Jejunal diverticulosis is a rare entity and jejunal diverticulitis is even rarer. Here we review a patient who presented with right upper quadrant pain and elevated white blood cell count, mimicking acute cholecystitis, as well as thrombocytopenia (21x10^3/mcl). The patient was managed non-operatively with antibiotics and had improvement in symptoms and thrombocytopenia. Our study highlights the importance of maintaining jejunal diverticulitis in the differential for right upper quadrant pain, and its association with severe thrombocytopenia.

Pneumocystis pneumonia (PCP) is a fungal infection with primary occurrence in immunocompromised individuals by the etiologic agent Pneumocystis jirovecii. In those without previously identified risk factors, maintaining an index of suspicion for this diagnosis with search for underlying immunosuppressive cause is of utmost importance. Here, we present a case of fatal PCP in a patient without a known history of immunocompromise subsequently diagnosed with idiopathic CD4 lymphocytopenia (ICL) with an absolute CD4+ T-cell count of 7 cells/uL.

Gaisböck's syndrome, first described by Félix Gaisböck in 1905, refers to the association of high blood pressure with an elevated hematocrit level in male patients, with a normal white blood cell count and in the absence of splenomegaly. We report the case of a young diabetic and smoking male who presented with severe hypertension and polycythemia. After ruling out other causes, a diagnosis of Gaisböck's syndrome was made. Antihypertensive treatment and lifestyle changes normalized his parameters.

A 24-year-old Saudi man presented with progressive lower-back pain for 3 months followed by lower-limb weakness, numbness, and urinary retention. Cerebrospinal fluid examination revealed high-protein, elevated white blood cell count with lymphocyte predominance. MRI scan of the spine demonstrated heterogeneously enhancing longitudinal spinal cord lesion from T6 through T9. FDG-PET/CT demonstrated prominently hypermetabolic lesion in the spinal cord. He had laminectomy and spinal cord (intramedullary) biopsy. Pathology revealed Schistosoma mansoni ova.

A 20-year-old man presented with 1 week of low back pain and progressive lower extremity weakness. Results of cerebrospinal fluid analysis demonstrated elevated total protein and a mildly elevated white blood cell count with lymphocytic predominance. Findings from imaging studies revealed a multifocal, heterogeneously enhancing, intramedullary lesion involving the cervicothoracic spinal cord and nodular enhancement of the cauda equina. The patient eventually underwent spinal surgery for tissue diagnosis. The differential diagnosis, pathologic findings, and diagnosis are discussed.
