recovery and neuroplasticity through the modulation of key cellular and molecular mechanisms. HBOT affects multiple primary pathways and cellular functions including mitochondrial biogenesis and function (increased Bcl-2, reduced Bax, and enhanced ATP production), neurogenesis (upregulation of Wnt-3 and VEGF/ERK signaling), synaptogenesis (elevated GAP43 and synaptophysin expression), an

Stress-induced plasminogen activator inhibitor-1 (PAI-1) as a blood biomarker and brain risk factor for PTSD.

Post-traumatic stress disorder (PTSD) is a severe stress-related psychiatric condition triggered by traumatic life-threatening events, characterized notably by an altered memory profile. Although clinically well-documented, no specific biomarker exists. This translational study identifies plasminogen activator inhibitor-1 (PAI-1) as a brain risk factor for PTSD, thereby supporting its potential as a blood-derived biomarker. Mice with genetically ablated PAI-1 were protected from developing a PTSD-like memory profile. Conversely, mice exhibiting PTSD-like cognitive impairment showed increased b

Cognitive reappraisal moderates the effect of combat or other exposures on negative behavioral health symptoms.

Understanding the individual differences that can buffer the impact of combat and other adverse exposures on deleterious behavioral health outcomes could lead to more targeted prevention and intervention efforts. Cognitive reappraisal, an antecedent-focused emotion regulation strategy, is linked to positive health outcomes such as lower levels of post-traumatic stress disorder, anxiety, and depression. This study examined the moderating effect of individual differences in cognitive reappraisal use on the association between combat exposure and behavioral health outcomes in active-duty U.S. Sol

Borderline personality disorder and sexual abuse: A systematic review.

Although sexual abuse (SA) is known to be frequent among borderline personality disorder (BPD) patients, few reviews evaluating that relationship have been published. This systematic review aimed to investigate SA (including adulthood) as a predictor of BPD diagnosis, clinical presentation and prognosis. Studies written in English or Portuguese from January 1997 until January 2017 were identified by searching the following keywords in three international databases: "borderline personality disorder" OR "borderline disorder" AND "sexual abuse" OR "sexual violence" OR "sexual victimization" OR "s

Exposure to potentially traumatic events in early childhood: differential links to emergent psychopathology.

OBJECTIVE: To examine associations between exposure to potentially traumatic events (PTEs) and clinical patterns of symptoms and disorders in preschool children. METHOD: Two hundred and thirteen referred and non-referred children, ages 24 to 48 months (MN = 34.9, SD = 6.7 months) were studied. Lifetime exposure to PTEs (family violence and non-interpersonal events) and recent stressful life events were assessed with the Preschool Age Psychiatric Assessment (PAPA) and Child Life Events Scale. Child psychiatric symptoms and disorders were assessed with parent-reports in the PAPA, a comprehensiv
