High altitude pulmonary hypertension (HAPH) is a chronic high-altitude disease caused by chronic hypoxia. Protecting right ventricular (RV) structure and function constitutes a key strategy for improving HAPH prognosis. Luteolin, a plant-derived flavonoid with cardiovascular protective properties, has been shown to reduce pulmonary artery pressure by improving vascular remodelling in HAPH. However, its direct effects on the right ventricle remain unclear. We investigated the effect of luteolin on RV structure/function in HAPH rats and its mechanism of action using hypertrophic H9c2 cardiomyocy

Background Right ventricular (RV) dysfunction is a prognostic factor for cardiovascular disease. However, its mechanism and pathophysiology remain unknown. We investigated RV function using RV-specific 3-dimensional (3D)-speckle-tracking echocardiography (STE) in patients with chronic thromboembolic pulmonary hypertension. We also assessed regional wall motion abnormalities in the RV and chronological changes during balloon pulmonary angioplasty (BPA). Methods and Results Twenty-nine patients with chronic thromboembolic pulmonary hypertension who underwent BPA were enrolled and underwent right

OBJECTIVE: Cardiac resynchronization therapy (CRT) with biventricular pacing has demonstrated cardiac function improvement in treating congestive heart failure. Traditional CRT through coronary sinus lead method is difficult to perform. Minimally invasive video-assisted epicardial lead (Epi-lead) CRT for the dilated cardiomyopathy heart failure cases was explored.
METHODS: From April 2007 to June 2009, a total of 12 patients (age, 63 ± 9 years) with depressed systolic left ventricular (LV) function (ejection fraction, <35%), left bundle branch block (mean QRS [Q wave, R wave, S wave], 158 ± 1

RATIONALE: Pulmonary arterial hypertension (PAH) is characterized by progressive narrowing of pulmonary arteries, resulting in right heart failure and death. BMPR2 (bone morphogenetic protein receptor type 2) mutations account for most familial PAH forms whereas reduced BMPR2 is present in many idiopathic PAH forms, suggesting dysfunctional BMPR2 signaling to be a key feature of PAH. Modulating BMPR2 signaling is therapeutically promising, yet how BMPR2 is downregulated in PAH is unclear.
OBJECTIVES: We intended to identify and pharmaceutically target BMPR2 modifier genes to improve PAH.

AIMS: Cyclic AMP phosphodiesterases (PDEs) are important modulators of the cardiac response to β-adrenergic receptor (β-AR) stimulation. PDE3 is classically considered as the major cardiac PDE in large mammals and human, while PDE4 is preponderant in rodents. However, it remains unclear whether PDE4 also plays a functional role in large mammals. Our purpose was to understand the role of PDE4 in cAMP hydrolysis and excitation-contraction coupling (ECC) in the pig heart, a relevant pre-clinical model.
METHODS AND RESULTS: Real-time cAMP variations were measured in isolated adult pig right ventr

BACKGROUND: Uhl's anomaly is an extremely rare congenital heart defect characterized by absence of the right ventricle myocardium and preserved left ventricular myocardium. Although the disease has a poor prognosis and is generally fatal in the perinatal period, some patients may reach adulthood.
METHODS: We describe a case of Uhl's anomaly complicated with heart failure and decompensated cardiac cirrhosis in a 42-year-old man treated by combined heart-liver transplant.
RESULTS: The patient underwent heart transplant using the bicaval technique followed by subsequent liver transplant with th

BACKGROUND: We sought to determine the prevalence of pulmonary complications and especially pulmonary arterial hypertension (PAH) in an Australian scleroderma population.
METHODS: Between July 2005 and June 2007, physicians in Western Australia were asked to refer patients with scleroderma specifically for pulmonary hypertension screening. All patients were assessed for PAH and other respiratory conditions using echocardiography, lung function testing and clinical assessments. Right heart catheterization was carried out in patients with evidence of increased right ventricular systolic pressur

To correlate clinical and laboratory variables in carcinoid heart disease, clinical data, echocardiograms, 24-hour urinary 5-hydroxyindoleacetic acid levels and liver function tests were evaluated in 30 patients with the carcinoid syndrome. The dominant cardiac lesion of carcinoid heart disease by echocardiography and Doppler was severe tricuspid regurgitation with right ventricular volume overload. A characteristic finding was thickened, retracted tricuspid valve leaflets that were fixed in a partially open position. Carcinoid heart disease was progressive and often fatal. The 17 patients wit

The presence of a pulmonary artery aneurysm, major aortopulmonary and coronary-pulmonary collateral vessels, and severe pulmonary hypertension in an adult with unilateral pulmonary artery agenesis and previous patent ductus arteriosus ligation is very rare. A 34-year-old man experienced these conditions. When he was 10 years old, catheterization and angiography revealed right pulmonary artery agenesis, dilation of the main pulmonary artery, multiple collateral vessels extending from the aorta to the right pulmonary system, and a patent ductus arteriosus (shunt ratio, 3.57) that was then ligate

BACKGROUND: Growing skull fracture (GSF), also known as post-traumatic leptomeningeal cyst, is a rare late complication of a linear skull fracture in young children. It requires an underlying dural tear with progressive herniation of arachnoid membrane and/or brain tissue.
CASE DESCRIPTION: We report a 15-month-old child who sustained a right frontoparietal linear skull fracture at 1 month of age and returned 14 months later with a pulsatile scalp swelling and left hemiparesis. Computed tomography showed a wide right frontoparietal diastasis and adjacent ventricular dilatation. At surgery, th

The impact of sectioning the jugular veins in the ritual slaughter of animals for food consumption has not been well-characterized. The purpose of this review is to evaluate the consequences of severing the jugular veins. In animal slaughter, when the jugular vein is transected, air travels antegrade into the right ventricle due to the powerful suction effect of right ventricular diastolic function and retrograde into the cerebral circulation, due to the natural buoyancy of the air bubbles. Air sucked into the heart induces hypotension because it creates an 'air lock' in the heart and elicits

Pulmonary hypertension (PH) patients with acute respiratory failure (ARF) face high morbidity and mortality rates. This study aimed to determine the optimal respiratory support strategy (high-flow nasal cannula [HFNC] vs noninvasive ventilation) across different grades of RV dysfunction severity in these patients. In this retrospective cohort study, we stratified 90 PH patients with ARF into mild, moderate or severe right ventricular (RV) dysfunction subgroups via propensity score matching. We compared 28-day mortality rates and intubation rates as primary outcomes between the HFNC and noninva

BACKGROUND: Persistent left superior vena cava (PLSVC) with absent right superior vena cava (RSVC) is a rare congenital venous anomaly, typically asymptomatic and discovered incidentally. In rare cases, it may contribute to hemodynamic disturbances and arrhythmias, potentially leading to heart failure, especially when associated with other cardiac anomalies or conduction abnormalities.
CASE PRESENTATION: We report the case of a 45-year-old male who presented with progressive symptoms of heart failure, including dyspnea, fatigue, and reduced exercise tolerance. Initial workup revealed an unexp

Chronic thromboembolic pulmonary hypertension (CTEPH) is one of the causes of pulmonary hypertension (PH) and requires invasive measurement of the mean pulmonary artery pressure (mPAP) during right heart catheterisation (RHC) for the diagnosis. 4D flow MRI could provide non-invasive parameters to estimate the mPAP. Twenty-five patients with suspected CTEPH underwent cardiac MRI. Mean vortex duration (%), pulmonary distensibility, right ventricular volumes and function were measured using 4D flow MRI and cine sequences, and compared with the mPAP measured by RHC. The mPAP measured during RHC wa

As patients on long-term left ventricular assist device (LVAD) face a substantial risk for open cardiac reoperation, interventional treatment approaches are becoming increasingly important in this population. We evaluated data of 871 patients who were on LVAD support between January 1, 2016 and December 1, 2020. Interventional treatments for LVAD-associated complications were performed in 76 patients. Seventeen patients underwent transcatheter aortic valve replacements (TAVR) and 61 patients underwent outflow graft interventions (OGI). TAVR improved symptoms in patients with severe symptomatic

We consider mitral valve disease requiring surgery in a patient with dextrocardia and situs inversus totalis to be an exceptional finding. The transseptal approach for mitral valve surgery in dextrocardia represents a technical challenge owing to its anatomic particulars. We present the case of a 56-year-old female patient who had been diagnosed with situs inversus totalis in childhood and with chronic atrial fibrillation in adulthood and was under oral anticoagulant treatment. She was referred to our hospital for increasing dyspnea and palpitation. Transthoracic echocardiography detected seve

OBJECTIVES: Arrhythmogenic right ventricular dysplasia (ARVD) is a rare, genetic disorder predominantly affecting the right ventricle. There is increasing evidence that in some cases, ARVD is due to mutations in genes, which have also been implicated in primary myopathies. This review gives an overview about myopathy-associated ARVD and how these patients can be managed.
METHODS: A literature review was done using appropriate search terms.
RESULTS: The myopathy, which is most frequently associated with ARVD, is the myofibrillar myopathy due to desmin mutations. Only in a single patient, ARVD

Pulmonary arterial hypertension (PAH) is characterised by excessive pulmonary vascular remodelling involving deregulated proliferation of cells in intima, media as well as adventitia. Pulmonary arterial endothelial cell (PAEC) hyperproliferation and survival underlies the endothelial pathobiology of the disease. The indispensable involvement of Notch1 in the arterial endothelial phenotype and angiogenesis provides intriguing prospects for its involvement in the pathogenesis of PAH.We observed an increased expression of Notch1 in lungs of idiopathic PAH (IPAH) patients and hypoxia/SU5416 (SUHx)

An 18-year-old woman taking an oral contraceptive was admitted to hospital because of a stroke due to occlusion of three branches of the right middle cerebral artery. She later developed renovascular hypertension due to occlusion of one of two renal arteries on the right side. Occlusion of the ceoliac artery was also found. The circumstances suggest that the occlusions were caused by multiple emboli, the source of which could not be identified. The kidney with the circulatory disturbance was shown to have a persistent abnormal renin secretion three and six months after the stroke, but the peri

Postoperative atrial fibrillation (POAF) is a common complication after cardiac surgery, increasing morbidity and healthcare costs. This study aimed to identify echocardiographic predictors of POAF to improve risk stratification. A total of 131 patients undergoing cardiac surgery were analyzed and divided into two groups based on POAF occurrence. Echocardiographic analysis showed that patients with POAF had larger left and right atrial dimensions and impaired atrial function. Prolonged total atrial conduction time (TACT), reduced atrial emptying volumes, and contractile function were more comm
