The ultimate development of mitral valve endocarditis: atrioventricular separation, atrioventricular groove abscess and hemorrhagic pericarditis.

A 52-year-old non-insulin-dependent diabetic man presented with cerebral emboli and mitral valve endocarditis with posterior leaflet vegetations and perforation. Surgical intervention demonstrated hemorrhagic pericarditis and an atrioventricular groove abscess. Extensive debridement of the pericardium, valve and abscess cavities, reconstruction of the mitral annulus with a patch of fresh autologous pericardium, and mitral valve replacement with a pericardial bioprosthesis was performed. The chest was left open. Postoperatively, the patient required dialysis and prolonged mechanical ventilation

Venous thromboembolic disease presenting as inferior vena cava thrombus extending into the right atrium.

With increasing use of echocardiography, especially transesophageal echocardiography, the diagnosis of intracardiac masses has surged. Masses that are most commonly seen in the atrial chambers include thrombi due to atrial fibrillation, cardiac myxomas often located in the atria, and valvular vegetations of infective endocarditis. In this report we present a case of a patient who developed thrombus in the inferior vena cava that extended up to the right atrium and presented as an apparent intracardiac mass. This mass embolized to the right pulmonary artery shortly after diagnosis resulting in

Pulmonary endarteritis and subsequent embolization to the lung as a complication of a patent ductus arteriosus--a case report.

The authors describe a case of pulmonary endarteritis and subsequent embolization to the lungs as a complication of a patent ductus arteriosus (PDA). Although 2-dimensional echocardiography has been shown to be of great value in the diagnosis of patients with infective endocarditis, echocardiographic detection of vegetation within the pulmonary artery and subsequent embolization to the lung is extremely rare and, to our knowledge, has been previously reported only in a few cases. In brief, our case not only shows the importance of echocardiography in making this rare diagnosis but also emphasi

[Neuroendocrine factors in pathogenesis of motion sickness and the protective effect of drugs].

The paper presents a generalization of many-year complex investigations performed at various departments of the Institute of Medico-Biological Problems (Moscow) and various laboratories and institutes of the Ministry of Public Health and the Russian Academy of Medical Sciences, devoted to the pathogenesis of motion disease (sea sickness, vestibulo vegetative syndrome, various forms of kinetosis). The main attention is given to interpretation of the mechanisms of therapeutic and prophylactic action of drugs used for the treatment of such disorders. In particular, the role of a hormonal componen

Bioecology of An. philippinensis in Andaman group of islands.

Studies on bioecology of An. philippinensis a vector of malaria was carried out in eight islands of the Andaman group. It was found that An. philippinensis preferred to rest and bite outdoors. Maximum biting was observed during 1800 to 2100 hrs on both cattle and human bait. The maximum breeding of An. philippinensis was recorded in slow moving stream followed by ponds with vegetation. The breeding sites infested with Chara and Spirogyra algae were most favourable whereas, with Lemna and Oscillatoria were unfavourable for the proliferation of this species. The breeding association of An. phili

Complete heart block resulting from quadricuspid aortic valve penicillin-resistant pneumococcal endocarditis: a case report.

A 62-year-old woman was referred to hospital to be treated for complete heart block with syncope. A quadricuspid aortic valve without vegetation and aortic valve regurgitation (AR) were observed by transesophageal echocardiography. Blood culture showed penicillin-resistant Streptococcus pneumoniae. The patient received a permanent pacemaker implantation (VDD type) and was given antibiotics. Although infection improved, heart failure was not improved because AR hemodynamic changes became worse. An aortic valve replacement was performed using a prosthesis on the 26th day of hospitalization. The

Multiple pulmonary artery aneurysms due to infective endocarditis.

Bilateral pulmonary artery aneurysms developed in the course of staphylococcal endocarditis in a 6-year-old girl with ventricular septal defect. Consecutive computed tomography scans revealed the progressive enlargement of one of the aneurysms. She underwent an urgent left upper lobectomy because of the impending rupture and a possible life-threatening hemorrhage. The second ipsilateral aneurysm was plicated in order to exclude the aneurysm sac. In the next operation the ventricular septal defect was closed and vegetations located on the tricuspid valve were removed. On follow-up spontaneous t

[Role of asthenoneurotic syndrome in the clinical picture of unstable stenocardia].

With the purpose of ascertaining the impact the asthenoneurotic syndrome has on specificties of ischemic heart disease, 132 patients with unstable angina (UA) were examined. In 34.8 percent of cases, it was found to be accompanied by manifestations of somatophorme vegetative dysfunction. UA patients with objective evidence (transesophageal electrocardiostimulation--TEES, veloergometry--VE, stress-echocardiometry--EchoCG) of the asthenoneurotic syndrome display a higher polymorphism of complaints and subjective manifestations while single UA is accompanied by a lower level of frequency of the s

[Piracetam in combined pathogenetic therapy of recurrent duodenal ulcer].

Duodenal ulcer cure, as a systemic gastroenterologic disease, can be achieved in some patients by the addition of the nootropic drug piracetam to current antisecretory and antihelicobacter therapy. Piracetam corrects vegetative and psychoemotional disorders in duodenal ulcer, normalizes gastric motility, has an antioxidant effect and improves cerebral circulation. An optimal effect on clinico-endoscopic manifestations of recurrent duodenal ulcer was achieved in combination of piracetam with current antisecretory (omeprazole) and antihelicobacter (de-nol, amoxicillin, metronidazole) medicines.

Brucella glomerulonephritis: review of the literature and report on the first patient with brucellosis and mesangiocapillary glomerulonephritis.

Although mild proteinuria is commonly observed during the course of brucellosis, biopsy-proven glomerulonephritis (GN) is quite rare. We present the first case of mesangiocapillary glomerulonephritis (MCGN) associated with brucellosis and summarize all cases of Brucella GN published to date. Our patient, who had a congenital bicuspid aortic valve, also had heart failure, fever, urinary abnormalities and proteinuria. Renal biopsy revealed MCGN. Although the clinical features raised the possibility of GN associated with endocarditis, transesophageal echocardiography did not show any vegetations.

Paecilomyces fungus infection of the paranasal sinuses.

Fungal infections caused by Paecilomyces species are very rare and occur in adult patients with impaired host defences or following foreign body implants. They are found worldwide in soil and decaying vegetation. We describe a case of an 8 year old child who came with complaints of left sided nasal obstruction and discharge, telecanthus, diplopia and epiphora. On examination and investigation he had pansinusitis caused by P. lilacinus. An endoscopic sinus surgery was done and the patient was on oral Itraconazole for 6 months. To our knowledge, this is the first case reported in the paediatric

Subacute encephalopathy in a 5-year-old boy.

A 5-year-old boy presented with an acute ataxia and altered mental status. Although he initially recovered from these symptoms, he presented a second time with myoclonus and seizures and rapidly became vegetative. Cerebrospinal fluid studies, magnetic resonance imaging, and brain biopsy all confirmed the presence of subacute sclerosing panencephalitis. Despite courses of therapy with cimetidine, amantadine, ribavirin, and inosine, no clinical improvement has been seen. Clinicians need to be alert to the possibility of subacute sclerosing panencephalitis even in the vaccinated child in the appr

Pyodermatitis-pyostomatitis vegetans: a specific marker for inflammatory bowel disease.

In pyodermatitis-pyostomatitis vegetans annular pustular cutaneous lesions may precede, accompany, or follow the usually extensive vegetating oral disease. Sometimes only cutaneous or only oral lesions occur and previously have been described as separate entities. Clinical, histopathologic, and immunopathologic evidence clearly indicates this is one disease and suggests that it is distinct from pemphigus vegetans. The association between pyodermatitis-pyostomatitis vegetans and inflammatory bowel disease, most commonly ulcerative colitis, has been amply confirmed. Pyodermatitis-pyostomatitis v

Streptobacillus moniliformis endocarditis: case report and review.

A 46-year-old man with rat-bite fever due to Streptobacillus moniliformis that was complicated by endocarditis is reported. Other unusual features of this case include septic arthritis with prominent involvement of the sternoclavicular joint and the absence of a rash. Sixteen cases of streptobacillary endocarditis have previously been described. This condition occurs most often in the setting of previously damaged heart valves, usually as a result of rheumatic heart disease. Echocardiography demonstrated valvular vegetations in two of the four cases in which it was performed. Embolic phenomena

Small bowel phytobezoar mimicking presentation of Crohn's disease.

A case of small bowel phytobezoar formed from unusual ingested vegetation is described. The patient presented with recurrent subacute obstruction and a right iliac fossa mass mimicking the presentation of Crohn's disease. None of the usual gastrointestinal disorders that predispose to bezoar formation were present. The phytobezoar passed spontaneously following small bowel enema and colonoscopy. It is possible that relaxation of the gut secondary to the antispasmodics administered at investigation or the physical disturbance during these procedures enabled migration through the ileocecal valve

Injury from silage wagon accident complicated by mucormycosis.

Infection due to farm machinery injuries may be caused by microorganisms found in soil or decaying vegetable material. A case of injury due to entrapment of a young boy in a silage wagon is reported here. His injuries were complicated by infection with Aspergillus species, Absidia species, Rhizopus species (the latter two are members of the Mucorales order), and Pseudomonas maltophilia. Successful treatment of his infection followed aggressive surgical debridement of the anterior abdominal wall, amphotericin B, hyperbaric oxygen therapy, and surgical closure utilizing delayed placement of spli

[Hormone-active paragangliomas of the base of the skull].

Biochemical examination of 15 patients with paragangliomas (PG) of the base of the skull was conducted. The content of epinephrine, and norepinephrine, dopamine, and dopa in urine and the content of serotonin and histamine in blood and tumor tissue were studied. Comparison of the biochemical findings with the vegetative symptomatology allowed 3 groups of PG to be distinguished: catecholamine-secreting, serotonin-secreting, and hormone-inactive. The group of serotonin-secreting tumors of the base of the skull has not been described in the literature. The conducted research shows that tests for

[Status of the nervous system and bioelectric activity of the brain in dry cleaning plant workers exposed to tetrachloroethylene].

The studies covered 87 women and 44 men--workers of chemical laundries exposed to tetrachloroethylene (PER). About 50% of subjects were exposed, at the beginning of their employment, to trichloroethylene (TRI). In clinical examinations neuroses prevailed. The objective examination revealed disturbances of the vegetative system and symptoms of organic lesions of the central nervous system in form of encephalopathy. The EEG tests indicated paroxysmal lesions, usually with unaltered basic activities. Analysis of the rate of neurological lesions and EEG implied that the TRI-exposure at the beginni

Double valve disease in systemic lupus erythematosus. Case report and literature review.

A young, nonhypertensive female with advanced systemic lupus erythematosus (SLE) presented in congestive cardiac failure due to aortic and mitral regurgitation. The valvular lesions resulted from organization of valvular pocket Libman-Sacks vegetations. Her clinical course mimicked infective endocarditis. She is only the third recorded patient with SLE valvular disease warranting double valve replacement. This patient, who had her valvular disease at presentation (prior to initiation of steroid therapy), illustrates that untreated SLE per se may produce severe organic valvular disease.

An intranuclear microsporidium associated with acute anemia in the chinook salmon, Oncorhynchus tshawytscha.

An intranuclear microsporidium is described from hemoblastic cells of the chinook salmon, Oncorhynchus tshawytscha. The infection is associated with an acute anemia in the fish. Up to 47% of the hemoblast nuclei were infected in anemic fish. The organisms, found only in spleen and kidney tissues, were 1-2 microns in diameter and consisted of vegetative and early sporulation forms. This microsporidium differs from known species which parasitize fish in its tissue location; however, the absence of mature spores and other life cycle stages precludes determination of its precise taxonomic identity
