[Is remodeling still possible after collapse in femur head necrosis?].

A case of a femoral head necrosis, with collapsed joint surface after fracture of the femoral neck will be presented. Following varisation osteotomy, the necrotic parts of the bone was removed and filled with cancellous bone. A standardized bone block of the iliac crest was implanted into the hole. After the operation the patient was painfree. The X-rays showed the improvement of the bony structure and later a spherical remodeling of the femoral head. This result is in opposition with the earlier opinion of the disability of femoral head remodeling in adults. It is technically very important t

Posterior stabilization of the cervical spine.

Operative management of the patient with cervical spinal instability depends on the specific anatomy and location of the injury. The unique features of the atlanto-axial complex require stabilizing procedures that are quite different from those used in the subaxial spine. A clear idea of the etiology of the instability, a definition of the pathological anatomy, and knowledge of the presence and extent of neural compression are essential for optimizing a successful outcome. In some instances the clinical circumstances mandate the use of an anterior approach, and in others a posterior operation

A review of tibial lengthening procedures.

Eight patients had tibial lengthening procedures according to Anderson's method. The necessity of early determination of callus formation and bone grafting, if necessary, is emphasized. The potential morbidity in this procedure is great and the parents must be made fully aware of them. As Anderson states, it is a procedure that should be reserved for the child between 8 and 12 years old with a predictable discrepancy between 4 and 15 cm. We believe it is a useful procedure in problems of unequal length of the lower limbs but should not be used whem simpler procedures such as femoral shortening

Mimicking bone remodeling scaffolds of polyvinylalcohol/silk fibroin with phytoactive compound of soy protein isolate as surgical supporting biomaterials for tissue formation at defect area in osteoporosis; characterization, morphology, andin-vitrotesting.

Mimicking bone remodeling scaffolds were developed as supportive biomaterials to promote tissue formation at defect sites in osteoporosis. Scaffolds made of polyvinyl alcohol (PVA) were mixed with varying weight ratios of silk fibroin (SF) and a phytoactive compound-based soy protein isolate (SPI); PVA30SF, PVA20SF10SPI, PVA15SF15SPI, PVA10SF20SPI, PVA30SPI. PVA was used as control. These components were mixed into aqueous solution and crosslinking with EDC before freeze thawing and freeze drying, respectively. Then, the scaffolds were characterized at the molecular level using Fourier transfo

Plate Augmentation in Aseptic Femoral Shaft Nonunion after Intramedullary Nailing: A Literature Review.

BACKGROUND: Femoral shaft fractures (FSFs) are a frequent injury in traumatology for which intramedullary nailing (IMN) is considered the gold standard treatment. Nonunion (NU) is one of the most frequent complications in FSF treated with IMN, with a percentage from 1.1% to 14%. Plate augmentation (PA), the addition of a compression plate and screws, with or without bone graft has been described as an effective option for the treatment of NU, improving the biomechanical conditions at the fracture site. The aim of this review was to analyze the literature relating to the use of PA in NU after I

The Management of Aseptic Non-unions of Distal Femur Fractures with Anatomical Lateral Locking Plates.

BACKGROUND: Distal femoral non-unions are challenging, and frequently associated with short distal fragments, poor bone stock, and with issues from previous implants. MATERIALS AND METHODS: A retrospective study of 31 patients admitted with distal femoral non-unions treated using anatomical lateral locking plates. Non-union scores were used. The Knee Society and Neer's scores were used for the comparison of results. The mean follow-up was 39.5 months (from 24 months to 60 months). RESULTS: Stable union was accomplished in all. There was a significant improvement in the average Neer's score (

Clinical outcomes of doubled-suture Nice knot augmented plate fixation in the treatment of comminuted midshaft clavicle fracture.

BACKGROUND: Free bone fragments were difficult to be fixed in many comminuted midshaft clavicle fractures, and the absence of cortical alignment in comminuted fractures had direct influence on the stability of fixation. This survey was performed to assess the efficacy of doubled-suture Nice knot augmented plate fixation in the treatment of comminuted midshaft clavicle fractures. METHODS: Between 2013 and 2018, all patients with comminuted midshaft clavicle fractures treated with doubled-suture Nice knot augmented plate fixation were retrospectively reviewed and included in this research. A biography of the point contact fixator (PC-Fix).

Starting in the mid-eighties, the AO (from the German "Arbeitsgemeinschaft für Osteosynthesefragen") Research Institute (ARI), Davos, Switzerland together with the commercial partners of the AO Foundation embarked on a decade-long project to design, develop, test in experimental animals and human clinical trials as well as bring to clinical use a new system for surgical osteosynthesis. The new plating system, what became known as the Point Contact Fixator (PC-Fix), addressed the shortcomings of the conventional plating by Dynamic Compression Plate (DCP) discovered either by careful examination

Arthroscopic Latarjet: Indications, Techniques, and Results.

The Latarjet procedure to treat instability was first described by the eponymous surgeon in 1954. Long-term results from this procedure have been favorable. In 2007, Lafosse et al. first described an all-arthroscopic technique for the Latarjet procedure. In the United States, the Latarjet procedure is most predictably indicated by surgeons in cases of significant glenoid bone loss, revision instability, and patients engaging in high-risk sport. In some European centers, the Latarjet has broader indications and is often also used as a first-line surgical intervention when conservative treatment

Radial Head and Neck Allograft for Comminute Irreparable Fracture-Dislocations of the Elbow.

Fracture-dislocations of the elbow can be difficult to treat, with unsatisfactory results in some cases. In general, it is preferable to preserve the fractured radial head when possible, but some patients present a unique treatment challenge because of extremely comminuted fractures and bone loss. In these cases, the only options available are radial head prosthesis or allograft. The authors present a case of a 45-year-old man with a fracture-dislocation of the left elbow that was treated with an allograft of the radial head and neck because of extreme comminution of the fracture. There have b

Outcomes of suture button repair of the distal tibiofibular syndesmosis.

BACKGROUND: Recently, a suture button device has been advocated as a simple and effective method of repairing the syndesmosis. Proponents of the device have cited earlier weightbearing and elimination of the need for device removal as potential advantages over metallic screws. However, the available reports generally have short followup. With longer followup, some concerns about the suture button device have surfaced. MATERIALS AND METHODS: We reviewed the clinical and radiographic results of 24 patients with acute injuries to the distal tibiofibular syndesmosis who were treated with suture b

Preliminary experience with biodegradable implants for fracture fixation.

BACKGROUND: Biodegradable implants were designed to overcome the disadvantages of metal-based internal fixation devices. Although they have been in use for four decades internationally, many surgeons in India continue to be skeptical about the mechanical strength of biodegradable implants, hence this study. MATERIALS AND METHODS: A prospective study was done to assess the feasibility and surgeon confidence level with biodegradable implants over a 12-month period in an Indian hospital. Fifteen fractures (intra-articular, metaphyseal or small bone fractures) were fixed with biodegradable implan

Horizontal ridge augmentation using autogenous block grafts and the guided bone regeneration technique with collagen membranes: a clinical study with 42 patients.

OBJECTIVE: To analyze the clinical outcome of horizontal ridge augmentation using autogenous block grafts covered with an organic bovine bone mineral (ABBM) and a bioabsorbable collagen membrane. MATERIAL AND METHODS: In 42 patients with severe horizontal bone atrophy, a staged approach was chosen for implant placement following horizontal ridge augmentation. A block graft was harvested from the symphysis or retromolar area, and secured to the recipient site with fixation screws. The width of the ridge was measured before and after horizontal ridge augmentation. The block graft was subsequent

Asymptomatic iatrogenic bilateral occlusion of vertebral artery after atlantoaxial fusion: a case report.

Vertebral arterial injury (VAI) remains a fatal complication of C1-C2 posterior screw fixation. Herein, we report asymptomatic bilateral VAI that was caused by screws following C1-C2 posterior fixation. A 34-year-old woman with cerebral palsy experienced quadriplegia after a fall. Cervical computed tomography (CT) showed increased ADI, with os odontoideum, for which C1 pedicle screw and C2 pedicle-lamina screw fixation were performed. Cervical magnetic resonance imaging (MRI) conducted for postoperative weakness in shoulder elevation demonstrated a well-decompressed spinal cord. However, neck
