A traumatic cause for acquired pes planus in children is uncommon and it is not often considered as a primary diagnosis. We describe a patient, who ruptured her plantar calcaneonavicular (spring) ligament and presented late with an acquired unilateral flat foot similar to a tibialis posterior rupture. Corrective surgery in the form of calcaneal osteotomy with iliac graft lengthening and medial advancement of tibialis posterior achieved excellent results with patient remaining asymptomatic and returning to routine activities. In the presence of an acute traumatic onset of unilateral pes planus,

BACKGROUND: Rotator cuff tears involving the subscapularis are less common than those involving the superior aspect of the rotator cuff. The purpose of the present study was to report the results of repair of isolated tears of the subscapularis.
METHODS: The records on eighty-four shoulders that had undergone open repair of the subscapularis tendon were reviewed. The mean age of the patients at the time of surgery was 53.2 years. The mean interval from the onset of symptoms to the time of surgery was 12.5 months. Fifty-seven tears were traumatic, and twenty-seven were degenerative. Twenty-thr

An 80-year-old woman with repeated hemarthrosis of the right shoulder was examined and treated arthroscopically. Plain radiographs indicated narrowing of the acromiohumeral interval (3 mm) with large acromial spur. Under fluoroscopic visualization, the impingement between the humeral head and the medial edge of the acromion was seen at 50 degrees abduction. The area of impingement of the humeral head seemed to be just medial to the greater tuberosity. Arthroscopically, a crater formation was seen at this site. Active bleeding was seen at the center of the crater, which was interpreted as the p

Leg pain in the athlete is common and has many different etiologies. The most common causes include muscle or tendon injury, medial tibial stress syndrome, stress fracture, and exertional compartment syndrome. Less common causes of leg pain include lumbosacral radiculopathy, lumbosacral spinal stenosis, focal nerve entrapment, vascular claudication from atherosclerosis, popliteal artery entrapment syndrome, and venous insufficiency. This article reviews the essential history and physical examination findings and the various causes of leg pain to help the clinician pinpoint the diagnosis and fa

In the period of 1976 to 1997 our clinic treated 33 patients after Colles fracture with ruptured extensor pollicis longus tendon. The occurrence of functional loss was observed after the trauma in 3 to 9 weeks. In 30 cases the surgical treatment of extensor indicis proprius tendon, in 2 cases a direct suture of the ruptured tendon was performed as a primary repair and in one patient a palmaris longus interposition was utilised.

The occurrence of full thickness rotator cuff tears (RCTs) at time of death, the size and distribution of those tears among the tendons of the rotator cuff, and the anterior to posterior and medial to lateral dimensions of the RCTs were determined. A subset of 57 shoulders from a group of 414 were found to have full thickness rotator cuff tears. The three-dimensional coordinates of specific bony landmarks and points defining the circumference of the RCT were digitized using a Flock of Birds DC electromagnetic tracking device. Bony landmarks were used to determine the separation between adjacen

A 10 mm wide ring graft from the second extensor compartment with periosteum from the floor of the sheath was used to correct bowstringing in six patients who sustained an isolated rupture of the A2 pulley. It was attached to the lateral rims of the sheath. Periosteum was used for additional graft fixation. Bowstringing was assessed by magnetic resonance imaging and ultrasound preoperatively and 19.5 months after surgery. It was corrected in five patients and improved in one. Pain was reduced from 35 to 7 points on a visual analogue scale. Digital circumference decreased from 76 to 71 mm. Flex

We report on the case of a bodybuilder and powerlifter who suffered from triceps tendon rupture complicated by acute ulnar nerve compression syndrome. The diagnosis was made clinically, radiologically, and sonographically. Ultrasound was helpful to demonstrate a large hematoma at the site of the injury. Early surgical intervention confirmed the presence of the hematoma compressing the ulnar nerve and led to a complete restoration of ulnar nerve and triceps muscle function. Few reports on distal triceps rupture have been published but its complication by acute ulnar nerve compression has not be

The purpose of this study was to evaluate the healing process of partial-thickness tears of the deep pectoral tendon in chickens which is considered to be equivalent to the human rotator cuff. The superior side (bursal-side) and the inferior side (joint-side) layers of the deep pectoral tendon of 80 adult chickens were lacerated in the same manner. The mRNA expression levels of alpha 1 (I) and alpha 1 (III) procollagen was at a level 2 weeks after laceration. The mRNA expression level of procollagen alpha 1 (I) was significantly higher than that of procollagen alpha 1 (III) in all experimental

Little is known about the fate of the donor site after the central 1/3 of the patellar tendon is harvested for anterior cruciate ligament reconstruction. This study evaluated the donor site in the patellar tendon at various times after a graft from the central 1/3 of the patellar tendon was harvested. Fourteen patients were studied with magnetic resonance images taken from 6 weeks to 2 years after anterior cruciate ligament reconstruction. Axial, sagittal, and coronal views of the patellar tendon were obtained. A second group of 8 patients who had previous anterior cruciate ligament reconstruc

Posterior tibial tendon (PTT) tears are the most common cause of unilateral flatfoot in adults. It is a chronic pathologic condition with a degenerative inflammatory character which usually affects middle-aged or elderly women, with no history of trauma. PTT tears occur more rarely in young patients, after an acute traumatic event. Clinically, PTT tears present with pain along the tendon, increased by weight bearing, swelling and functional limitation. PTT tears are surgically classified in three types: with tendon hypertrophy, with tendon atrophy and with complete tendon tear. The three diffe

Anterior cruciate ligament (ACL) reconstruction in adolescents with open physes remains a difficult problem for the orthopedic surgeon, especially in view of growing teenage participation in contact sports. Traditionally, treatment of ACL tears in adolescents has been conservative; the patient is advised to delay surgery up to several years for fear of damaging physes by drilling holes across them. Unfortunately, this waiting period may inflict irreparable knee damage. This paper suggests an ACL reconstruction technique that utilizes no drill holes, thus causing no harm to physes or other esse

Clarification of posttraumatic wrist complaints is relatively difficult. Symptoms are often dismissed as wrist sprain. In an arthrogram a communication between the inferior radio-ulnar joint and the radiocarpal joint may indicate an injury of the triangular disc. In elderly patients, however, this communication can be present due to degenerative changes. Arthroscopy helps to evaluate not only the condition of the triangular disc, but also the cartilage of the distal forearm, the proximal carpal row, and some of the intercarpal ligaments. The authors report the results of wrist arthroscopy in t

Several apparently conflicting mechanisms have been proposed to explain the seemingly spontaneous delayed rupture of the extensor pollicis longus tendon (EPL). The following case, the first of its kind of which we are aware, may help to clarify the relationships between these mechanisms. Traumatic hyperextension of a patient's wrist brought the styloid process of the third metacarpal into contact with Lister's tubercle, fracturing it. Chronic chafing of the EPL on the nonunited fracture's jagged surface seemed to cause its spontaneous rupture 18 months later. The long period between accident a

Arthrographic study of the knee was performed in six patients for diagnosis of acute rupture of the quadriceps tendon. All of the patients gave a history of attempting to recover from a fall. Pain and tenderness over the suprapatellar area were common findings in all patients. Four patients had a positive arthrogram demonstrating extravasation of the radiopaque material outside the knee along the quadriceps tendon. Of these four patients, two had complete loss of quadriceps function, but the other two were able to partially extend the knee when the gravity was eliminated. Two patients had nega

Both inversion stress examination under general anesthesia and arthrography are sensitive indicators of lateral ankle ligament pathology (91%, 96% resp.). Arthrography is significantly superior to inversion stress examination done under local anesthesia (96%, 70% resp.). Peroneus tendon sheath filling is always pathological, but although this does not always indicate calcaneo-fibular ligament rupture, such rupture cannot be excluded in its absence. Talar tilt difference cannot be correlated with the extent of ligamentous rupture and therefore inversion stress examination, while providing valua

In patients with tennis elbow, the common extensor tendon showed a pronounced reactive change consisting of mesenchymal cell proliferation along with aggregates of newly formed vascular channels. When studied ultrastructurally, many of the mesenchymal cells adjacent to the vascular channels were surrounded by a basal lamina, which is not normally seen around tenocytes. The cytoplasm of these cells showed features of both endothelial cells and tenocytes. It thus appeared, that the endothelial cells of the newly formed vascular channels were the source of proliferating mesenchymal cells differen
