We report a case of thyroid abscess in a 17-year-old girl. She was presented with swelling and pain in the region of the left thyroid lobe. The diagnosis was confirmed by ultrasonography and fine needle aspiration. After incision, evacuation of the pus and antibiotic treatment manifestations of disease disappeared. Barium swallow did not prove any hypopharyngeal fistula. The girl is now without any problems for 3 years. The patient had infectious mononucleosis 2 months prior development of the thyroid abscess. We contemplate that it may had changed the immunological state that facilitated this

Eagle syndrome includes elicitation of pain on swallowing, turning the head, or extending the tongue. The syndrome is thought to be caused by irritation of the glossopharyngeal nerve, most commonly caused by its impingement against an elongated styloid process. We present a rare case of a granular cell tumor presenting as Eagle syndrome. Granular cell tumors orignate from Schwann cells and are most common in the subcutaneous tissue of the head, neck, and oral cavity, especially the tongue. A granular cell tumor is typically benign and solitary, rarely malignant. The differential diagnosis, dia

An original surgical technique is proposed for management of primary laryngopharyngeal cancer. A special cut of the skin provides both effective operation on cervical fat, the tumor and reliable suturing of the operative wound. Plastic repair is performed by transplantation of mucosa and soft tissues from intact part of the larynx. This technique has been tested with satisfactory results in 21 patients with primary laryngopharyngeal cancer stage III-IV. Swallowing recovered in all the patients on postoperative day 10-16. As shown by a 13 month to 5.5 year follow-up 13 patients are alive withou

Möbius syndrome is typified by bilateral facial nerve palsies, usually with abducens palsies. We examined an infant with Möbius syndrome who had bifacial weakness and third nerve palsies, but intact abduction of both eyes. Lower cranial nerve involvement, leading to respiratory, swallowing, and cardiac difficulties, was also present. Pathologic examination of the brainstem showed absent or hypoplastic third, seventh, tenth, and twelfth nerve nuclei. The fourth, fifth, sixth, and eighth nerve nuclei were intact. In Möbius syndrome with ocular motor palsies, rarely the sixth nerve may be spared.

This article analyzes some of the causes and the mechanisms which provoke malnutrition during the symptomatic phase of AIDS. Dietetic advise to follow is specified for the following circumstances: weight loss; diarrhea; esophagus-related problems or difficulty in swallowing; if a patient suffers nausea or vomiting; when a patient suffers from anorexia or the feeling of being full quickly. This article also deals with nutritional supplements which may be used with these patients, when enteral or parenteral nutrition is indicated, and other treatment strategies which call for the use of appetite

We report on a female with a interstitial deletion of 10p13 and a phenotype similar to that seen with the 22q deletion syndromes (DiGeorge/velo-cardio-facial). She had a posterior cleft palate, perimembranous ventricular septal defect, dyscoordinate swallowing, T-cell subset abnormalities, small ears, maxillary and mandibular hypoplasia, broad nasal bridge, deficient alae nasi, contractures of fingers and developmental delay. This could indicate homology of some developmental genes at 22q and 10p so that patients with the velocardiofacial phenotype who do not prove to be deleted on 22q are can

Reflux of gastric acid and pepsins into the lower oesophagus causes symptoms such as heartburn and nausea, and tissue injury leading to erosive oesophagitis and stricture formation. This article reviews the mechanisms involved in protecting the oesophagus against acid-mediated injury, including the role of the lower oesophageal sphincter, secondary oesophageal peristalsis and swallowed saliva. The oesophageal mucosa has inherent abilities to resist acid damage, and recent data from three laboratories suggest a secretory function with local production of bicarbonate and mucus responsive to loca

Patients with cerebral palsy usually suffer from lack of coordination in the neuromuscular mechanism in their upper airway and digestive tract. Difficulty in swallowing and aspiration are common problems in these patients, and stridorous breathing sometimes develops as a secondary symptom. Laryngoscopic examination revealed that redundant tissue in the aryepiglottic fold area was the cause of stridor and upper airway obstruction in four patients with cerebral palsy. We report on these four patients in whom laser reduction of the redundant mucosa led to dramatic improvements in stridor. Seconda

A 32-year-old woman complained of swallowing difficulty after a general seizure. Neurological examination revealed unilateral palsies of the 9th, 10th, and 12th cranial nerves. CT, MRI and internal carotid artery angiogram were normal. Selective catheterization of the external carotid artery and ascending pharyngeal system suggested a cranial nerve ischaemic arterial syndrome. The apparent sparing of the eleventh nerve may be explained by the double vascularization of this nerve. This may also be related to the double innervation of the trapezius and sterno-cleido-mastoid muscles by the 11th n

In addition to conventional endoscopic operation procedures for vocal fold lateralisation, laser surgery has been used to perform endolaryngeal arytenoidectomy by individual laryngologists during the past few years. With respect to a regular swallowing function, the endoscopic technique presented in this paper preserves the arytenoid cartilage. Instead, a submucosal cordectomy is performed and the mucosal flap is glued onto the endolaryngeal wound. Tracheostomy is usually not required with this technique. 15 own cases are described, 14 of whom showed a sufficient respiratory result. Indication

A 41-year-old man was admitted to our hospital because of pharyngeal pain and vomiting after intentionally swallowing sodium hydroxide. Gradually esophageal stenosis took place. After 9 months from the taking poison the patient was operated. We tried blunt dissection of esophagus. But the procedure failed in because the wire of stripper was cut. So performed total esophageal resection under the right thoracotomy. The right side colon was used to re-construction through antero-sternal space that was anastmosed to cervical esophagus and stomach. The postoperative course was uneventful.

We report a patient who developed a delayed onset of oculopalatal myoclonus, characterized by synchronous oscillatory movements of the eyeballs, soft palate and the orbicularis oculi muscle, 5 months after a pontine hemorrhage. The involuntary oscillations did not affect the speech or swallowing but continued in sleep with decreased amplitudes. These oscillations also failed to respond to clonazepam therapy and the patient was unable to voluntarily suppress them for several minutes. A temporary association in synchronous titubation of the head was also observed. A characteristic vertical ocula

The function of the Eustachian tube under atmospheric pressure, on the grounds of our studies, is discussed as follows: (1) Criteria of natural valvular function of the auditory tube under a normal pressure environment. (2) Opening action of the Eustachian tube and mode of swallowing movement in comparison with soft palate movement. (3) High pressure environment and (opening action of) Eustachian tube. (4) Children undergoing tubing and their Eustachian tube function. (5) Ventilatory function of the Eustachian tube under normal pressure conditions. (6) Factors influencing changes in pressure i

We analyzed the extent, pattern, and evolution of neurologic dysfunction in 87 patients with heatstroke at the Mecca pilgrimage. Disturbance of consciousness and constricted pupils were seen in all patients. Deep coma, areflexia, and absent brainstem reflexes were seen in 25 patients (29%). Automatic complex movements (chewing, swallowing, and lip smacking) were seen in 17 patients (30%). Body shivering during cooling occurred in six patients (7%). Recovery was uncomplicated in 75 patients (87%). Two (2%) recovered but developed pancerebellar syndrome. Ten patients (11%) died.

A retrospective study of ten patients with median rhomboid glossitis (MRG) revealed the presence of a nonspecific inflammatory reaction of varying degrees of severity in the overlying palatal mucosa. Clinical records and photographs indicated that the palatal reactions physically approximated the underlying glossal lesions, suggesting a relationship between these entities. The nature of the relationship is uncertain, but it may involve a localized infection of the palatal mucosa by Candida species, from the MRG. Alternatively, irritation by soluble, possibly Candida-related factors from the MR

The choice of palliative therapy in patients with carcinoma of the lower esophagus and cardia is mainly between irradiation and surgery. The present report is a retrospective analysis of 40 consecutive patients who recovered after resection of the tumor. The operation was considered to be palliative in all patients. The ability to swallow in proportion to the duration of the remaining life is used as a parameter for the quality of life. After esophago-gastrostomy, the average survival amounts to 24 months, permitting the patient to enjoy an almost normal diet. In spite of the palliative intent

In a retrospective study of tracheoesophageal fistula repairs in the Oxford Region, 1968 to 1982, 64 consecutive cases were classified according to Waterston group and gap length (less than 2 cm or greater than 2 cm); where possible their current status was assessed by interview or questionnaire. Waterston group and gap length varied independently. Both affected the course of the first admission but thereafter, Waterston classification was a poor prognostic guide. However, those with a long gap were significantly more likely to have respiratory and swallowing difficulties and stunted growth.

Methyl salicylate (oil of wintergreen) in the form of candy flavoring was ingested by a 21-month-old male infant who subsequently developed vomiting, lethargy, and hyperpnea. A "swallow" of the solution resulted in a serum salicylate concentration of 81 mg/dL six hours after ingestion. The infant was treated with parenteral fluids and sodium bicarbonate and he recovered rapidly. Hazards associated with salicylate use in this form include lack of parental awareness of the substance's toxic potential, the attractiveness of the candy-like odor, and the availability of the liquid in non-child-resi

Standard treatment of infants with "wide gap" esophageal atresia usually requires a staged procedure consisting of gastrostomy at birth (with or without cervical esophagostomy) and then a bowel interposition between the oesophagus and stomach at 1 year of age. Because this approach procedures significant morbidity and swallowing dysfunction, an alternative method for treating the wide gap atresia is recommended. The upper and lower esophageal pouch can be elongated by preoperative bougienage. This technique allows successful primary esophageal reconstruction in infants with wide gap esophageal

The authors report 5 cases of bronchial cysts of the carina tracheae in girls. Symptomatic cysts were located on the left. Diagnostic delay was often very long: about 2 years between the first signs and surgery. When the cyst was on the left side, it was not directly visible on standard x-ray films or tomographies of the chest. Barium swallow shows a typical compression on the anterior part of the esophagus. In children with relapsing atelectasis or over distension, localized on the left, CT scan might allow for earlier diagnosis, as the density of bronchial cysts in high: about 50 Hounsfield
