One-stage Reconstruction with the Composite Anterolateral Thigh Flap for Skin and Extensor Tendon Defects of the Thumb by Snakebite.

Venomous snakebites can cause severe injury. The loss of tendon and skin of the hand is incredibly challenging for the surgeon. A single-staged reconstruction with the free composite anterolateral thigh flap is an acceptable option for a complex thumb injury. In this case, reconstruction for a 23-year-old patient with a complex cobra-induced thumb injury had failed to cover the defect with a skin graft. There was a limitation in choice, and the patient was treated with the free composite anterolateral thigh (ALT) flap and fascia lata flap in one stage to reconstruct both the extensor tendon an

Safety of immediate open traumatic extensor tendon repair performed at an emergency department minor procedure room.

INTRODUCTION: The superficial location of the extensor tendons makes them particularly vulnerable to lacerations. Patients most commonly present to the emergency department (ED) after these injuries. We aimed to measure the safety of immediate surgical repair of traumatic open extensor tendon injuries in an ED minor procedure room (MPR) under local anesthesia, and the associated post-operative infection and complication rates. MATERIALS AND METHODS: We retrospectively evaluated all patients undergoing traumatic open extensor tendon repairs in the ED MPR over a 3.75-years period. Data collecte…

Republication of "Open Repair of Acute Achilles Tendon Ruptures: Is the Incidence of Clinically Significant Wound Complications Overestimated?".

BACKGROUND: Conflicting evidence exists regarding the optimal management of acute Achilles tendon ruptures. Operative repair is thought to afford patients a lower risk of rerupture, albeit at a higher overall risk of wound complications. METHODS: A retrospective chart review of 369 consecutive patients undergoing open repair of acute Achilles tendon ruptures performed by a single foot and ankle fellowship-trained orthopedic surgeon was undertaken. Healing was classified as no complications, complications without prolonging treatment, complications requiring prolonged local treatment, and comp

Fibrolipoma of Flexor Digitorum Profundus Tendon of Ring Finger Causing Triggering and Intermittent Carpal Tunnel Syndrome.

INTRODUCTION: Lipomas are most common soft-tissue tumors but rarely are found to be symptomatic. <1% of lipomas are found in hand. Subfascial lipomas can cause pressure symptoms. Carpal tunnel syndrome (CTS) can be idiopathic or secondary to any space-occupying lesion. Triggering usually occurs due to inflammation/thickening of A1 pulley. Most have reported a lipoma in distal forearm or near median nerve, leading to triggering of index or middle finger and carpal tunnel symptoms. All the cases reported had either an intramuscular lipoma in flexor digitorum superficialis (FDS) tendon slip of in

Retrospective Study on the Risk of Nerve Injury After Distal Biceps Tendon Repair Using Cortical Button.

BACKGROUND: Distal biceps tendon ruptures are relatively rare injuries that typically require surgical intervention to restore flexion and supination strength. Concerns have been raised regarding the risk of nerve injuries, particularly the posterior interosseous nerve (PIN), associated with the use of cortical buttons in distal biceps repair. This study aimed to estimate the incidence of PIN injury as well as injuries to the lateral cutaneous nerve of the forearm and superficial branch of the radial nerve following distal biceps repair using cortical buttons. METHODS: A retrospective review

The Effect of Timing Between Traumatic Flexor Tendon Injury of the Hand and Surgical Intervention on Repair Failure Rates.

Flexor tendon injuries of the hand, especially in zone II, pose a challenge for hand surgeons because of the region's intricate pulley mechanism and local avascularity, and post-surgical complications such as repair failure are not uncommon. One proposed predictor of outcomes following flexor tendon repair has been timing of surgery from initial injury. However, the effect of the timing of flexor tendon repair on failure rates remains controversial and understudied. The purpose of this study was to compare the failure rates of zone II flexor tendon repairs in patients at various time intervals

Microfracture Lateral to the Greater Tuberosity of the Humerus Enhances Tendon-to-Bone Healing in a Rat Rotator Cuff Model.

BACKGROUND: Microfracture at the rotator cuff insertion is an established surgical marrow-stimulation technique for enhancing rotator cuff healing. However, the effect of lateralized or medialized microfracture on the insertion is unknown. PURPOSE: To compare the biomechanical and histologic effects of microfracture at 3 different regions for rotator cuff repair in a rat model. STUDY DESIGN: Controlled laboratory study. METHODS: A total of 72 Sprague-Dawley rats with bilateral supraspinatus tendon insertion detachment were allocated into 4 groups with 4 different interventions: no microfrac

Imaging and histological evaluation of the long head of the biceps tendon in the presence of different types of rotator cuff tears.

BACKGROUND: A comparison of changes in the long head of the biceps tendon for different types of rotator cuff tears has not been previously performed. Furthermore, the correlation between the thickening and degeneration of the long head of the biceps tendon and the cause of these changes have not been fully clarified. We evaluated the relationship between degenerative changes in the long head of the biceps tendon and rotator cuff tears in a rat model using imaging and histology. METHODS: Ninety-six 12-week-old Sprague-Dawley rats were divided into anterior (subscapularis tear), anterosuperior

Simulations suggest walking with reduced propulsive force would not mitigate the energetic consequences of lower tendon stiffness.

Aging elicits numerous effects that impact both musculoskeletal structure and walking function. Tendon stiffness (kT) and push-off propulsive force (FP) both impact the metabolic cost of walking and are diminished by age, yet their interaction has not been studied. We combined experimental and computational approaches to investigate whether age-related changes in function (adopting smaller FP) may be adopted to mitigate the metabolic consequences arising from changes in structure (reduced kT). We recruited 12 young adults and asked them to walk on a force-sensing treadmill while prompting them

A Minimally Invasive Technique for Harvesting Quadriceps Tendon Autograft With a Bone Plug for Anterior Cruciate Ligament Reconstruction.

Anterior cruciate ligament reconstruction (ACLR) with quadriceps tendon (QT) was first described decades ago. Recent studies have demonstrated superior graft characteristics (diameter, strength, and stiffness) and reduced postoperative morbidity. However, limited instrumentation options currently available to surgeons allow for minimally invasive QT harvest with a bone plug. As an alternative to traditional QT harvest techniques, we describe a surgical technique allowing for minimally invasive QT autograft harvest with a bone plug (QuadVantage Technologies, Inc). This approach can offer techni

The adductor magnus tendon is a viable option for medial patellofemoral ligament reconstruction, but with a busy surrounding neurovascular topography.

PURPOSE: To describe the proximity of the neurovascular structures surrounding the adductor magnus (ADM), to delineate a safe boundary focus … formed in a minimally invasive way. The study results are clinically relevant, as they suggest that tendons should be shorter than the minimum distance from the nerve. If in some cases the length of the MPFL is longer than the distance of the ADM from the nerve, the results suggest that a partial dissection of the anatomical structures might be needed. Di …

Periprosthetic joint infection after total hip arthroplasty induces histological degeneration of the gluteus medius tendon.

AIMS: A revision for periprosthetic joint infection (PJI) in total hip arthroplasty (THA) has a major effect on the patient's quality of life, including walking capacity. The objective of this case control study was to investigate the histological and ultrastructural changes to the gluteus medius tendon (GMED) in patients revised due to a PJI, and to compare it with revision THAs without infection performed using the same lateral approach. METHODS: A group of eight patients revised due to a PJI with a previous lateral approach was compared with a group of 21 revised THAs without infection, pe

Scapulothoracic tenodesis using hamstring tendon graft for treatment of problematic scapula winging: A new surgical technique.

INTRODUCTION: Winging of the scapula occurs due to dysfunction of its stabilising muscles, most commonly serratus anterior and/or trapezius, for example in facioscapulohumeral muscular dystrophy. Resultant loss of scapular control and abnormal kinematics can decrease shoulder function due to glenohumeral joint instability, loss of range of motion and pain. Previously described treatment for cases resistant to physiotherapy includes scapulothoracic arthrodesis which involves risk of non-union and metalwork failure, as well as reduced respiratory function due to immobilisation of a segment of th

Rupture of the Flexor Pollicis Longus Tendon from the Metacarpophalangeal Sesamoid and Joint in a Rheumatoid Patient: A Case Report.

CASE: A 54-year-old woman with rheumatoid arthritis presented with a flexor pollicis longus (FPL) rupture at the level of the metacarpophalangeal joint secondary to attritional damage from metacarpophalangeal (MCP) degenerative changes and exostoses from the radial sesamoid. She underwent direct tendon repair with debridement of the MCP joint and radial sesamoidectomy. CONCLUSION: Rheumatoid arthritis can potentially lead to rupture of the FPL tendon in locations distal to the carpus, namely at the level of the MCP joint. Contrary to other reports, a quality outcome may be obtained with direc

MRI appearance of multiple displaced osteochondritis dissecans cartilage fragments in the bicipital tendon sheath of a juvenile dog.

A 1-year-old Bernese Mountain dog presented with an acute onset of left thoracic limb lameness. Magnetic resonance imaging (MRI) of the left shoulder was performed, showing a subchondral bone defect in the caudomedial aspect of the humeral head. In addition, several round hypointense structures were visible in the biceps tendon sheath. A left shoulder arthroscopy was performed, which confirmed an osteochondritic lesion. Exploration of the biceps tendon sheath via a small open approach allowed retrieval of the fragments, which likely migrated from the joint. Histopathology confirmed the structu

Electromyography-informed musculoskeletal modeling provides new insight into hand tendon forces during tennis forehand.

Lateral epicondylitis, also known as tennis elbow, is a major health issue among tennis players. This musculo-skeletal disorder affects hand extensor tendons, results in substantial pain and impairments for sporting and everyday activities and requires several weeks of recovery. Unfortunately, prevention remains limited by the lack of data regarding biomechanical risk factors, especially because in vivo evaluation of hand tendon forces remains challenging. Electromyography-informed musculo-skeletal modeling is a noninvasive approach to provide physiological estimation of tendon forces based on…

Bifurcated distal biceps brachii tendon coexisting with separated bicipital aponeurosis: a complex variational case report.

Variations appearing in biceps brachii muscle are common with accessory head, different origins, variant insertion, and different pattern of nerve innervation. However, variations appearing in both origin and insertion, and with other anomalous morphology at the same time are seldom. Here we report a complex variational case on the right arm of a 91-year-old Japanese female cadaver. The complex variations included (1) the biceps brachii muscle bifurcated at its distal ending; (2) the long head had its own tendon, which divided into two parts, i.e., a lateral part fused into the fascia between

Finite element analysis of conductive hearing loss caused by fixation and detachment of ligament and tendon in the middle ear.

BACKGROUND AND OBJECTIVE: The fixation of ligament and tendon of the middle ear often occurs after chronic otitis media surgery. However, there are relatively few studies on the effect of ligament and tendon on sound transmission in the human middle ear. Here, the finite element model and lumped parameter model are used to study the effect of ligament and tendon fixation and detachment on sound transmission in human ear. METHODS: In this paper, the finite element model including the external auditory canal, middle ear and simplified inner ear is used to calculate and compare the middle ear fr
