The innervated free toe web flap for clitoris reconstruction.

Clitoris reconstruction is often used in transgender reassignment. Most techniques now reconstruct the clitoris using glans pedicled on the dorsal phallic neurovascular bundle. Other techniques of clitoris reconstruction re-shape retained elements of the proximal segment of the corpora cavernosa, as well as techniques mobilising local flaps. We are presenting a new technique for clitoris reconstruction using an innervated free web space flap. The operative technique and outcome are illustrated and discussed in this case report.

A novel technique of all-inside arthroscopic triangular fibrocartilage complex repair.

Peripheral triangular fibrocartilage complex (TFCC) tears are amenable to repair. Limitations of current repair techniques include prolonged recovery and button or knot intolerance. We present a novel technique of an all-inside repair using existing technology (FasT-Fix; Smith & Nephew Endoscopy, Andover, MA) to circumvent these complications. This technique is faster, easily performed, safe, and potentially stronger than current repairs. Earlier motion and rehabilitation are instituted after this repair. The tear is debrided to stimulate angiogenesis. The FasT-Fix is inserted through the 3-4

Fournier's gangrene: a review of 43 reconstructive cases.

BACKGROUND: Fournier's gangrene is a rare and potentially fatal infectious disease characterized by necrotic fasciitis of the perineum and abdominal wall, along with the scrotum and penis in men and the vulva in women. Fournier's gangrene is a true surgical emergency. Skin loss can be very incapacitating and difficult to repair. METHODS: The authors reviewed retrospectively the clinical records of a series of 43 patients with Fournier's gangrene between the years 1985 and 2003 who, after initial treatment by the Departments of Urology and Surgery, were referred to the Department of Plastic Su

The advantages of free tissue transfer in the treatment of posttraumatic stiff knee.

BACKGROUND: Open fractures of the distal femur involving the joint, surrounding ligament, and soft tissues are among the worst types of injuries that may eventually lead to stiff knee. Release procedures have been described as the common treatment option for posttraumatic stiff knee. However, the importance of simultaneously applied free flaps to replace the pliable soft tissue around the knee joint has not been discussed previously. METHODS: Between 1996 and 2002, nine patients with posttraumatic severe stiff knees were operated on. All patients underwent release procedures, such as scar rem

Improvement of vision after a combined midfacial and maxillary distraction with a rigid external distraction device.

We report a rare case of visual improvement in a 19-year-old patient with Apert syndrome after segmental distraction with a rigid external distraction device. We used the Rigid External Distraction Device II System (R.E.D. II, KLS Martin) after Le Fort I and III osteotomies.

A clinical test to avoid sural nerve injuries in percutaneous Achilles tendon repairs.

Percutaneous repairs of Achilles tendon ruptures has gained popularity due the reduced incidence of wound complications, however its use is still limited by the high incidence of sural nerve injuries associated with these repairs. The only technique described to avoid this adverse event is to surgically expose the nerve peri-operatively. In our study, we describe and validate a clinical technique to identify the sural nerve. The sural nerve was mapped using this technique both clinically and by Ultrasound (US) in a cohort of male subjects with intact Achilles tendons. We demonstrated an excell

Mucocele of the paranasal sinuses as a cause of acquired orbital hypertelorism: the second case.

BACKGROUND: Orbital hypertelorism is defined as a lateralization of the total orbit. This condition is associated almost always with congenital abnormalities. An acquired total displacement of the orbit is almost impossible once the bony pillars of the craniofacial complex are established. The aim of this article was to discuss a case of a young man with paranasal sinus mucocele, who developed OHT. CASE DESCRIPTION: A 20-year-old man was admitted to us with orbital hypertelorism in 1993. The patient's examination revealed marked orbital hypertelorism, and no other abnormal finding was noted,

Treatment posibilities in ectromelia.

INTRODUCTION: Ectromelia is a congenital abnormality characterised by limb growth disturbances (aplasia or hypoplasia) during the period from 4th to 8th gestation week. CASE OUTLINE: We present a case of hemimezomelic longitudinal ectromelia of the right upper limb associated with other skeletal abnormalities, surgically treated. An important role in the management of this case is attributed to the complex rehabilitation programme done before and after each surgical intervention. CONCLUSION: The aim of the complex therapy is to diminish the permanent invalidity of these patients.

Soft tissue reconstruction of traumatic lower extremity injuries.

Traumatic injuries of the lower extremity vary from minimal lacerations to limb threatening open fractures. Complications and morbidity associated with these injuries can be significant. With appropriate treatment from simple wound closure to complex free tissue transfer morbidity can be minimized and the threat of limb loss can be averted. We review the present treatment of traumatic soft tissue injuries of the lower extremity.

Adverse health outcomes in offspring of mothers with cosmetic breast implants: a review.

BACKGROUND: To assess whether maternal cosmetic breast implants are associated with adverse health outcomes among offspring, the authors examined published findings of epidemiologic studies that addressed this hypothesis. METHODS: Four epidemiologic studies, all from Scandinavia, were identified. Women with breast implants were identified from existing public and private registers of patients, and their offspring were traced through nationwide population and birth registers. The studies included a total of 11,445 women with breast implants and 3248 children born after the mothers' implantatio

Surgical treatment of posterior cruciate ligament injury.

Successful posterior cruciate ligament (PCL) reconstruction is challenging because of the complex structures and difficult reconstruction techniques that are required. The reported results have been inconsistent. Variables that affect the results of surgery to restore PCL function include combined associated ligaments injury, difficulty to duplicate PCL anatomy, wide variation in broad femoral insertion footprint, difficulty in accurate placement of the transtibial tunnel, tunnel erosion, high internal graft stresses and graft elongation. The outcome of conservative treatment of isolated PCL i

Penile reconstruction by the free scapular flap and malleable penis prosthesis.

BACKGROUND: : Penile reconstruction has always been a challenging problem for plastic surgeons while facing patients with severe congenital deformities and gender dysphoria and those who have suffered penile loss because of trauma, self-amputation, malignancy, and so on. Since 1936, when Bogoras first constructed a total penis, attempts have been made by different techniques, including skin flaps or myocutaneous flaps. And with development of free tissue transfer and microsurgical techniques, various free skin flaps, such as the radial free forearm flap, the superficial inferior epigastric art

Utilization of skin flap for reconstruction of the genitalia after an electric burn.

The objective of this article is to describe a case of an electric burn to the genitalia causing scrotal and testicular lesion, and the subsequent reconstruction using a skin graft. The patient was a 10-year-old boy who was victim of an electric burn that harmed the genitalia. There was extended skin loss, penile, scrotal and partial testicular lesion. The treatment consisted of plastic surgery to reconstruct the genitalia with skin flaps grafted on the left thigh, the scrotum and the base of the penis. The patient recovered well and was discharged after two weeks. We concluded that in severe

[Improvement of tomographic reconstruction in bone SPECT].

AIM: The comparison between iterative reconstruction and filtered backprojection in the reconstruction of bone SPECT in the diagnosis of skeletal metastases. PATIENTS, METHODS: 47 consecutive patients (vertebral segments: n = 435), with suspected malignancy of the vertebral column, were examined by bone scintigraphy and MRI (maximal interval between the two procedures +/- 5 weeks). The SPECT-data were reconstructed with an iterative algorithm (ISA) and with filtered backprojection. We defined semiquantitative criteria in order to assess the quality of the tomograms. Conventional reconstructio
