Primary central nervous system vasculitis (PCNSV) is a vasculitis limited to the brain and spinal cord. Induction therapy often consists of steroids and cyclophosphamide. Maintenance therapy includes a prednisone taper and may be combined with medications such as azathioprine or mycophenolate mofetil. Relapse is common in PCNSV and an increased dose of steroids is often given, sometimes with a change in therapy. Medications such as rituximab and mycophenolate mofetil may be good alternatives in those who do not respond to initial treatment or who have relapse of disease. Mortality rates of 8% …

Frosted angiitis is a rare form of retinal vasculitis in which venous development gives it an appearance of perivascular frost. It is most often of idiopathic origin but can also be found in the context of infectious (most often viral) or inflammatory pathologies (lupus erythematosus, sarcoidosis or granulomatosis with polyangiitis). As with most vasculitis, frosty angiitis can be complicated by occlusion of the central retinal vein. Close monitoring is necessary as well as rapid treatment based on high-dose systemic corticosteroid therapy as well as intraocular injections of anti-VEGF compoun…

A 40-year-old woman with systemic lupus erythematosus (SLE) presented with high-grade fever and severe thrombocytopenia. Acalculous cholecystitis and thrombocytopenia were initially suspected to be complicated with SLE and vasculitis. Contrary to our expectation, however, the patient was finally diagnosed with Helicobacter cinaedi bacteremia. SLE patients show various symptoms, especially when their condition is complicated with vasculitis, which mimics H. cinaedi bacteremia. It is therefore difficult to provide a definite diagnosis. Physicians should be mindful of the presence of H. cinaedi i…

We report two unusual presenting manifestations of Wiskott-Aldrich syndrome (WAS), recurrent acute hemorrhagic edema of infancy (AHEI); a form of cutaneous vasculitis and hyperostosis of the tibia. Though cutaneous vasculitis is known to occur in WAS, presentation in early infancy and as AHEI is extremely uncommon. Hyperostosis is not a well-recognized association in WAS; only three patients with this association have been previously reported. In our patient these two unusual manifestations preceded the onset of recurrent infections. Recognition of this rare presentation led us to an early dia…

Wegener's granulomatosis (WG) is manifested by granulomatous necrotizing inflammatory lesions involving multiple organs. Limited WG is classification of WG with the absence of disease features that pose immediate threats to either a critical individual organ or to the patient's life. The most common skin lesions are palpable purpura, necrotic ulcerations, papules and nodules with many histological pattern, leukocytoclastic vasculitis, granulomatous vasculitis, and palisading granulomas. We report a patient with a limited form of WG who presented with a chronic large scalp ulcer that rapidly re…

Patients with polymyalgia rheumatica have been shown to have an increased risk of peripheral arterial disease on longitudinal follow-up. Possible explanations for this include premature atherosclerosis related to chronic inflammation, as with other inflammatory rheumatological conditions. Alternatively, polymyalgia rheumatica can be associated with vasculitis, even in the absence of clinical giant cell arteritis, and peripheral vascular disease may represent subclinical vasculitis. Further work is required to elucidate the reasons for this increased risk. Currently, it would remain reasonable …

Varicella is a common infectious disease; it is usually benign and self-limited, and complications are believed to be rare. A case is presented of a two-year-old girl who developed a left hemiparesis four weeks after the onset of chickenpox. Laboratory studies ruled out all known causes of stroke. MRI revealed the infarction, and MRA showed segmental narrowing of the right internal carotid artery, compatible with focal vasculitis. Based on the presumed diagnosis of varicella-associated cerebral vasculitis, the patient was treated with acyclovir, methylprednisolone and aspirin.

The best therapeutic strategy in virus-induced vasculitides should take into account the etiology of the disease and be adapted to the pathogenesis. The combination of antiviral treatments and plasma exchanges has been proven effective in polyarteritis nodosa (PAN). In human immunodeficiency virus (HIV)-related vasculitis this strategy is also effective and does not jeopardize, like cytotoxic agents, the outcome of AIDS. In vasculitis related to HCV-associated cryoglobulinemia, plasma exchanges improve the outcome but the poor effectiveness of antiviral drugs is not able to favor, usually, a d…

After identification of the hepatitis C virus (HCV) in 1989, evidence was established supporting its role in the pathogenesis of a number of cutaneous diseases. This evidence ranges from mere epidemiologic associations, such as lichen planus, to molecular biological investigations that have identified the virus in the pathologic tissues of cutaneous vasculitis, vasculitis with mixed cryoglobulinemia, and porphyria cutanea tarda. We describe a 52-year-old man who was diagnosed with chronic hepatitis C, preceding the appearance of lichen planus, erythema nodosum, and erythema multiforme that coi…

A case of panarteritis with purpura fulminans, mononeuritis multiplex, gastrointestinal manifestation and presumably cardiac involvement in a previously healthy 22-year-old man with a history of drug abuse including cocaine, cannabinoids and methamphetamines is described. Histopathological examination of the gut led to the diagnosis of panarteritis without immune deposits. Antineutrophil antibodies were negative. Besides the drugs, no other possible cause of vasculitis was found. The patient recovered completely after 1 year. Drug abuse is a thus possible cause of severe extracerebral disablin…

Antibodies against cytoplasmic antigens of neutrophils are a recently described group of autoantibodies associated with some types of primary systemic vasculitis and necrotizing glomerulonephritis. In clinical practice they are a useful laboratory examination which helps with the diagnosis of this group of diseases and to follow up their activity and response to treatment. The existence of ANCA implies also a new aspect of classification of vasculitis. The autoimmune reaction of ANCA with lysosomal enzymes is also another possible mechanism of development of tissue damage. The submitted paper …

Vasculitis is accepted to be the basis of Behçet's disease (BD) which is a multisystem disease, and the arachidonic acid(AA) metabolites acting as balancing mediators in the organism are accepted to be responsible for the vasculitis. In this study, we examined the prostaglandin E2 (PGE2) and leukotriene C4 (LTC4) levels of the patients with BD before and after colchicine therapy. We found a statistical decrease in the PGE2 and LTC4 levels after colchicine therapy compared to the previous levels, concluding that colchicine inhibits the inflammation and the polymorphonuclear leukocyte (PML) chem…

Seventy-five patients with suspected rheumatic diseases underwent intraarterial DSA of the hand arteries. In 81% of the patients we were able to establish the diagnosis of vasculitis according to angiographic criteria. Neither clinical nor chemical or immunological features allowed the diagnosis of an inflammatory disease of joint and perivascular tissue at this time. However, histological examination and immunofluorescence microscopy confirmed the angiographic findings and showed signs of an immune complex vasculitis. A strong predictor for an abnormal angiogram of the hand arteries is the pr…

Ocular involvement occurs in 40% to 50% of patients with Wegener's granulomatosis. The frequent involvement of the eyes in Wegener's disease is largely based on the anatomical contiguity of the orbit and the upper airways. On the other hand, it is also attributable to an isolated ocular involvement in the form of a focal necrotizing vasculitis. In about 60% of patients suffering from Wegener's granulomatosis there are anticytoplasmic antibodies. There seems to be a correlation between the serum level of ACPA and disease activity. In 10 patients out of 12 with ocular vasculitis due to Wegener's…

The incidence and characteristics of pulmonary haemorrhage in a series of 89 patients with systemic vasculitis were analysed. Pulmonary haemorrhage occurred in 32 of these patients and was associated with haemoptysis in all 32, alveolar shadowing in the chest radiograph in 28, and a significantly raised transfer coefficient in 30. Pulmonary haemorrhage usually resolved with treatment by immunosuppressive drugs but was the cause of death in 11 patients. In contrast with patients with antibasement membrane antibodies there was no correlation between pulmonary haemorrhage and cigarette smoking. P…

Prolonged pulmonary eosinophilia are defined by the association of a pneumonia and a blood eosinophilia persisting for over a month. They cover a wide range of disorders : prolonged Loeffler's syndrome, Carrington's chronic eosinophilia pneumonia, allergic bronchopulmonary aspergillosis, necrotizing vasculitis as Churg and Strauss allergic granulomatosis and angeitis and polyarteritis nodosa, and the hypereosinophilic syndrome. All these conditions may form a continuum centered by the poorly defined chronic eosinophilic pneumonia. This entity may result from a non treated Loeffler's syndrome o…

IgA nephropathy (IgAN) is a fairly common association with alcoholic liver disease. However, IgA vasculitis (IgAV) is quite an uncommon association with alcoholic liver cirrhosis and only a handful of cases have been reported in literature. Secondary IgAN usually presents in a docile manner, progressing slowly in about 5-25 years. It is usually responsive to steroid therapy, very rarely progressing to End-Stage Renal Disease. Here, we present a man in his late 50s, a known hypertensive and alcohol related liver-cirrhotic, who presented to our hospital with rash and rapidly progressive renal fa…

PURPOSE: To describe a case of outer retinitis with frosted branch angiitis associated with mumps infection treated with hyperbaric oxygen (HBO) therapy.
METHODS: Observational case report.
CASE REPORT: A four-year-old boy with bilateral blindness was diagnosed with necrotizing outer retinitis with frosted branch angiitis associated with serologically confirmed mumps virus infection. He was treated with HBO therapy. Visual acuity subsequently improved to 20/40 in the right eye and to 20/320 in the left eye. Sequential follow-up optical coherence tomography examinations showed progressive rec

In childhood, cutaneous small-vessel vasculitides include Henoch-Schönlein syndrome, a systemic vasculitis, and Finkelstein-Seidlmayer syndrome, a skin-limited vasculitis. Both Henoch-Schönlein and Finkelstein-Seidlmayer syndromes are seen more frequently in white or Asian compared with black children and occur especially in winter and spring with a male-to-female ratio of approximately 2:1. In everyday clinical practice, both conditions are diagnosed on clinical grounds without histological confirmation. The characteristic cutaneous hallmarks of Henoch-Schönlein syndrome include a purpuric ra…
