Photocoagulation therapy for laryngeal dysplasia using angiolytic lasers.

In the management of laryngeal pre-cancerous lesions such as dysplasia or carcinoma in situ (CIS), it is important that lesion regression occur without any complications. As a minimally invasive treatment option, photocoagulation therapy using angiolytic lasers has been attracting attention. Therapeutic effects have been reported for this type of treatment, however, vocal function after treatment has not been well discussed. In this retrospective case series, we examined the therapeutic effects of photocoagulation therapy on laryngeal dysplasia and the impact on vocal function. Twenty-four pat

[Quiet Technology of MRI].

A number of clinical applications for MRI have been developed in accordance with the advancement in imaging technology. Recently, medical checkup of brain or heart are becoming popular. In this situation, there is a growing need for performance improvement of MRI and thus acoustic noise during examinations tends to become louder with the use of higher static magnetic field and higher gradient performance. Noise reduction measures for MRI are classified into two categories, pulse sequence optimization and hardware renovation. For pulse sequence, noise is reduced by decreasing the changes of the

Electrical Stimulation of the Ear, Head, Cranial Nerve, or Cortex for the Treatment of Tinnitus: A Scoping Review.

Tinnitus is defined as the perception of sound in the absence of an external source. It is often associated with hearing loss and is thought to result from abnormal neural activity at some point or points in the auditory pathway, which is incorrectly interpreted by the brain as an actual sound. Neurostimulation therapies therefore, which interfere on some level with that abnormal activity, are a logical approach to treatment. For tinnitus, where the pathological neuronal activity might be associated with auditory and other areas of the brain, interventions using electromagnetic, electrical, or

Increased endothelial progenitor cell circulation and VEGF production in a rat model of noise-induced hearing loss.

CONCLUSIONS: The vascular endothelial growth factor (VEGF)-mediated mechanism of endothelial progenitor cell (EPC) mobilization, migration, and differentiation may occur in response to noise-induced acoustic trauma of the cochlea, leading to the protection of cochlear function. OBJECTIVE: The purpose of this study was to analyze changes in the cochlear vessel under an intensive noise environment. METHODS: Sixty male Sprague-Dawley rats were randomly divided into six groups. Acoustic trauma was induced by 120 dB SPL white noise for 4 h. Auditory function was evaluated by the auditory brainste

Loudness of time-varying stimuli with electric stimulation.

McKay, Henshall, Farrell, and McDermott [J. Acoust. Soc. Am. 113, 2054-2063 (2003)] developed a practical method to estimate the loudness of periodic electrical signals presented through a cochlear implant. In the present work, this method was extended to time-varying sounds based on two models of time-varying loudness for normal listeners. To fit the model parameters, loudness balancing data was collected with six cochlear implant listeners. The pulse rate of a modulated pulse train was adjusted to equalize its loudness to a reference stimulus. The stimuli were single-electrode time-limited p

The atretic plate--a conduit for drill vibration to the inner ear?

CONCLUSION: In the presence of simulated congenital aural atresia with fixation of the ossicular chain to the surrounding bone, access to the chain for placement of a middle ear prosthesis using the rotating burr is not associated with significant stapes movement that would cause vibrational trauma to the cochlea. OBJECTIVES: To determine the energy transmitted to the cochlea while drilling the mastoid in an ear with simulated congenital aural atresia and fixation of the ossicular chain to the surrounding bone. METHODS: Eight human cadaveric temporal bones were used. Cement was placed in the

Clinical anatomy of the auriculotemporal nerve in the area of the infratemporal fossa.

The auriculotemporal nerve is a sensory branch extending from the posterior section of the mandibular nerve trunk. Its nerve roots form a short trunk, which gives off a number of branches, innervating: the temporomandibular joint, the temporal region, structures of the external ear: auricle, and external acoustic meatus, and the parotid gland. It also conducts excretory fibres to the buccal and labial glands. Anatomical relationships between the auriculotemporal nerve and the muscles of mastication, temporomandibular joint, and surrounding vessels in the area of the infratemporal fossa create

Treatment of vocal fold scar by carbon dioxide laser and collagen injection: retrospective study on 12 patients.

Vocal fold scars are the result of injury to the vocal fold lamina propria. This condition leads to an impaired vibration and usually to poor voice quality. The purpose of our study was to compare the pre- and posttreatment voice assessment scores in patients treated by CO(2) laser-assisted freeing of the scar followed by collagen injection. A group of 12 patients (10 women and 2 men) with vocal fold scars was studied retrospectively. Voice assessment was based on stroboscopy and on perceptual scores using the Grade (G) of the GRABS scale, subjective evaluation by the use of the Voice Handicap

Elastography, progression factor in liver ultrasound.

Elastography is a method which, using ultrasound, obtains images and measurements of tissue elasticity when applying a force on it. As an imaging method it was developed to quantify objectively the pathological changes related to the presence of an abnormal tissue, compared to the surrounding tissues, giving information about the elasticity/stiffness of the examined tissue, the degree of fibrosis, the degree of stiffness compared to tumor free tissue. The tissue analysis can be done through a compression technology "eSie touch elasticity imaging" (with applications for the surface elastography

Preparation and performance of obstacle steps: interaction between brain and spinal neuronal activity.

This study investigated the interactions of supraspinal with spinal neuronal circuits during obstacle steps by recordings of electroencephalography (EEG), reflex activity and limb muscle electromyography (EMG). Subjects walking with reduced vision on a treadmill were acoustically informed about an approaching obstacle and received feedback about task performance. Only following a task-relevant acoustic signal, spinal reflex responses, evoked by tibial nerve stimulation during mid-stance, were enhanced in proximal arm and leg flexor muscles prior to obstacle compared to normal swing, reflecting

A computational study of the effect of false vocal folds on glottal flow and vocal fold vibration during phonation.

The false vocal folds are believed to be components of the acoustic filter that is responsible for shaping the voice. However, the effects of false vocal folds on the vocal fold vibration and the glottal aerodynamic during phonation remain unclear. This effect has implications for computational modeling of phonation as well as for understanding laryngeal pathologies such as glottal incompetence resulting from unilateral vocal fold paralysis. In this study, a high fidelity, two-dimensional computational model, which combines an immersed boundary method for the airflow and a continuum, finite-el

Neurootological differentiations in endogenous tinnitus.

Vertigo and tinnitus are very frequent complaints. Often, we find multisensory syndromes combined with tinnitus, hearing impairment, vertigo, and nausea. From more than 10,000 cases, we evaluated 757 randomly selected neurootological patients suffering from endogenous tinnitus. First, we classified the 10,000 patients into those suffering from the basic tetrad of tinnitus forms: bruits, endogenous (maskable) tinnitus, exogenous (nonmaskable) tinnitus, and other syndromes such as the slow brainstem syndrome. Then, of all the endogenous tinnitus patients, we randomly selected our study sample (n

Selective activation of nuclear factor kappa B in the cochlea by sensory and inflammatory stress.

Damage response pathways triggered by mechanical stress might reasonably be expected to be conserved throughout evolution. However, using a nuclear factor kappa B (NF-kappaB) reporter mouse we show here that this phylogenetically recent transcription factor plays a major role in the response to mechanosensory stress in the mammalian inner ear. The protective action of NF-kappaB is exerted in neither sensory nor non-sensory epithelial cells, but rather in connective tissue cells within the spiral ligament and spiral limbus. In the spiral ligament, predominantly type I fibrocytes are activated f

Age impact on the efferent system activities in cochlear mechanical properties in normal hearing individuals.

UNLABELLED: The medial olivocochlear tract has efferent control over the outer hair cells, regulating the slow contractions and damping the fast ones. Using ipsilateral, contralateral or bilateral otoacoustic emissions amplitude studies, it is possible to estimate the conditions of this tract, since the effect resulting from the reduction/suppression of these emissions indicate the tract's functioning. Aging implies an activity reduction in the central auditory system, because of the degeneration of the structures involved in hearing skills. AIM: our goal was to investigate the effects of age

On correlating otoacoustic emissions with blood glucose levels.

The long term objective of this research is to develop a new means for diabetic patients to painlessly and non-invasively monitor their blood glucose levels. We propose a novel method for noninvasive glucose monitoring based on measurement and analysis of otoacoustic emissions (OAEs). OAEs are low-intensity sounds generated by the cochlea in response to acoustic stimuli. Evoking and measuring OAEs is done using a tiny speaker and microphone that fit snugly inside the ear canal. The OAE response can be partially masked or reduced in amplitude by presenting competing acoustic stimuli contralater

Observations on mastoid versus ear canal recorded cochlear microphonic in newborns and adults.

The cochlear microphonic (CM) may play an important role in the diagnosis of auditory neuropathy (AN) in newborns. However, since the CM tends to mirror the waveform of the acoustic stimulus, conscientious recording methodology must be applied to separate true response from artifact. The difficulty in achieving this separation has limited the clinical usefulness of the CM. In an effort to call attention to the importance of recording protocol when measuring the CM, the present study was designed to optimize CM recordings in humans by investigating the following parameters: (1) secondary minus
