AIM: Sporotrichosis is a fungal zoonotic infection caused by the pathogenic clade of the Sporothrix species complex. Considered a neglected tropical disease, sporotrichosis now has a cosmopolitan distribution through species like Sporothrix globosa. Moreover, multi-drug-resistant S. globosa strains are refractory to conventional antifungal therapy. The present study explores the anti-sporotrichotic effects of the essential oil of Artemisia gmelinii leaves (AgLEO) against a multi-drug-resistant strain of S. globosa.

Candida albicans is an important human pathogenic yeast, that can become resistant to commonly used antifungal agents, such as azoles. One mechanism of drug resistance is efflux via ATP binding cassette transporters, such as Cdr1. Several studies have investigated the structural organization, binding mechanisms, function and regulation of Cdr1. This review summarizes the findings on the structure and function of Cdr1 and highlights important aspects to consider in future research relating to multidrug ABC transporters.

Sporotrichosis is the most common subcutaneous or implantation mycosis in Mexico. The case of a preauricular cutaneous-fixed sporotrichosis simulating atypical bacterial cellulitis is reported in an elderly patient with no history of trauma. The biopsy showed a suppurative granuloma with scarce yeast. Sporothrix schenckii was identified in the culture and confirmed by molecular biology. She was treated with itraconazole and a clinical and mycological cure was obtained. The case of atypical presentation is presented, coming from a semi-arid zone with extreme weather.

Many women seek care for vulvar, vaginal, or pelvic complaints. Primary care providers should possess a solid understanding of the differential diagnosis and treatment of gynecologic infections. Many infections in the reproductive tract are sexually transmitted, whereas other common infections are attributable to an overgrowth of the normally present bacteria or yeast in the vagina. Presenting symptoms and signs are helpful in determining the source of infection, but often a battery of tests must be performed to make a definitive diagnosis.

We retrospectively evaluated the epidemiology of onychomycosis and/or paronychia in 172 patients attending the Clinic of Dermatology and Venereology over a 5 year period. Although yeast isolates, belonging to the Candida species, represented the most frequent etiologic agents of these infections, an increasing prevalence of fungal infections due to emerging fungal pathogens (EFP) was noted throughout this time period. In particular, EFP as causative agents of these infections increased from 0 to 28.4% from 1998 to 2002.

Seborrheic dermatitis is a chronic inflammatory disease that mainly affects seborrheic areas of skin. An inflammatory response to the yeast Pityrosporum ovale has been thought to be important in the etiology of the condition. Therefore, topical antifungals and corticosteroids have been the mainstay of treatment. The recent development of topical macrolactam immunomodulators has offered a useful, safe alternative to corticosteroids in the treatment of various inflammatory skin disorders. We report successful treatment of seborrheic dermatitis with pimecrolimus.

There are 9 channels of the ClC family in mammals and few others in fishes, plants, yeast and bacteria. Several authors have suggested that african and north american forms of Blastomyces dermatitidis appeared separate taxa. Morphologic and pathogenic differences not only prevail, but also the failing to mate between them. African forms lose their mycelium-yeast conversion capability in old cultures. So I propose to create the complementary appellation Blastomyces dermatitidis tunisiens nov.subsp. from the african forms, in memory of the first case of systemic blastomycosis in this continent, discovered in Tunisia.

Yeast cells of Paracoccidioides brasiliensis can resemble the cysts of Pneumocystis carinii in smears stained with Grocott's modification of the Gomori methanamine silver stain. Furthermore, P. brasiliensis can cross-react in material stained with a widely used P. carinii immunofluorescent stain which uses monoclonal antibodies. The need to differentiate P. brasiliensis and P. carinii will become more important as the increasing incidence of immunosuppression results in the reactivation of latent P. brasiliensis infections.

We report a case of histoplasmosis capsulati endocarditis simultaneously infecting a mitral and aortic porcine bioprostheses. Histologically, the fungus demonstrated a diversity of morphologies, ranging from typical, intracellular, 2- to 5-micron yeast cells, to septate and branching hyphae, to bizarre hyphae with vesicular swellings. Cultures obtained from both valves were positive in four days. The combination of unexpected histologic appearance and rapid growth in culture may pose problems in diagnosis.

This is the first human infection caused by Hyphopichia burtonii, resulting in peritonitis in a patient on peritoneal dialysis initially diagnosed as sterile peritonitis, resulting in delayed diagnosis and treatment. This pathogen posed a challenging diagnosis, causing low-grade peritonitis and difficulty to culture with standard bacterial broth. Moreover, automated platforms for pathogenic yeast identification could not specify the species, but broad-range PCR targeting rDNA followed by DNA sequencing successfully solved the etiology.

Histoplasmosis is a rare disease in nonendemic areas. We report a case of a 23-year-old male patient who presented with fever of unknown origin, cytopenias, organomegaly, and allograft dysfunction 4 months after renal transplant with father as donor. Bone marrow examination showed intracellular budding yeast cells, which was confirmed as histoplasmosis by culture of bone marrow biopsy sample. The patient was treated with intravenous liposomal amphotericin and responded well.

This is a case report of feline sporotrichosis in a 3-year-old male intact DSH stray cat in Bangkok, Thailand. Cytology and histopathology revealed Sporothrix yeast-like organisms in ulcerative cutaneous lesions. Fungal culture and sequence analysis of ITS region of rDNA confirmed the diagnosis and the causative agent as Sporothrix schenckii. This is the first case report of feline sporotrichosis in the country. The case report emphasizes the role of stray cats as this zoonotic disease carrier.

Mucormycosis fungemia is rarely documented since blood cultures are nearly always negative. We describe a case of Mucor circinelloides fungemia in a patient with a history of a sinus infection, sarcoidosis, and IgG deficiency. The identity of the isolate was supported by its microscopic morphology and its ability to convert into yeast forms under anaerobic conditions. The early detection, initiation of liposomal amphotericin B treatment, and reversal of underlying predisposing risk factors resulted in a good outcome.
